In vitro and in vivo activities of Syn2190, a novel beta-lactamase inhibitor.

Syn2190, a monobactam derivative containing 1,5-dihydroxy-4-pyridone as the C-3 side chain, is a potent inhibitor of group 1 beta-lactamase. The concentrations of inhibitor needed to reduce the initial rate of hydrolysis of substrate by 50% for Syn2190 against these enzymes were in the range of 0.002 to 0.01 microM. These values were 220- to 850-fold lower than those of tazobactam. Syn2190 showed in vitro synergy with ceftazidime and cefpirome. This synergy was dependent on the concentration of the inhibitor against group 1 beta-lactamase-producing strains, such as Pseudomonas aeruginosa, Enterobacter cloacae, Citrobacter freundii, and Morganella morganii. However, against beta-lactamase-derepressed mutants of P. aeruginosa, the MICs of ceftazidime plus Syn2190 were not affected by the amount of beta-lactamase, and the values were the same for the parent strains. The MICs at which 50% of isolates are inhibited (MIC(50)s) of ceftazidime plus Syn2190 were 2- to 16-fold lower than those of ceftazidime alone for ceftazidime-resistant, clinically isolated gram-negative bacteria. Similarly, the MIC(50)s of cefpirome plus Syn2190 were two- to eightfold lower for cefpirome-resistant clinical isolates. The synergies of Syn2190 plus ceftazidime or cefpirome observed in vitro were also reflected in vivo. Syn2190 improved the efficacies of both cephalosporins in both a murine systemic infection model with cephalosporin-resistant rods and urinary tract infection models with cephalosporin-resistant P. aeruginosa.

[Transient cortical blindness and convulsion mimicking a hemorrhagic complication during embolization of the cerebellar AVM].

We report a case of transient cortical blindness and convulsions during embolization of the cerebellar AVM. A 29-year-old man was admitted to our hospital with subarachnoid hemorrhage. Cerebral angiography showed the right cerebellar AVM fed by the right superior cerebellar artery. Preoperative embolization of the AVM was performed. During embolization, the patient lost consciousness suddenly and appeared to be in a decerebrate posture. About 15 minutes later, the patient came around and complained of total blindness. Cerebral angiography revealed the disappearance of the aneurysm and AVM. There were no abnormal findings in the vertebrobasilar system. Computed tomography obtained just after the procedure, showed considerable contrast enhancement of the occipital lobes. Two hours later, a repeat computed tomography showed clearance of contrast enhancement. Magnetic resonance imaging obtained 12 hours after the procedure, showed no evidence of infarction in the occipital lobes. Two days after, the patient's sight gradually returned. Seven days after the procedure, the patient recovered completely. We speculated that these clinical features might be convulsions due to contrast material. In the reported cases, convulsions and transient cortical blindness due to non-ionic low osmolar contrast materials is a rare complication. This case suggested disruption of the blood-brain barrier demonstrated by the computed tomography appearance of contrast enhancement in the occipital lobes.

The first component of polyphasic motor evoked potentials is resistant to suppression by paired transcranial magnetic stimulation in humans.

The suppressive effect of paired transcranial magnetic stimulation on the different components of motor evoked potentials (MEPs) from the left first dorsal interosseous muscle (FDI) was investigated in ten healthy human subjects. Conditioning stimuli were delivered via a round coil centered on the vertex, and test stimuli via a figure-eight coil over the right motor area. The threshold stimulus intensity of the minimally contracted FDI was determined with each coil. The stimulus intensities of conditioning and test stimuli were 80% and 130-140% of the threshold intensity, respectively. The coil current direction at the intersection of the figure-eight coil was medio-lateral. The coil position for test stimuli was adjusted to evoke discretely polyphasic MEPs in the contracted FDI. Conditioning stimuli were followed by test stimuli at intervals of 1-10, and 15 ms. When the interstimulus intervals were less than 5 ms, the second negative phase of MEPs was suppressed compared to that of the unconditioned responses, whereas the first phase was not suppressed. The present results imply that the first phase reflects direct corticospinal volleys, which are resistant to cortico-cortical inhibition.

Embolic materials for endovascular treatment of cerebral lesions.

