The natural history of carcinoma of the cervix in young women.

Invasive carcinoma of the cervix was treated in 1,522 patients over a 10-year period at Groote Schuur Hospital. Data for each patient included place of residence, pathological characteristics of the tumour, nodal spread, histological evidence of human papillomavirus (HPV) infection, disease stage, treatment type, treatment complications, site of recurrence and survival time. Patients aged under 35 years comprised 11.6% of the total patient group. A retrospective cohort study compared women aged under 35 years of age at the time of entry into the study and women aged 35 years or more. In all, 82 eligible young patients were compared with random sample of 82 eligible older patients. Contingency tables, life tables and proportional hazards analysis were used where appropriate. There was no significant difference across the two age groups in the incidence of non-squamous tumours, or in the grades of squamous tumours. Similarly there was no evidence of differences in lymph node involvement or HPV infection. Young patients appeared to enter the study at significantly earlier stages of the disease, and a greater proportion of them underwent surgical treatment (diff = 36.4% +/- 13.5%). In both groups treatment complications were less frequently observed after surgery than after radiotherapy. Sites of tumour recurrence were similar in both groups, and there was no apparent difference in survival times.

Non-neoplastic ovarian cysts with ectopic pregnancy.

Thirty non-neoplastic ovarian cysts, discovered at laparotomy in association with 27 cases of tubal ectopic pregnancy, were examined to document changes seen in early pregnancy and possibly to identify precursor lesions to the luteinized cysts of later pregnancy. The ages of the patients ranged from 16 to 40 years, and length of gestation varied between 5 and 11 weeks. Of the 30 cysts, 12 were corpus luteum cysts; 10 were called lutein cysts, nonspecific, and had a lining of luteinized cells of a single cell type; three cysts were luteinized follicles; four cysts were unlined or lined by a single layer of cells. The origin of these four was not obvious, and they were called "simple cysts." The last was a granulosa-theca lutein cyst. Corpus luteum cysts are assumed to arise from the corpus luteum, and simple cysts appear to arise from both corpus luteum cysts and other follicular structures. The nonspecific lutein cysts appeared to arise from structures other than the corpus luteum or corpus luteum cysts; these nonspecific lutein cysts may be precursors to the so-called large solitary luteinized follicle cyst of pregnancy.

DNA ploidy in papillary tumours of the breast.

The DNA content of benign and malignant papillary tumours of the breast as well as a group of papillary tumours where this distinction could not be made with certainty was measured by flow cytometry. All the benign papillomas were diploid; 5 of 19 carcinomas were aneuploid; and 1 of 15 tumours of undecided type was also aneuploid. Comparison of the DNA indices, proliferation indices and coefficients of variation were of no value in distinguishing between benign and malignant specimens.

Brenner tumours--a clinicopathological study.

In a clinicopathological study of 70 Brenner tumours in 61 patients seen at Groote Schuur Hospital during the 30-year period 1956-1985, 68 were benign, 1 malignant and 1 proliferating. Fifty-seven were pure tumours and the remainder were mixed with other ovarian neoplasms. The age range of patients was 26-71 years (median 49 years). Most of the Brenner tumours were incidental findings and probably not responsible for the symptoms or signs, which were most commonly a pelvic mass (28 patients) and abnormal vaginal bleeding (15).

The effect of chemotherapy on the morphology of human breast carcinoma.

Between biopsy diagnosis and mastectomy 16 patients with carcinoma of the breast received at least three courses of chemotherapy. When the biopsies and the tumour in the mastectomy specimens were compared, an increase in nuclear size, pleomorphism, vacuolation/chromatin clumping and nucleolar prominence were seen in 14 cases. Cytoplasmic changes included accumulation of lipid. There was no correlation between these morphological changes and the disease-free survival period and they could not be used to predict response to chemotherapy.

Paratesticular rhabdomyosarcoma. Case reports.

During the 30-year period 1955-1984, 9 cases of paratesticular rhabdomyosarcoma were seen in the pathology departments of the University of Cape Town, including the Red Cross War Memorial Children's Hospital. Three of these patients were lost to follow-up; 2 were alive at 6 months and 6 years after presentation. Two patients absconded with clinical disease and are presumed dead and 2 died of disease 6 and 18 months after presentation. Although rhabdomyosarcoma is known to be a disease of the young, none of the patients in this series was in the first decade of life and only 3 were below 20 years of age.

