[Effect of coronary revascularization by percutaneous coronary transluminal angioplasty on the diastolic filling of the left ventricle].

Percutaneous transluminal coronary angioplasty (PTCA) is useful to study the cardiac function in patients with coronary artery disease (CAD). Left ventricular (LV) diastolic filling is often altered in CAD and Doppler-echocardiography can be used in order to assess the behavior of LV diastolic filling. Thus, 68 patients, divided into 2 groups (52 with CAD and 16 normal subjects as controls), underwent Doppler-echocardiography to evaluate the following diastolic filling indexes: peak velocity of early- (E) and late- (A) diastolic filling, the velocity-time integrals of early- (VTI E) and late- (VTI A) diastolic filling, the ratios E/A and VTI E/A. The CAD group was divided into 4 subgroups according to the severity of coronary disease. The overall CAD group showed a significant reduction of E/A and VTI E/A when compared to controls (E/A: 1.10 +/- 0.27 vs 1.67 +/- 0.48; p less than 0.001) (VTI E/A: 1.84 +/- 0.43 vs 2.84 +/- 0.82; p less than 0.001). No differences were found between the 4 subgroups. PTCA was performed in 21 patients: 15 successfully and 6 not. The latters did not show significant changes in diastolic filling parameters (E/A: 0.71 +/- 0.21 vs 0.72 +/- 0.25; NS) (VTI E/A: 1.24 +/- 0.31 vs 1.25 +/- 0.43; NS). The other 15 patients showed significant improvement of E/A ratio (1.07 +/- 0.21 vs 1.37 +/- 0.19; p less than 0.01) and VTI E/A (1.78 +/- 0.30 vs 2.31 +/- 0.29; p less than 0.001). In conclusion, LV diastolic filling indexes are significantly altered in patients with CAD and seem reversible after PTCA.

[Effectiveness of diltiazem in controlling ventricular response and improving exercise capacity in chronic atrial fibrillation. Double-blind, cross-over study].

Although digoxin is often the drug of choice to control the ventricular response in chronic atrial fibrillation, it fails to control exercise-induced increase in heart rate. The efficacy of diltiazem to control ventricular response and to improve cardiovascular performance during maximal exercise was investigated in 13 digitalized patients with chronic atrial fibrillation. A placebo controlled prospective randomized double-blind study, was preceded by open titration phase. During the diltiazem treatment phase, mean ventricular response diminished at rest (85 +/- 12 versus 107 +/- 19 during digoxin therapy and versus 101 +/- 18 during digoxin and placebo therapy; p less than 0.001), as well as during maximal exercise (142 +/- 13 versus 159 +/- 17 during digoxin treatment and versus 160 +/- 14 during digoxin plus placebo treatment; p less than 0.001). During exercise (50 W), in a subgroup of 7 patients, mean ventricular rate dropped: 109 +/- 19 versus 142 +/- 21 during digoxin treatment and versus 143 +/- 17 during digoxin plus placebo treatment; p less than 0.001). In all patients, the mean rate at rest decreased about 19.3 +/- 6.9% and at maximal exercise about 10.3 +/- 4.9%. In the subgroup of the 7 patients ventricular mean rate at a load of 50 W decreased about 23.6 +/- 7.9%. In all the patients, maximal exercise capacity improved: the MET mean value increased from 3.59 +/- 1.3 to 4.52 +/- 1.18 (p less than 0.001); the mean value of the maximum exercise (MEC), according to the Redfords formula, increased from 65 +/- 48 to 132 +/- 70 (p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

[A computerized system for the management of clinical data in a cardiology service].

The authors describe a computerized system for medical records built in order to obtain rapid information about the data of the overall patients and to quickly manage the clinical research of the Institution. The hardware, an Olivetti M 19 personal computer with a 10 Mbyte hard disk, can manage the clinical data on 3000 patients or more; the software, DBase III Plus, by Ashton-Tate, has a programming language easy to use, even by non-experts. The system allows immediate statistical analysis of all patients and of all 350 parameters included in the database.

Mitral systolic honk in a case of congenitally corrected transposition of the great vessels.

A woman suffering from TCGV associated with steno-insufficiency with a prolapse of left atrioventricular valve (tricuspid) and an insufficiency with a prolapse of right atrioventricular valve (mitral) presented a systolic honk which was to be registered in all the precordial area. The increase of this noise at the end of inspiration and after amyl nitrite, in the absence of any relevant change as to the prolapse of both atrioventricular valves, demonstrated that the honk originated from the right atrioventricular valve more than from the left one.