Recently developed soft microcatheters can be maneuvered endovascularly into the brain, permitting treatment of lesions without conventional neurosurgery. Progress in biomaterial science has contributed significantly to the development of this new therapeutic modality termed intravascular neurosurgery or interventional neuroradiology. Although embolic materials play an important role, ideal materials have yet to be devised. Various embolic materials in clinical use are reviewed, such as cyanoacrylates, ethylene-vinyl alcohol copolymer mixtures, Ethibloc, ethanol, estrogen, poly(vinyl acetate), cellulose acetate polymer, poly(vinyl alcohol), gelatin sponges, microfibrillar collagen, surgical silk sutures, detachable balloons, and coils. The materials are reviewed in the context of treatment application for various brain lesions, such as arteriovenous malformations, cerebral aneurysms, and head and neck tumors. Further developments in biomaterial polymer science can bring about progress against brain diseases.

SYN-1012: a new beta-lactamase inhibitor of penem skeleton.

A new beta-lactamase inhibitor, SYN-1012, with a penem skeleton was synthesized and its biological activity compared with clavulanic acid, sulbactam, tazobactam and BRL-42715. The beta-lactamase inhibitory activity of SYN-1012 was comparable to BRL-42715. Clavulanate and penam sulphones (sulbactam and tazobactam) were more active against TEM-1 and OXA-1, but were less active against TEM-3 and cephalosporinase (Case) than SYN-1012. In combination with piperacillin, SYN-1012 exhibited comparable or slightly lower synergistic effects than BRL-42715 against all the Gram-positive and Gram-negative isolates tested with only exception of Pseudomonas aeruginosa. The separate combinations of SYN-1012 and BRL-42715 with ceftazidime and cefotaxime provided comparable results against Gram-negatives, but not against Gram-positive isolates. Tazobactam was inferior to SYN-1012 in all cases. In comparison to tazobactam, SYN-1012 and BRL-42715 were relatively unstable in human and mouse plasma, and in mouse liver and kidney homogenates. Serum level of SYN-1012 and BRL-42715 after an intravenous administration of 20 mg/kg in rabbit was undetectable after 1 hour.

[Development of beta-lactamase inhibitors].

The proportion of beta-lactamase producing bacteria in each species of clinical isolates is high (over 90%) in these years. According to the survey of bacterial resistance in 1995, higher proportion of resistant bacterial species against ampicillin, piperacillin (PIPC), cefazolin, cefotiam was observed. To overcome the bacterial resistance against these beta-lactam antibiotics, we have made a development of beta-lactamase inhibitor and its combination antibiotic. New beta-lactamase inhibitor, tazobactam (TAZ) showed strong inhibitory activity against various kinds of beta-lactamases including cephalosporinases. The combination antibiotic, TAZ/PIPC(TAZ combined with PIPC by 1 to 4) showed stronger antibacterial activity than PIPC against beta-lactamase producing stains. And also the activity of TAZ/PIPC was superior to PIPC in the mixed bacterial infection model in mouse. The in vitro and in vivo frequency of emergence of resistant bacteria from Enterobacteriacae treated with TAZ/PIPC was lower than that of PIPC or ceftazidime (CAZ). By these data, combined antibiotics with beta-lactamase inhibitor was effective to resolve the problems of bacterial resistances caused by beta-lactamases.

Bone transport using intramedullary fixation and a single flexible traction cable.

The transfixing wires used in the limb-lengthening and bone-transport procedures described by Ilizarov are the primary source of complications, including infection, restricted joint motion, and pain. In an attempt to eliminate the need for external fixation, a traction cable device was devised to perform a 3-cm bone transport in the canine femur using an interlocking intramedullary rod for fixation. Twelve dogs were divided into 2 groups. Transport began 7 days after osteotomy and continued at either 1 mm per day (n = 6) or 2 mm per day (n = 6). The in vivo peak force, resting force, and stiffness of the transport segment increased throughout distraction to a mean of 150 Newtons, 58 N, and 58 N per millimeter, respectively, at the time of docking. The regenerate and docking sites united in 10 of 12 dogs. Failure of the regenerate associated with deep infection around the rod occurred in 2 animals (17%). Modifications of this technique to reduce the risk of infection are discussed. This study confirms that intramedullary fixation is compatible with distraction osteogenesis and shows that a single traction cable can provide the force necessary for bone transport without transfixing wires in this setting.

Evaluation of collagen ceramic composite graft materials in a spinal fusion model.

