[The prevalence of HBsAg in hospitalized children as a marker of hepatitis B virus infection].

UNLABELLED: The aim of this study was to determine the prevalence of hepatitis B surface antigen (HBsAg) in hospitalised children, as specific marker for hepatitis B virus (HBV) infection. Our study group consists of 517 children, 68 of them diagnosed with chronic hepatitis. For HBsAg determination we used an ELISA test (Labsystems); for some children we also tested by ELISA the following markers: the antibodies and anti-hepatitis C virus (HCV) antibodies. From 517 children 24.28% were HBSAg positive and 75% of children with chronic hepatitis were positive for the same marker. Almost 100% of chronic active hepatitis (CAH) patients was positive for HBSAg. CONCLUSIONS: 1. The prevalence of HBsAg was much higher as compared with the healthy population prevalence; it is a clear prove that HBV infection has an important role in chronic hepatitis appearance. 2. For all HBsAg positive patients, it is necessary to determine other markers like HBeAg-anti-HBe antibodies system as well as markers for other viral hepatitis (HDV, HCV). 3. The anti-HBV infection vaccine will reduce significantly the prevalence of HBV and HDV infections; 4. Biological molecular technique, like PCR will be necessary in our country, in the future, even the price is so high, to monitoring the IFN treatment for chronic infection as unique solution for these patients.

Enteroviral etiology of some epidemic outbreaks in a zone of Romania.

The authors describe three enterovirus epidemics: an outbreak of Coxsackie B1 epidemic myalgia, one of echovirus 4 meningitis and one of epidemic vomiting and diarrhea caused by Coxsackie A9 virus. Etiological diagnosis was established by virological and serological tests.

[The evaluation of a method for determining apolipoproteins as a sensitive indicator in assessing lipid metabolic disorder in a population at risk for essential arterial hypertension (EAHT)].

The increased efficiency of epidemiological screening tests in determining the prevalence of risk factors carriers required the use of highly sensitive markers able at detecting early, in reversible stages, the disturbances in the lipid metabolism genetically induced or acquired during lifetime, known as major risk factors for essential hypertension and other cardiovascular diseases. The authors report the preliminary results of some investigations on the predictive value of apolipoproteins. Immunokits manufactured by BioMérieux--France were used. In 50 admitted patients with essential hypertension and family history of risk, high levels of apoA1 and apoB were recorded although cholesterol and other biological markers were within the normal. At the same time, in our case the apoB/apoA ratio was supraunitary, fact proving the validity of the method.

[Seroepidemiological considerations of the incidence of viral hepatitis C in some population groups].

In the interval 1985-1994, in the Iaşi district, the reported cases of viral hepatitis (VH) have recorded annual morbidity rates ranging between 67/100,000 inhabitants in 1993 and 373/100,000 in 1990. VH attributed to HAV and HBV has represented, with yearly differences, 62% to 94% of all cases. Posttransfusion viral hepatitis (PTVH) has determined a morbidity of 0.51/100,000 inhabitants in 1986 and 8.34/100,000 in 1994. PTVH without AgHBs ranged from 16.3% in 1993 to 94.0% in 1987, as compared to 6.0% in 1987 and 1988 and 75.0% in 1985 for PTVH with AgHBs. In the interval under study 70.9% of the PTVH cases were most likely determined by HCV and 28.9% by HBV (AgHBs+). From all PTVH cases, 64.0% were from urban areas, 59.3% were females, 92.5% belonged to over 20 years age groups, and 31.3% were industrial workers, the remainder belonging to other 5 occupational groups. In 241 patients (18.2%) admitted for liver disorders, posttransfusion hepatitis included, AcHCV was present in 39.4%, of which 45.2% belonged to the age groups 20-60 years. The results of some investigations carried out on 4471 blood donors (72.4% males, 85.4% aged between 21 ad 50 years, 86.4% from urban areas, 62.1% workers in industry and building) are presented. Out of these, 217 (4.9%) presented to hepatitis C virus (AcHCV). Occupational distribution of AcHCV possessors shows that 44.2% are workers in the heavy industry, 14.8% in the light industry and 12.9% unemployed. Sera were tested by Monolisa New Antigens, and those with AcHCV were reassessed with Monolisa, Murex anti HCV, ORTO-HCV 3.0 and confirmed by Immunoblot DECISCAN, with 5 antigens.