Ependymal cyst of the ovary. A case report.

An unusual morphological variant of a mature cystic teratoma was found at caesarean section in a 23-year-old patient; it appeared as a thin-walled cyst containing clear fluid. Microscopy showed the cyst to be lined by ependyma and the wall to be composed of glial tissue with a small protuberance containing squamous epithelium, adipose tissue and bone.

The effect of progestins on the mitotic activity of uterine fibromyomas.

Previous studies have shown increased mitotic activity in uterine fibromyomas in patients using exogenous hormones. This study has compared the mitotic activity of fibromyomas in three groups of patients. The first group was using a progestin-only preparation (usually depo-medroxyprogesterone acetate). The second group of age-matched controls had never used any exogenous hormone. The third group of patients was using a combined estrogen/progestin oral contraceptive. The results show that patients using a progestin-only preparation have a significantly higher mitotic activity in fibromyomas than patients from the other two groups; results from patients using a combined estrogen/progestin preparation were the same as the controls. The highest count was 39 mitoses/100 high power fields (HPF), but focal areas containing up to 8 mitoses/10 HPF could be found. In none of the fibromyomas examined did the count exceed 10 mitoses/10 HPF, and none showed significant pleomorphism that might have been confused with malignancy.

Sclerosing stromal tumor of the ovary: demonstration of ligandin in three cases.

The clinical and pathological features of three cases of sclerosing stromal tumors are presented. Two of these were discovered during pregnancy. Two occurred as unilocular cysts. Ligandin, a probable indication of steroidogenesis in the gonad, was demonstrated by immunohistochemistry in all cases, supporting the suggestion that these tumors have an endocrine potential.

Massive oedema of the ovary. Case reports.

Six cases of massive oedema of the ovary are presented. This is an uncommon condition occurring in young women, and is thought to result from periodic torsion. The patients most commonly present with lower abdominal pain, but there may be endocrine effects which are probably related to luteinization of stromal cells within the oedematous ovary. Although the reported treatment has been oophorectomy, since the lesion is benign conservative therapy should be considered.

Ligandin in the human ovary.

Human ovaries from different times during the menstrual cycle, pregnancy and the post-menopausal state were examined by immunohistochemistry using an antibody to ligandin. The results showed that antiligandin was localized to those cells producing steroids and probably acts as an intracellular transport protein but may also have an enzyme function in steroidogenesis. In demonstrating this relationship this study has indicated that antiligandin may be of value in the morphologic investigation of ovaries in conditions where there are assumed alterations in steroid production.

Corpus Luteum↗

Angiosarcoma of the breast.

We report a case of the rare angiosarcoma of the breast, illustrating the not infrequent error of making the initial incorrect diagnosis of benign haemangioma and the subsequent relentless progression of the lesion despite mastectomy, external beam and interstitial radiotherapy, and chemotherapy.

Verrucous carcinoma of the uterine cervix. Case reports.

Three cases of verrucous carcinoma of the uterine cervix are presented and the literature on this lesion at this site is reviewed. This is a slow-growing tumour, but in order for adequate therapy to be instituted early diagnosis is necessary. Because of the relatively bland microscopic appearance such early diagnosis requires examination of an adequate biopsy specimen and good clinicopathological correlation.

Ovarian neoplasms in the Western Cape.

An analysis of ovarian neoplasms seen at Groote Schuur Hospital during the 10-year period 1966-1975 shows a relatively high incidence of mature cystic teratomas when compared with European and North American experience. The cause is an apparent increased incidence of these tumours among Coloured and Black patients. The reasons for this increase are not apparent.

Adolescent↗

Verrucous carcinoma of the cervix with endometrial involvement.

A verrucous carcinoma of the uterine cervix in a 42-year-old patient is presented that showed superficial spread to involve the whole of the endometrial cavity. Despite widespread growth, there was no alteration in the menstrual cycle and only superficial stromal invasion.