Autogenous bone graft is highly effective in inducing a bone healing response in most clinical settings. However, significant morbidity can occur related to the harvest of an autograft. This makes the development of synthetic or purified nontissue bone grafting materials highly desirable. Both purified bovine Type I collagen and calcium phosphate ceramics have been proposed as promising osteoconductive bone graft substitute materials. One collagen ceramic composite, Collagraft, is approved for use in acute long bone fractures. This study evaluated composites of purified bovine Type I fibrillar collagen and a granular biphasic hydroxyapatite/tricalcium phosphate ceramic in the posterior segmental canine spinal fusion model. Materials were compared based on union score and mechanical testing in 3 separate fusion sites (L1-2, L3-4, L5-6). All composites were found to be inferior in union score to an equal volume of autogenous cancellous bone. In addition, the combination of the collagen ceramic composite with autogenous cancellous bone graft reduced the effectiveness of the autogenous bone graft significantly. These data should be a caution to the clinician who may consider use of collagen ceramic composites similar to Collagraft for spinal fusion applications.

Application of thermosensitive polymers as a new embolic material for intravascular neurosurgery.

Application of thermosensitive polymers as an embolic material for intravascular neurosurgery was investigated. We intended to use thermosensitive polymers to occlude vessels by precipitation in response to body temperature. Copolymers of N-isopropylacrylamide (NIPAM) and N-n-propylacrylamide (NPAM) were selected as thermosensitive polymers. To determine the optimal lower critical soluble temperature (LCST) for the embolic material, we developed an in vitro flow model. In this study the copolymers with an LCST of 24-26 degrees C showed appropriate precipitation. To prove the occlusion of vessels in vivo, we injected the copolymers into a rabbit kidney through a microcatheter. The extent of embolization was judged by angiography and histological examination. An acute toxicity test of the copolymer of NIPAM and NPAM was performed in comparison with that of the NIPAM monomer. The copolymer used in this paper showed no acute toxicity in mice. Water solubility, non-adhesiveness, and non-toxicity are the advantages of the use of thermosensitive polymers as an embolic material. By changing the LCST, various embolic materials can be designed. Based on our results, we believe that the application of thermosensitive polymers as a new embolic material is very promising.

Brainstem arteriovenous malformation with a pedicle aneurysm treated by endovascular surgery and proton-beam radiosurgery--case report.

A 39-year-old male presented with a small pontine hemorrhage and subarachnoid hemorrhage. Angiography showed a small left pontine arteriovenous malformation (AVM) associated with a small aneurysm on the pedicle feeding the AVM. The pedicle aneurysm was occluded by microcoils. The AVM was then treated by proton-beam radiosurgery. Follow-up angiography 2 years later revealed that the AVM had disappeared completely without neurological deficit. The combination of embolization and proton-beam radiosurgery was curative in this patient with a pontine AVM associated with an aneurysm on the feeding artery, showing that these techniques can be used to treat inoperable vascular lesions safely.

Correlation of histopathological factor of brain tumor and high thallium-201 uptake in single photon emission computed tomography.

This study was undertaken to investigate the relationship between several histological features and the degree of TI-201 uptake in brain tumors. T1-201 SPECT was performed on 52 patients with intracranial lesions. Histological examinations were carried out to determine the gradation of tumor cell density, vascularization, small-cell density, and matrix loosening, and the presence of necrosis, atypia, mitoses, and endothelial proliferation. The histological findings were classified into three categories. While the early uptake of T1-201 depended on the degree of necrosis of glial tumor, the delayed T1-201 uptake was closely related to the degree of necrosis, tumor cell density, and small-cell density, and may thus be of value for estimating the viability and degree of malignancy of glial tumors. In the non-glial tumors, the early and delayed T1-201 uptakes were closely related to all histological parameters, with the increase of necrosis and vascularization in the tumor tissue being particularly closely related to high uptake levels.

Leukoencephalopathy associated with intra-arterial ACNU in patients with gliomas.

Thirty cases of gliomas treated by surgery, radiotherapy and intra-arterial (IA) ACNU were reviewed with a focus on the late side-effect known as leukoencephalopathy. All cases were classified into three groups; remission (10 cases), regrowth (15 cases) and leukoencephalopathy (5 cases) from their outcome. The average total doses of IA ACNU were 49.8 mg/sqm body surface area in the remission group, 157.3 mg/sqm in the regrowth group and 203.1 mg/sqm in the leukoencephalopathy group. There were significant differences in the total IA ACNU doses between the remission group and both regrowth and leukoencephalopathy groups, while no significant differences were noticed in the dose of radiation given. There was a correlation between the total dose of IA ACNU and the occurrence of leukoencephalopathy. An autopsy of a typical case of leukoencephalopathy revealed various degrees of myelin breakdown and thickening of arterial walls, which probably manifested progressive dementia accompanied by urinary incontinence and gait disturbance.