[A clinico-etiopathogenic update on mental stress as a cardiovascular risk factor].

In the psychic stress, ADH releases at the same time with CRF which intensifies the effects of it. Endogenous opioids cause above all the analgesia and paradoxical euphoria of stress. Psychic stress needs through the risk of heart frequency a higher capacity and it aggravates the valvular insufficiency and the valvular stenosis. The patient with rheumatic mitral stenosis could have a genetic psychic location predisposed to a higher vulnerability for psychic stress.

[Multidisciplinary epidemiological observations on the role of mineral elements and other environmental factors in inducing essential arterial hypertension].

The results of a pluridisciplinary epidemiological study carried out on a series of 225 adults from rural areas, in whom the prevalence of arterial hypertension disease (AHTD) and borderline (AHTB) was of 19.0% and 14.4%, respectively, are presented. In 19.0% of the carriers of over five associated risk factors for cardiovascular disease, the classical lipid constants (triglycerides-TG. total lipids-TL. total cholesterol-TC. HDL-cholesterol-HDL-c), plasma (PML) and erythrocyte magnesium level (EML), as compared to the degree of drinking water mineralization were determined. The environmental conditions, also expressed in water hardness (21.650 G) or magnesium water intake (85.13 mg/l) correlate in 41.0% of the subjects with a decrease in PML (1.8 mg/l) and in 70.48 with a decrease in EML (4.8 mg/l) through the association of some behavioral or metabolic risk factors: stress, consumption of atherogenic foods, excess alcohol and lipid dysmetabolism (TG: 130-140 mg/l = 59.5%; TL: 8 g%-30.9%; TC: 220 mg/l = 19%; HDL-c: 35 mg/l + 1.9%).

[The clinico-epidemiological premises for antioxidant prevention and treatment in chronic myocardial infarct].

The improvement of therapeutic and recoverable proceeding to chronic myocardial infarction (IMC) imposes a complex epidemiological estimation of the global cardiovascular risk, of their clinical and biological status. The retrospective evaluation of the risk factors (RF) by epidemiological methods, in 38 hospitalized prevalence of both constitutional and behavioral factors, and the quantification of the risk state, according to graph Euro 194, has pointed on the fact that 50% of IMC cases have presented a global risk of 10-20%. That justified the application of a preventive conduct in order to continually neutralize the associates FR. The biochemical determinations have allowed to frame the dyslipidemic patients, according to the ARCOL classification in the classes D (29.2%), E (25.0%) and B (20.8%) with direct implication in the adopted therapy. The colorimetric dozing with thyo-barbituric acid of the malonly dialdehyde (MAD) each is a product of the lipidic peroxidation, as well as the interpreting or correlation of the registered values with an evolutive clinic stage, and with the existence of some existing diseases (diabetes, chronic hepatitis etc.) along with signification of other dismethabolical parameters (cholesterol, triglycerides, LDLc, apoB) have confirmed the necessity of a complex therapy, including an antioxidative treatment, having the role to directly inhibit or exclude the free radicals resulted after the oxidative stress of the infarct.

[Incidence of HBsAg among different population groups in Moldavia in 1991 compared with 1973].

AgHBs incidence in various population groups from the Iassy region was relatively high in 1991, ranging from 3.8% in blood donors to 75.0% in patients with severe acute virus hepatitis. As compared to the incidence values recorded in 1973, 18 years ago, in the population of the same territory, some mutations were observed, the most important being the more then threefold reduction of the AgHBs incidence in blood donors. In subjects with liver diseases and in risk groups, the frequency of AgHBs carriers maintains at a high level.
