Tumor necrosis factor induction of endothelial cell surface antigens is independent of protein kinase C activation or inactivation. Studies with phorbol myristate acetate and staurosporine.

We have investigated whether TNF-induced changes in human endothelial cell (EC) surface Ag expression are mediated by protein kinase C (PKC). This suggestion arose from the observations that PMA, a potent PKC activator, can mimic TNF by inducing expression of endothelial leukocyte adhesion molecule 1, intercellular adhesion molecule 1 (ICAM-1), and class I MHC molecules on human EC. However, in contrast to the actions of PMA, TNF neither causes membrane translocation of PKC nor induces the phosphorylation of the myristoylated alanine-rich C kinase substrate, two measures of PKC activation. Moreover, the PKC inhibitor staurosporine can block PMA-induced endothelial leukocyte adhesion molecule 1 expression at 4 h, but does not inhibit the actions of TNF. At 24 h, staurosporine itself induces intercellular adhesion molecule 1 and class I MHC, and acts additively with TNF. Twenty four hour treatment with PMA causes loss of PKC. We propose that at 24 h, staurosporine and PMA share a mechanism of action, namely diminution of PKC activity. However, 24 h treatment with TNF does not reduce the amount of PKC nor does it prevent activation of PKC by PMA. We conclude that TNF effects in EC are not mediated by PKC activation or inactivation.

Assay of HIV-1 proteinase: a colorimetric method using small peptide substrates.

A colorimetric assay for HIV proteinase using small protected peptide substrates is described. Substrates are cleaved to release N-terminal prolyl peptides which react with isatin to form a blue product which is measured spectrophotometrically. The assay is suitable for use with pure enzyme or crude extracts derived from genetically engineered Escherichia coli.

Type III tibial fractures in the elderly: results of 23 fractures in 20 patients.

A series of 23 type III fractures of the tibia in 20 elderly patients (over 60 years of age) presenting for combined plastic and orthopaedic surgical management over a period of 15 years are reviewed. Overall limb salvage was 53 per cent, with an average inpatient duration not significantly different from that for amputation, suggesting that an age of over 60 years is not necessarily a contraindication to attempts at limb salvage in type III injuries.

Prophylactic digitalization for thoracotomy: a reassessment.

A prospective, controlled, randomized clinical study of 140 patients undergoing elective thoracic operations over a period of 1 year in a regional referral unit was performed in which one group received digoxin and the other did not. The incidence of cardiac arrhythmia was compared in each group. Overall mortality was 5.7%. There was no significant difference in incidence of cardiac arrhythmia in each group, and we conclude that the prophylactic use of digoxin in elective thoracic operations should be revised.

Sutures, staplers, leaks and strictures. A review of anastomoses in oesophageal resection at Royal Victoria Hospital, Belfast 1977-1986.

Leakage from an oesophagogastric anastomosis has a high morbidity and mortality. Recent evidence suggests that mechanical tissue stapling devices can decrease the rate of anastomotic breakdown but at the expense of an increase in the occurrence of fibrotic strictures at the anastomosis site. This study examines the rate of leakage and stricture in hand sutured and stapled anastomoses. A retrospective study was made of 221 oesophagogastric anastomoses following resection for carcinoma between 1977 and 1986. There were 122 sutured and 99 stapled anastomoses. Leak occurred in 21 sutured (17.2%) and 7 stapled (7.1%), P less than 0.05. If the stapled anastomosis was completely satisfactory and required no reinforcing sutures, the breakdown rate was in fact only 3% (2/69), P less than 0.01. A stapled anastomosis which required reinforcement had a similar chance of breakdown as a sutured anastomosis (16.7%). There was little difference in the performance of registrars in training and consultants at hand-sewn anastomoses with leakage rates of 13.7% and 18%, respectively; P greater than 0.05. The registrars, however, did not improve with the use of the stapler with a leakage rate of 14.3% compared to the consultants' rate of 1.75%, P less than 0.05. Involvement of the limits of resection with tumour slightly favoured breakdown--15.5% compared to 11.6% if the limits were free from tumour, P greater than 0.05. The incidence of malignant strictures was similar in both groups but benign stricture was more common in the stapled group--13% (13/99) compared to 1.6% (2/122), P less than 0.01. The mechanical stapler brings uniformity to the anastomosis but cannot compensate for deficiencies in surgical technique.

Experience in low velocity gunshot injuries: a more conservative approach in selected cases.

Sixty-eight patients with gunshot and blast injuries who had sustained 200 wounds were treated in a single surgical unit over an 18-month period. Many of these were low velocity injuries to the soft tissues of the lower limbs. A change of policy was adopted for this selected group which involved minimum wound debridement under local anaesthesia in association with antibiotics. This policy enables early discharge from hospital and has resulted in a low infection rate with no significant morbidity.

Inhibition of the aspartic proteinase from HIV-2.

Kinetic constants were determined for the interaction of the HIV-2 aspartic proteinase with a synthetic substrate and a number of inhibitors at several pH values. Acetyl-pepstatin was more effective towards HIV-2 proteinase than the renin inhibitor, H-261; this effect is exactly the opposite from that observed previously for the proteinase from the HIV-1 AIDS virus.

Staples versus sutures in the closure of scalp wounds: a prospective, double-blind, randomized trial.

Over a 6-month period, patients attending the Accident and Emergency Department of the Mater Infirmorum Hospital with scalp wounds were randomly allocated to one of two groups for closure of their wounds with either nylon sutures or metal staples. Each group contained 100 patients. Patients were reviewed at 5 days (95 per cent) after the removal of sutures or staples and again at 3 weeks (61 per cent). The two groups were similar for age, sex, aetiology and wound characteristics. There was no difference in morbidity or rate of wound infection but stapling was significantly faster and less painful. The use of staples carries no risk of accidental needle-stick injury and should therefore be safer than suturing for the user.

Reproductive behavior of female rats treated with ethanol on postnatal days 1-7 or 8-14.

Effects of postnatal administration of ethanol upon adult reproductive behavior of female rats was investigated in the two studies reported here. Ethanol, in a nutriment vehicle, was administered by intragastric intubation to avoid confounding effects of underfeeding. Estrous cycles appeared typical in all animals. Treatment groups did not differ in fertility nor with respect to litter sizes or sex ratios of subsequent litters.
