We report two cases in which partial resection and reconstruction of the superior vena cava (SVC) were performed with the use of a temporary SVC-right atrial internal bypass for complete resection of carcinoma of the right lung. In the first case, the SVC was invaded by the primary tumor itself, arising in the anterior segment of the right upper lobe. In the second case a metastasized tracheobronchial lymph node had invaded the SVC. In both cases, partial resection of SVC was performed safely with the internal bypass technique.

1. Seventy cases of the leiomyogenic tumors were found in 5451 resected stomachs; 49 leiomyoma, 17 leiomyosarcoma and 4 leiomyoblastoma. These leiomyogenic tumors occupied 40.5% of 173 submucosal tumors of the stomach. 2. The average age was 51.7 in leiomyoma, 57.3 in leiomyosarcoma and 41.6 in leiomyoblastoma with male predominance. 3. The leiomyoma and leimyosarcoma were frequently found in the upper 2/3 of the stomach. 4. The size of the tumors was within 3 cm in diameter in about 80% of leiomyoma with ulcer formation of 10.2% and over 3 cm in about 88% of leiomyosarcoma with ulcer formation of 64.7%. 5. The classification of the leiomyogenic tumors into the benign and malignant was occationally difficult. For the diagnosis of the leiomyosarcoma, the number of mitosis, irregularity of nucleus, increase of cellularity and decrease of smooth muscle production were helpful.

The heterotopic pancreas collected from the surgically resected stomachs were studied clinico-pathologically. The number of cases of heterotopic pancreas were 64; 43 in the stomach and 21 in the duodenal bulbus. They were corresponded to 1.2% in the 5446 resected stomachs. The operated cases with this lesion were frequently found in the age of thirty to fifty with male predominance. Thirty eight cases of them were combined with other gastro-duodenal diseases, especially gastric or duodenal ulcers. About 90% of these lesions were found in the pylorus and antrum. They were situated frequently in the greater curvature, posterior and anterior wall, and rarely in the lesser curvature. Their size was ranged about 0.3 to 4.7 cm in diameter, and about 80% of them within 3 cm in diameter. Macroscopically, 49 polypoid, 8 nipple-like, 7 flat and 1 thickening folded lesions were found, respectively. The bridging folds were found in 9 cases, 13.8%. Microscopically, 30, 29 and 6 lesions of type I, II, and III types according to Heinrich's classification were found, respectively. The ulcer formation of the mucosal surface of the heterotopic pancreas was found at rate of 10%, and cystic transformation in about 6%.

The cases of the gastric carcinoma combined with distant benign gastric ulcer were 90 of 1257, 7.1%, and the carcinomas were frequently found in the lesser curvature of the antrum and the combined gastric ulcers frequently in the lesser curvature and posterior wall of the corpus. Average age in the combined cases was 58.8 which was older about 7 years than that in the cases of the gastric ulcer. The cases, in which the ulceration in the gastric ulcer was same or deeper than that in the gastric carcinoma, occupied about 70% of these combined cases. It should be considered that the malignant transformation does not always occur in the preceding ulcerative lesion. However, the intestinal metaplasia was usually severe, and so the well-differentiated carcinomas were twice frequent to the low-differentiated ones. The repeated regeneration of the epithelial cells associated with the erosive or ulcerative change may be of some relation to the carcinogenesis, because the intestinal metaplasia appears usually associated with the repeated defect and repair of the propria mucosae.

The gastric ulcer and carcinoma in the fundic gland area were investigated with the surgically resected cases. 1. The frequency of the ulcer in the fundic gland area was 8.2% of 2194 cases of gastric ulcer and that of carcinoma in this area was 8.4% of 780 cases of gastric carcinoma. These two diseases showed high frequency in the age group of forty to fifty years withpreponderance in female. 2. The ulcers in the fundic gland area usually showed high acidity and high frequency of massive bleeding like as melena and hematemesis comparing with other lesions or the ulcers in other sites. 3. The ratio of the low-differentiated carcinomas to the well-differentiated ones was about 3.7 to 1 in the fundic gland area and 1 to 1.3 in the pyrolic gland area. 4. The frequency of the early gastric carcinomas was 27% and 41% in the fundic and pyrolic gland areas, respectively. 5. In the fundic gland area, as observed in the pyrolic gland area, the low-differentiated carcinomas usually occurred on the non-metaplastic mucosa and the majority of well-differentiated ones were frequently found in association with the intestinal metaplasia. 6. Although the solitary carcinoma in the anterior wall or greater curvature was found not infrequently, the solitary ulcer was not found in these sites in this series.