Exercise capacity in young patients after total repair of Tetralogy of Fallot.

Tetralogy of Fallot is the most common form of cyanotic congenital heart disease. Measurement of physical activity is usually performed as a routine part of the patient's cardiac evaluation. The aim of this study was to examine the exercise performance of young patients operated on for tetralogy of Fallot, assessing the possible influence of known negative prognostic factors related to the surgical repair. The study group comprised 41 consecutive patients (29 male and 12 female, ages 11.2 +/- 3.9 years, range 6-16 years) operated on for tetralogy of Fallot. Patients in the study group were divided in subgroups in relation to the age of surgical intervention (before or after 2 years of life), the surgical approach (combined transatrial/transpulmonary approach or right ventriculotomy), and the presence of aortopulmonary shunts prior to performing total correction. Their data were compared with those of 33 aged-matched asymptomatic control subjects (19 male and 14 female, ages 11.9 +/- 1.3 years, range 11-16 years). Blood pressure and heart rate measured at rest were similar between control and Fallot groups. A normal increase in systolic blood pressure was observed in response to exercise intensity for all subgroups. No significant difference between control and Fallot groups was found under conditions of mild or moderate exercise or for diastolic blood pressure at rest and in response to exercise. Lower maximal heart rate and systolic blood pressure values were recorded in all patients when compared with the control subjects. Significant differences in peak workload were detected between control and Fallot groups and between the control and each subgroup; however, no difference was found between subgroups. In conclusion, despite their very satisfactory clinical status, all patients showed a reduced peak workload, irrespective of the surgical approach, age at surgery, and aortopulmonary shunts prior to performing total correction.

Analysis of recovery time indexes in 5-fluorouracil-treated cancer patients.

Based on our previous experience in doxorubicin-treated patients, in whom we observed a significant increase of ventricular recovery time indexes, we analyzed these non- invasive parameters of myocardium electrical instability in forty-three 5-fluorouracil-treated patients, to test the hypothesis of a mechanoelectrical disarrangement occurring in 5-fluorouracil (5FU) cardiotoxicity. All patients enrolled were studied at the first presentation and following chemotherapy. The study showed the absence of any significant changes in recovery time indexes or in other electrocardiographic parameters. Our data suggest that 5FU does not interfere with electrical properties of myocardial fibers.

[Mitral prolapse and the ergometric findings].

In evaluating the significance of arrhythmias and ECG changes during exercise, 42 consecutive patients (pts) with mitral valve prolapse (MVP) underwent a symptom-limited cycloergometer Exercise Stress Test (EST) with load increase of 25 watts every 3 minutes. Eight patients (19%) were positive for anamnestic cardiopalm. The patients were divided in 2 groups, according to echocardiographic evidence of the MPV: group A (12 patients, mean age 32 +/- 13) with prolapse of one mitral leaflet and group B (30 patients, mean age 30 +/- 13) with prolapse of both mitral leaflets. Exercise duration doesn't differ significantly in the two groups. No arrhythmias during EST were found in group A, while arrhythmias were present in 6 patients (20%) in group B. A strong correlation was found between anamnestic cardiopalm and arrhythmias during EST (6/8 = 75%). Three of four patients (75%) with ST impairment during EST, showed at thallium myocardial scintigraphy a non reversible perfusion defect after exercise. These data showed a higher incidence of arrhythmias during EST in patients with MVP of both leaflets and good relationship with symptomatology of cardiopalm.

Expression of HECA-452 in parapsoriasis and mycosis fungoides.

We have investigated the HECA-452 expression in large plaque parapsoriasis (PP) and mycosis fungoides (MF) patients, evaluating the potential role of this biomarker in both cutaneous disorders. Skin specimens from 72 PP and 61 MF patients were selected in this study. We compared their actual histological diagnosis with their previous diagnosis and we found that all 72 PP patients had the same diagnosis as before (stable PP), while 26 out of 61 MF had a previous PP histological diagnosis (evolving PP). Our results show an increased expression of HECA-452 in MF compared to PP (p<0.01). Furthermore, evolving PP showed a significantly higher level of HECA-452 than stable PP (p<0.05). We conclude that HECA-452 expression increases during the natural history of Mycosis Fungoides. HECA-452 could be used as a biomarker for MF and predict which PP evolves to MF.