Evaluation of a pressure sore model using monoplegic pigs.

This study evaluated a newly developed swine monoplegic pressure sore model by testing the uniformity of the initial wound size. The natural healing also was evaluated. Nine minipigs were used. The hindlimb was denervated by transecting the unilateral nerve roots L1 through S2. At 5 to 14 days, 800 mmHg of pressure was applied to the denervated skin over the trochanteric area with a 3-cm-diameter disk compressing the skin and subcutaneous tissue against the underlying bone for 48 hours. The wound was then debrided of devascularized tissue. Wound surface areas were calculated from photographs. Wound volumes were obtained by measuring the volume of saline needed to fill the wound defect. Full-thickness sores of uniform size developed in all pigs without mortality or complications. Wound surface area and volume increased initially, peaked, and then decreased in an exponential fashion. This model provides a new tool for direct comparison of pressure sore treatment modalities for short- and long-term studies.

TOC-39, a novel parenteral broad-spectrum cephalosporin with excellent activity against methicillin-resistant Staphylococcus aureus.

TOC-39, a new parenteral cephalosporin, is a hydroxyimino-type cephem antibiotic with vinylthio-pyridyl moiety at the 3 position. TOC-39 was evaluated for antibacterial activity against various clinically isolated strains. TOC-39 had excellent activity, stronger than that of methicillin, oxacillin, the cephalosporins tested, imipenem, gentamicin, minocycline, tobramycin, ofloxacin, and ciprofloxacin against methicillin-resistant Staphylococcus aureus (MRSA) and had an MIC comparable to that of vancomycin (the MICs of TOC-39 and vancomycin for 90% of the strains tested were 3.13 and 1.56 micrograms/ml, respectively). Against Enterococcus faecalis strains, which are resistant to cephalosporins, TOC-39 was twice as active as ampicillin. Against methicillin-susceptible S. aureus, coagulase-negative Staphylococcus spp., and Streptococcus pneumoniae, TOC-39 was twice as active as or more active than cefotiam, ceftazidime, flomoxef, and cefpirome. Against Streptococcus pyogenes, TOC-39 was superior to cefotiam, ceftazidime, and flomoxef and was similar to cefpirome. In addition, the activity of TOC-39 was equal to or greater than that of cefotiam, ceftazidime, flomoxef, and cefpirome against Haemophilus influenzae, Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Morganella morganii. In terms of bactericidal effect against MRSA, TOC-39 was superior to vancomycin. No mutant resistant to TOC-39 or vancomycin was obtained from susceptible MRSA strains. In murine systemic infection models, TOC-39 showed potent activity against S. aureus and E. coli. Against highly MRSA, the activity of TOC-39 was comparable to that of vancomycin.

Decrease in surface charge density of Klebsiella pneumoniae treated with cefodizime and enhancement of the phagocytic function of human polymorphonuclear leucocytes stimulated by the drug-treated bacteria.

Treatment of Klebsiella pneumoniae 109 with cefodizime, ceftazidime, cefbuperazone, cefotaxime or flomoxef at a sub-minimum inhibitory concentration (sub-MIC) (1/4 MIC) for 1 h altered its morphology. The bacteria treated with cefodizime at sub-MICs (1/8, 1/4 MIC) enhanced the chemiluminescencence (CL) of human polymorphonuclear leucocytes (PMNs), implying an increase in the production of active oxygen species in association with phagocytosis, whereas the cells treated with other cephalosporins at the same sub-MICs did not. Furthermore, a significant decrease in electrophoretic mobility was induced by the bacteria treated with cefodizime at sub-MICs (1/8, 1/4 MIC), but other cephalosporins neither increased nor decreased the electrophoretic mobility significantly. These findings suggested that cefodizime caused a morphological change, with a decrease in negative surface charge density of K. pneumoniae, more easily than ceftazidime, cefbuperazone, cefotaxime or flomoxef, followed by an increase in the phagocytic activity of PMNs.
