Lithium carbonate and imipramine in the prophylaxis of unipolar and bipolar II illness: a prospective, placebo-controlled comparison.

Twenty-seven patients with recurrent unipolar depression and 22 with bipolar II illness in remission for at least six months were randomly assigned on a double-blind basis to treatment regimens using lithium carbonate, imipramine hydrochloride, lithium carbonate plus imipramine, or placebo. Lithium carbonate was found to help prevent depressive relapse among patients with unipolar disease, and relapse of any type among those with bipolar II disease. No effect or interaction of imipramine was found in either group. These results add to a growing body of data that suggest the usefulness of lithium carbonate in the prophylaxis of unipolar depressive illness. The relative usefulness of lithium carbonate and imipramine requires further study.

C-reactive protein in juvenile chronic arthritis: an indicator of disease activity and possibly amyloidosis.

C-reactive protein (CRP) was found to be particularly high early in the course of those cases of juvenile chronic arthritis (JCA) with a systemic onset, the mean level being 12 mg/dl (120 mg/l). It was also raised in cases with a polyarticular onset, mean level 6 mg/dl, while in cases with a pauciarticular onset it was associated with only a modest increase up to 1.5 mg/dl (15 mg/l). At the onset of disease there was a good correlation with the erythrocyte sedimentation rate (ESR). Regression of systemic disease was associated with a steady fall in C-reactive protein, but those patients who developed amyloidosis within 5 years from onset had persistently high values until cytotoxic therapy was introduced. Patients who developed amyloidosis later tended to have high CRP levels in the months or even years before diagnosis. In a few patients with polyarthritis the CRP appeared to reflect severe disease more closely than their relatively low ESR.

Monoamine oxidase inhibitors in bipolar endogenous depressives.

Clinical lore suggests that monoamine oxidase inhibitors (MAOIs) are not effective in endogenous depression. A review of previous placebo-controlled trials of MAOI in patients with endogenous depression neither refutes nor confirms their utility in this patient group. We present patients in the depressive phase of bipolar illness refractory to treatment with tricyclics who have responded to MAOI. These open trials require confirmation in controlled studies. Bipolar illness may be a heterogeneous disorder. Presence or absence of X-linkage, low platelet MAO, or response to MAOI may indicate different forms of the disorder.

Grouping of beta-haemolytic streptococci by agglutination.

Streptococcal grouping sera, diluted and absorbed to remove cross-reactions, were bound to staphylococci and used to group trypsinised beta-haemolytic streptococci by coagglutination. The results compared well with those obtained using the Phadebact streptococcal grouping kit. The same sera bound to staphylococci without prior dilution and absorption could be used to group enzyme extracts of haemolytic streptococci by slide agglutination, the results again comparing favourably with those of the Phadebact kit.

Mixed connective tissue disease in children.

A diagnosis of Mixed Connective Tissue Disease (MCTD) had been made in five juveniles in the past two years. All have antibodies to the ribonucleoprotein (RNP) component of extractable fluorescence pattern. No child has had life-threatening complications, though three have required steroids to control their symptoms. Antibodies to ENA were looked for in the other sub-groups of Juvenile Chronic Arthritis (JCA); only two of 75 patients with chronic iridocyclitis and antinuclear antibodies had weakly positive tests.

Escherichia coli serotypes throughout the gastrointestinal tract of patients with intestinal disorders.

The O and H serotypes of Escherichia coli that were present along the entire length of the gastrointestinal tract of patients with small intestinal bacterial overgrowth were studied. Multiple sero- and biotypes were represented, although usually a single serotype predominated in each patient. In a number of cases the different O:H serotypes were antigenically related indicating that antigenic degradation was occurring. The serotypes isolated from the stomach and small intestine were represented in the faeces. In general, within the limitations of this study, there appears to be a stable ecosystem in each patient and it may require specific oral antibiotics to alter it.

Evaluating outcome criteria used in methadone maintenance programs.

This report reviews the results of several large methadone maintenance programs and analyzes the outcome measures used. Criteria measures were often vague and sometimes not defined. Many studies base their findings on unverified patient self-report because costs for more reliable and systematic data collection are prohibitive. Available data does not allow for resolution of the pro- or antimethadone maintenance views for treatment of heroin addiction.

Anthropological perspectives on population growth.

Rapid population growth affects many different system levels simultaneously: the individual, the family, the community, the nation-state and the world. What may appear to be an optimal population level for one system, given certain value premises, may not be optimal for another. Anthropologists can contribute to an understanding of rapid growth by generating an appreciation for the complexity of the phenomenon, and by providing representation for a range of cultural viewpoints. This may help to reduce the risk we now run of committing all of mankind to a singular path which may prove to be an evolutionary dead end.
