Children's comprehension of time-compressed speech: effect of speaker's familiarity.

An experiment was conducted with 42 third grade children from a parochial school system in a large metropolitan area. 21 classmates whose teacher served as the speaker for this experiment comprised the experimental group. A matched group of 21 classmates from another third grade classroom formed the control group. Each subject individually listened to a 940-word narrative passage that had been compressed at a rate of 45% or 275 words per minute. 10 questions about the material presented in the final three-fourths of the passage were asked of the subjects. The difference between the mean number of correct responses produced by the experimental group (5.5) and the control group (4.9) was not statistically significant. The results indicate that familiarity with the speaker does not facilitate 8-yr.-olds' comprehension of time-compressed speech.

IgA nephropathy: a syndrome of uniform morphology, diverse clinical features and uncertain prognosis.

Isolated glomerulonephritis with mesangial IgA deposits was the most common single finding encountered in a large biopsy series in an Australian community and was found in 50 patients, 18% of those presenting for the investigation of primary glomerular disease. A uniform histopathological picture of mesangial enlargement, with or without focal and segmental features was present. A tendency to progression was suggested by the frequent presence of glomerulosclerosis, interstitial scarring and vascular hyalinization. Because of the uniform histological immunofluorescence and ultrastructural appearances, the term IgA nephropathy has been used for this condition. The clinical picture, however, was heterogenous. Presenting symptoms included macroscopic hematuria (34%), proteinuria (32%) acute nephritis (10%), nephrotic syndrome (6%), malignant hypertension (8%), acute renal failure (6%) and chronic renal failure (4%). There was a striking correlation of increased blood pressure and decreased renal function with increasing age. While the period of follow-up was too short to assess individual patient data, a gradual and progressive decrease in renal function over several decades is suspected in patients with this condition.

Morphological and immunological evidence of coagulopathy in renal complications of pregnancy.

Renal biopsies in 14 patients with P.E.T. or eclampsia showed constant I.F. reactions for IgM and fibrin, with frequent reactions for C1q and C3. The glomeruli showed reversible mesangial proliferation and swelling, with characterictic E.M. deposits, and segmental lesions were present in seven patients. Similar I.F. reactions occurred in three other patients with clinical diagnoses of P.E.T. whose biopsies demonstrated coexistent glomerular disease. Serum complement studies showed a significant rise in C3 in the third trimester of normal pregnancies and a further significant elevation in C1q and C3 in the third trimester of a series of unselected P.E.T. patients. In contrast, four patients from the biopsy series with eclampsia or severe P.E.T. showed profound depression of serum C3 and C4, at the time of maximum clinical severity, which was shown to return to normal in two patients. The I.F. findings confirm those of Petrucco et al (6), and, with the other data, suggest that immune-complex deposition and activation of the classical complement pathway could interrect with intravascular coagulation to produce the glomerular lesions of P.E.T. and eclampsia.

Prediction of maximal oxygen consumption. Comparison of the Bruce and Balke treadmill protocols.

This study investigated the hypothesis that an individual's maximal oxygen consumption can be realistically predicted by the maximal time achieved in the Balke or Bruce treadmill protocols. The range of maximal oxygen consumption that can be expected for healthy individuals of any given age and activity was also evaluated. The maximal oxygen consumptions achieved by 79 men exercised using the Balke protocol and 77 men using the Bruce protocol were linearly regressed by a least-squares fit technique on maximal treadmill time and on age with activity status classified. Statistical analysis demonstrated an inadequate relationship for predicting maximal oxygen consumption from maximal treadmill time using either protocol. Also, maximal oxygen consumption correlated poorly with age even though activity status was considered. These findings make the nomogram for predicting an individual's functional aerobic impairment a clinical technique of questionable value. Since maximal oxygen consumption can only be grossly estimated from the maximal time performed in the Bruce or Balke protocols, there is no necessity to use them in preference to other clinically acceptable protocols.

Normal coronary angiography in an aircrewman with serial exercise test changes.

A 45-year-old asymptomatic aircrewman is presented who developed an abnormal ST segment response to maximal treadmill testing after normal responses for 3 previous years. Coronary angiography performed after his abnormal stress test reveled no change from a previously normal study. It has been demostrated that serially performing exercise tests can increase the sensitivity of detecting latent coronary artery disease. The importance of the case report is to demonstrate that the change from a normal exercise test to an abnormal one does not absolutely indicate that an individual has a coronary artery disease and that it is mandatory to search for other possible causes. Further studies will be required to determine the significance of such serial changes.

The nature and properties of the inducible sodium-plus-potassium ion-dependent adenosine triphosphatase in the gills of eels (Anguilla anguilla) adapted to fresh water and sea water.

1. Gill tissue from eels adapted to fresh water or to sea water was disrupted in 0.32m-sucrose containing 0.1% (w/v) sodium deoxycholate and the subcellular distribution of (Na(+)+K(+))-dependent adenosine triphosphatase was determined. 2. About 70% of the recovered enzyme was in a fraction sedimenting between 225000g(av.)-min and 6000000g(av.)-min; the specific activities of enzymes from tissues of freshwater and seawater eels were 16 and 51 mumol of phosphate/h per mg of protein respectively. 3. The enzymes from gills of freshwater and seawater eels were indistinguishable on the basis of a number of parameters. These included phosphorylation by [gamma-(32)P]ATP, the binding of [(3)H]ouabain, the extent to which bound [(3)H]ouabain was displaced by increasing concentrations of KCl and pH optima. 4. Electrophoresis on polyacrylamide gels in sodium dodecyl sulphate showed that enzyme preparations from both sources had an identical number of protein components. 5. The higher specific activity of (Na(+)+K(+))-dependent adenosine triphosphatase from tissue of seawater eels was accompanied by increased amounts of two protein components. One of these proteins retained (32)P after treatment of the enzyme with [gamma-(32)P]ATP and had mol.wt. 97000; the other component was a glycoprotein with mol.wt. approx. 46000. 6. The results are discussed in terms of the nature of the transepithelial NaCl pumps in the gills of freshwater and seawater fish.

