Effects of a new glucocorticoid, deflazacort, on pituitary-adrenal function in man: a comparison with prednisone.

The influence on plasma ACTH and cortisol and on blood sugar have been evaluated in seven volunteers after single oral doses of prednisone 4 mg and 8 mg and deflazacort 5 mg and 10 mg. Drugs were administered at midnight to achieve a maximum inhibitory effect on the hypothalamic-pituitary-adrenal axis. No difference was detected in the majority of cases between the high and the low dose of each drug. In particular, the results obtained with the higher doses show a significant effect of both drugs on ACTH (at hour 4 after drug administration) and on cortisol (at hours 4, 8, 12 and 16). As for blood sugar, the hyperglycemic activity of deflazacort appears to be lower than prednisone at hours 12 and 16. On the whole, the potency of deflazacort appears similar to that of prednisone.

Relationship between seminal PGE and 19-OH PGE and seminal fructose in man.

Seminal PGE, 19-OH PGE and fructose were assayed in normal volunteers and in infertile patients. From data obtained it appears that seminal PGE are probably not synthesized exclusively at seminal vesicles level since no relationship was observed between PGE and fructose. On the other hand the seminal vesicles appear to be the preferential site of seminal 19-OH PGE production since a close relationship was observed between 19-OH PGE and fructose in those case in which these prostaglandins were below minimum normal values. Various hypotheses are advanced in order to interpret the lack of correlation in the presence of high 19-OH PGE levels.

Treatment and follow-up of patients with infertility due to spermagglutinins.

A therapeutic trial based on pharmacologically induced azoospermia followed by the administration of corticosteroids was carried out in 48 patients with infertility due to spermagglutinins. Three types of responses were observed: type A, no modifications in the blood and spermagglutinating (SA) titer (19 cases); type B, disappearance or significant decrease in SA titer during treatment, with return to normal values upon resumption of spermatogenesis (7 cases); and type C, disappearance or significant decrease in SA titer for at least 1 year after stopping treatment (22 cases). Within the 1st year after stopping therapy only men who responded in type C fashion were able to impregnate partners, three pregnancies ended in abortion and nine in normal births.

Clinical experimental evaluation of the effect of dibromotyrosine on thyroid function.

In order to evaluate the effects of dibromotyrosine (DBT) on the thyroid function (already demonstrated in animals), thyroid tests were performed in 10 subjects with functional signs of thyroid hyperfunction, before and after administration of 1800 mg (600 X 3) of DBT for 10 days. The results show a mild decrease of the RAI uptake, but no significative interference on the thyrodal hormonogenesis. According to the author, the DBT seems to act on the hypothalamic centers regulating the thyroid function, due to its peculiar molecular components. It is therefore particularly suitable for the treatment of patients with involvement of the C.N.S.

Hepatic activity in the generation of somatomedins: effects of administration of a liver extract.

The problem of the generation of growth factors (somatomedins) (SM) is at present a matter of investigation. The liver has been proposed as the primary site of production of SM. In order to verify this hypothesis, SM activity has been assayed in patients with impairment of liver function. The subjects have been divided into three groups according to the severity of the hepatic damage, assessed with conventional diagnostic tests. In every group, somatomedin A (ASM) has been assayed by means of a biological procedure, in basal conditions and after treatment with a liver extract. The results show that the ASM activity is reduced in proportion to the degree of hepatic damage. The administration of liver extract restores the production of the ASM in the first two groups of patients, i.e. in those with mild and moderate hepatic involvement.

High deoxyribonucleic acid content of spermatozoa from infertile, oligospermic human males.

A study has been carried out, by means of an indole microchemical technique, of the DNA content of spermatozoa from a group of 13 normal subjects and 13 patients affected by infertility, characterized by oligospermia with a clinical, hormonal, and testicular picture peculiar to "idiopathic spermatidic arrest." The DNA content of spermatozoa from the infertile, oligospermic patients was nearly double that of normal subjects. This finding can be explained tentatively on the basis of genetic and/or cytologic events occurring during meiosis.

Response of adnexal glands to testosterone stimulation in the normal adult male.

Normal male volunteers were studied in order to set up a functional exploration test based on administration of testosterone propionate for use on the adnexal glands of the male genital tract. Seminal acid phosphatase and citric acid levels showed no significant variations compared with spontaneous physiologic oscillations. Concentration and total quantity of fructose were considerably reduced. Alkaline phosphatase showed an intermediate pattern with constant values of concentration and decrease of total quantity. The hypothesis is advanced that adnexal glands possess a secretory metabolism with a maximal basal response to androgen stimulation, but with a different mechanism of action: partly adaptable at the level of the prostate and of the "all or nothing" type at the level of the vesicles.
