A case of dermatomyositis (DM) with only one abnormal muscle function, bilateral diaphragmatic paresis, is reported. The coexistence of interstitial lung disease allowed us to sample the diaphragm during an open lung biopsy. Characteristic features of myositis were found. Lung volumes, compliance, maximal inspiratory pressure, and transdiaphragmatic pressure gradient (delta Pdi) were lower than normal. Lung recoil pressure was normal. Reversible small airways obstruction was also found. Treatment with prednisone produced dramatic improvement, with all functional values becoming normal except for compliance, which remained slightly decreased. A thorough evaluation of inspiratory muscle strength is advised in all patients with DM in order to identify varying degrees of altered diaphragmatic function.
Persistence of symptoms in patients with myasthenia gravis who have undergone previous thymectomy has been attributed to thymus remnants. Patients with partial or no recovery were studied 40 +/- 31 months (mean +/- SD) after surgery, which had been carried out by the transcervical approach in 20 and trans-sternal approach in four. Lateral x-ray tomography of the mediastinum after injection of air showed images compatible with residual thymus gland in 18 patients (75 percent). Thirteen of these underwent reoperation by the trans-sternal approach, and thymic tissue was found in 11 (85 percent). After repeated thymectomy, 67 percent of the patients improved clinically. Therefore, it is quite reasonable to infer that incomplete removal of the thymus was responsible, at least partly, for failure of the first procedure.
Data on methicillin-resistant Staphylococcus aureus have not been presented before in Argentina. Due to non adequate microbiologic techniques, this phenomenon may be overlooked. Heterogeneous strains are only detected with 5% NaCl agar, low temperature (30 degrees C) or prolonged incubation and/or high inocula (10(6) C.F.U.). Minimal inhibitory concentrations (MICs) of methicillin were studied by the agar dilution technique in antibiotic medium number 4 with and without 5% salt. This study was performed for 107 strains at 37 degrees C, after 24 and 48 h incubation, with an inoculum size of 10(6) CFU. S. aureus strains were considered resistant whenever MICs were greater than or equal to 8 mg/l. Forty seven (44%) methicillin-resistant strains were detected. Thirty one were high level resistant mutants and sixteen showed the heterogeneous phenomenon. Due to the high frequency of resistant strains among clinical specimens in this country, the routine use of elaborate screening tests with special media and or prolonged incubation time is justified.

Cefamandole penetration was studied in 32 normal bone specimens. No antibiotic bone binding was found. Blood contamination of bone samples was measured by the haemoglobin method. Mean corrected levels attained (5.8 micrograms/g) exceed MIC values of sensitive Staphylococcus aureus, Haemophilus influenzae and Enterobacteriaceae. Similar diffusion was observed in cortical and trabecular bone. The results obtained invite further studies on preoperative prophylaxis and treatment of osteomyelitis.

A giant pleural fibrous mesothelioma was the cause of severe nonobstructive ventilatory incapacity, myocardial restriction with congestive failure and hypoglycemic episodes in a 58-year-old man. Surgical resection resulted in complete recovery of respiratory function and disappearance of hypoglycemic episodes.
In the case reported here, a patient with severe Plasmodium falciparum infection with 20% of erythrocytes parasitized was cured by exchange transfusion in conjunction with classical drug therapy.
A 42 year old pregnant woman was admitted in acute respiratory failure. Viral pneumonia was suspected and oxygen therapy, CPAP, water restriction and diuretics were started with good response. She remained febrile and had an abnormal chest X-ray, a diagnosis of miliary tuberculosis was confirmed by transbronchial fibreoptic lung biopsy.
Blood gases and acid-base balance were measured in a group of patients in status asthmaticus. Simple or combined metabolic acidosis was found in 37.9% of the patients. In 25 of these, lactic acid (LA), pyruvic acid (PA) and LA/PA were increased. Increased glycolysis and anaerobic respiratory muscle glycolysis during extreme airways obstruction may be instrumental in these changes. Base deficit, not accounted for by organic acid increase, may be dependent on previously present hypocapnic hyperventilation. Possibly hyperlactemia provoked by hyperventilation may be exaggerated in severe asthma.