Macroscopic and microscopic assessments of disease burden by MRI in multiple sclerosis: relationship to clinical parameters.

We have evaluated macroscopic white matter abnormalities (visible lesions) together with microscopic abnormalities in the normal appearing white matter (NAWM) of patients with multiple sclerosis (MS) to determine their relative contributions to the development of disability. The total visible lesion volume (TLV) was computed as a measure macroscopic changes, whereas both texture analysis and T2 were used as possible indicators of diffuse disease in the NAWM. Dual echo T2-weighted SE images were obtained from 41 patients with definite MS: 10 primary progressive (PP), 11 secondary progressive (SP), 10 benign (BE), 10 early relapsing remitting (ERR), as well as from 10 healthy controls. Calculation of T2 and texture parameters were performed in a region of frontal NAWM of patients and controls. The TLV of each patient was measured using a semiautomated lesion detection program. No significant differences were found between the controls and the patients for all texture parameters examined. However, NAWM T2 was longer in the patients than in the controls (P = .02). Mean TLV was highest for SP and lowest for BE and ERR patients. A significant correlation was found between TLV and EDSS (P < .01) but not between NAWM T2 or texture and expanded disability status score (EDSS). Our study suggest that: (a) diffuse changes are present in NAWM, (b) texture analysis is unable to detect any subtle structure in the NAWM abnormalities, possibly because of the limited image resolution; (c) in the development of disability in MS, macroscopic lesions are more important than microscopic abnormalities in the NAWM.

Value of exercise testing for screening asymptomatic men for latent coronary artery disease.

Treadmill exercise testing identifies a group of men at high risk for coronary atherosclerotic heart disease. However, the predictive value and sensitivity limitations are obvious. An abnormal electrocardiographic response does not absolutely predict the presence of coronary atherosclerotic heart disease, and a normal response does not rule out this possibility. Thus in appropriate instances when the minimal risk of coronary angiography is justified this procedure can be used to determine the anatomic correlation of exercise-induced functional ST-segment changes.

Acute asthma exacerbations and air pollutants in children living in Belfast, Northern Ireland.

The incidence of childhood asthma, a common condition, is on the rise worldwide. Despite reductions in the emission of urban smoke, traffic pollution is now a major worldwide problem. Belfast, Northern Ireland, is an old industrial city with major pollution problems. In this study, the authors investigated the rates of acute asthma admissions to Belfast's major children's emergency department. The admissions were studied, relative to day-to-day fluctuations in thoracic particulate matter, sulfur dioxide, nitrogen dioxide, nitric oxide, oxides of nitrogen, ozone, carbon monoxide, benzene, temperature, and rainfall. Daily admissions for acute asthma at the emergency department of the Royal Belfast Hospital and average daily pollution were recorded for the 3-yr period between January 1, 1993, and December 31, 1995. The authors used Poisson regression to assess independent association(s). Individually, small associations were seen for thoracic particulate matter (relative risk = 1.10), sulfur dioxide (relative risk = 1.09), nitrogen dioxide (relative risk = 1.11), nitric oxide (relative risk = 1.07), oxides of nitrogen (relative risk = 1.10), carbon monoxide (relative risk = 1.07), and benzene (1.14); no associations were noted between meteorological factors (temperature and rainfall) or ozone and asthma emergency-department admissions. The authors adjusted for the aforementioned parameters, and benzene level was the only variable associated independently with asthma emergency-department admissions in children. Benzene may be a more reliable method of measuring exposure to vehicle exhaust emissions than measurements of other pollutants.

The assessment of handicap: an evaluation of the Environmental Status Scale.

The assessment and measurement of disability and handicap are of particular relevance in rehabilitation, as these are the main foci of therapeutic intervention. The area of handicap is the least developed with few measuring tools to provide an objective assessment of status. The Environmental Status Scale (ESS) is now becoming increasingly utilized to measure handicap. It was used in a study of 50 patients on admission and discharge in a neurorehabilitation unit, to analyse its validity and usefulness as an outcome measure of the rehabilitation process. The results suggest it has limited validity, mixes disability and handicap issues and in some sections has a misleading scoring system. It requires modification to overcome these difficulties.

Introduction of integrated care pathways for patients with multiple sclerosis in an inpatient neurorehabilitation setting.

Rehabilitation of progressive neurological disorders such as multiple sclerosis poses particular problems, and clear setting of goals and clinical audit are essential for effective management. Integrated care pathways (ICPs) offer a unique opportunity to document and audit the rehabilitation process. This preliminary study has shown that ICPs are useful in both assessing process and auditing goals. Their introduction has led to the identification of the key worker role within the neurorehabilitation unit (NRU), and has provided an opportunity to increase the participation of patients and carers in the rehabilitation process. Continuing refinement of the pathway is necessary, and its application to other neurological disorders such as cerebrovascular accident may be appropriate.
