Dental and periodontal health status in Campanian children and relation between caries experience and socio-economic behavioural factors.

AIM: Our aim was to record oral health situation through mean value of DMFT/dmft, Significant Caries Index (SiC) and CPITN values in a young-population sample in Campania (Italy) and to determine the possible relationship between oral health behaviour, socioeconomic factors and caries experience. STUDY DESIGN: Observational study. METHODS: 101 children (age range 5-18 years) were randomly selected from 10 public schools of the Regional Campanian district and visited at the Paediatric Dentistry Department of Federico II University of Naples (Italy). Clinical examinations were conducted, under standard light, using a plane buccal mirror, a dental probe and air drying to evaluate caries experience and a WHO CPITN ball-point probe to record the periodontal health of each child. A questionnaire (investigating demographic and oral health behaviour data) was filled. X-ray bitewing were also taken. The comparison of quantitative variables among groups was carried out using the one-way analysis of variance (ANOVA). RESULTS AND STATISTICS: The caries prevalence was 81%; the sample mean values were 3.5 (SD=3.79) for DMFT and 3.8 (SD=3.39) for dmft. The variables influencing statistically DMFT/dmft values in the sample are the following: family socio-economic level, level of educational attainment of children mothers and use of the school canteen service. Most of the examined children show CPITN levels between 0 and 1. CONCLUSION: Our results confirm a high caries prevalence and also a need for preventive and educational programmes to obtain caries decrease.

Early combination therapy (bromocriptine and levodopa) does not prevent motor fluctuations in Parkinson's disease.

Early combination therapy with bromocriptine (Br) and levodopa (LD) is believed to delay or prevent the onset of late treatment complications typically associated with LD monotherapy in Parkinson's disease (PD). Studies recommending this regimen have been uncontrolled. We evaluated this possibility in a 4-year, double-blind, randomized, parallel group trial comparing Br and LD both alone and in combination in 22 PD patients never before treated with dopaminergic medications. In the group receiving Br monotherapy, 17% had motor fluctuations (end-of-dose failure or on-off), 17% chorea, 33% dystonia, and 83% freezing. In the LD group, 33% had motor fluctuations, 56% chorea, 100% dystonia, and 22% freezing. In the combination group, 71% had motor fluctuations, 57% chorea, 71% dystonia, and 57% freezing. The frequency of dystonia was significantly lower with Br monotherapy than in the other two treatment groups. No other significant differences were observed. LD monotherapy appeared to have superior efficacy in the treatment of PD. Mean final doses of LD and Br were similar for the different treatment groups. Early combination therapy does not prevent or delay the onset of motor fluctuations or dyskinesia in PD.

Surgical management of adrenal cortical carcinoma.

The experience with the surgical management of 14 patients with adrenal cortical carcinoma is presented. Four patients were males and 10 females (mean age: 37 years). Seven patients (50%) has proven hormonal activity and 7 (50%) had "non functioning" tumors. The location of carcinoma involved the left adrenal gland in 9 cases, the right in 3 and it was bilateral in 2. The mean diameter of the mass was 10 cm. The most helpful diagnostic tests were shown to be sonography and computed tomography. The surgical procedure was chosen according to the patients condition: this included resection of primary lesion and excision of local lymph nodes and all involved structures. Adjuvant treatment with either mitotane, 5-FU or local irradiation did not result in any benefit with respect to the expected survival. There was a slightly better survival for functioning versus non functioning tumors. Surgery remains the treatment of choice for these tumors. Prognosis is exceedingly poor: only 5 patients (36%) have survived beyond two years after surgery.

[Periapical lesions treated with CaO. Histological controls].

The results of histological examinations conducted on 2 granulomas found in teeth and treated with calcium oxide are reported. It is concluded that the treatment produces significant changes in the histological picture with the formation of scar tissue that precedes the growth of newborn in a relatively short time.

Radioimmunoassay of gastrin in human saliva.

We have developed a personal procedure for the radioimmunoassay of gastrin (lower detection limit 3 pg/ml). Using 5-fold concentrated lyophilized salivary samples, we are able to detect the basal content of gastrin immunoreactivity in saliva of normal fasting people (mean +/- SD: 1.38 +/- 0.61 pg/ml) compared to plasma gastrin (mean +/- SD: 28.88 +/- 11.00 pg/ml). We found that after meal stimulus, the value of salivary as well as plasma gastrin concentration increased (the correlation coefficient r among salivary and plasma gastrin being 0.899; p less than 0.001). We believe that gastrin appears in a very low concentration in saliva and that it is not produced by the gland, but derives probably from the serum clearance.
