Decreased Th1-like and increased Th2-like cells in systemic lupus erythematosus.

The proportion of the lymphocytes which produce the cytokines corresponding to murine T helper- (Th1) or Th2 cells was studied using flow cytometry in systemic lupus erythematosus (SLE). When the peripheral mononuclear cells were stimulated with phorbol myristate acetate and ionomycin in the presence of monensin, which blocks the secretion of cytokines, the positive rates for the cytoplasmic IL-2 and IFN-gamma were lower and those for the cytoplasmic IL-4 and IL-10 were higher in SLE than in normal subjects. When the cells were cultured with monensin alone, the positive rates for these 4 cytokines were slightly increased in SLE. These data suggest that the mononuclear cells are already activated in vivo and a deviation of the proportion of the Th cells to the Th2-like ones might be associated with the polyclonal B cell activation seen in SLE.

Anti-proliferative effects of unmodified antisense oligodeoxynucleotides targeted against c-raf mRNA: use of poly (lysine/serine) copolymers or cationic lipopolyamines.

1. It is now known that nuclease-resistant phosphorothioate antisense oligodeoxynucleotides (ODN) have some actions that are unrelated to antisense mechanisms. In the present study we assessed the anti-proliferative effects of phosphorothioate (PS) and phosphodiester (PO; unmodified) antisense ODN targeted against c-raf mRNA on pancreatic cancer cells in vitro, using poly (lysine/serine) copolymers conjugated with polyethylene glycol (PLSP) or cationic lipopolyamines (Transfectam) as carriers. 2. The anti-proliferative effect of the PO antisense ODN was significantly (P < 0.05) greater than that of the PS ODN, either complexed with PLSP (2 mumol/L ODN) or the Transfectam (0.5 mumol/L ODN). However, the effect of the PS or PO antisense ODN was not dependent on the antisense sequence. The c-raf mRNA levels, assessed by reverse transcription-polymerase chain reaction, were obviously reduced by both PO and PS antisense ODN compared with mismatched ODN when complexed with the Transfectam (1 mumol/L ODN). 3. Although the anti-proliferative effects were mainly unrelated to antisense mechanisms, unmodified antisense ODN complexed with some carriers could be used as anti-tumour agents considering that synthetic carriers can be modified to improve functions, such as delivery.

Case report: Pneumatosis cystoides intestinalis associated with post-surgical bowel anastomosis: a report of three cases and review of the Japanese literature.

We report three cases of pneumatosis cystoides intestinalis (PCI) occurring in association with post-surgical bowel anastomosis. A 74-year-old man, a 58-year-old woman, and a 62-year-old woman were found to have PCI at the colonic side of a bowel anastomosis at 4 years, 3 years and 1 year after operation, respectively, for right colon carcinoma, although all were asymptomatic. They all had a positive anti-nuclear antibody test and had received postoperative cancer chemotherapy. The clinical features of 123 cases of PCI reported in Japan between 1981 and 1995 were also reviewed. On the basis of the present and previous cases, we propose that post-surgical anastomosis, cancer chemotherapy, and predisposition to collagen vascular disease might be responsible for the damage to intestinal mucosa that leads to the development of PCI.

Characteristic pancreatic duct appearance in autoimmune chronic pancreatitis: a case report and review of the Japanese literature.

We report a case demonstrating the progressive narrowing of the pancreatic duct, which is presumed to be characteristic of autoimmune pancreatitis, and we review the 37 cases of chronic pancreatitis in which autoimmunity was suggested as an etiological factor in the Japanese literature. A 55-year-old man presented with abdominal discomfort, jaundice, and diffuse swelling of the pancreas on ultrasonography. Serial endoscopic retrograde pancreatography demonstrated the progression of an irregular narrowing of the main pancreatic duct forming diffusely over the course of 2 months. Because the patient had hyperglobulinemia and tested positive for autoantibodies, he was diagnosed as a case of autoimmune chronic pancreatitis. Steroid therapy was carried out with excellent success.

[Clinical study of itraconazole for systemic fungal infections complicated with hematological malignancies--multicenter cooperative study. West-Japan Systemic Fungal Infection Study Group].

Itraconazole, an oral antifungal agent, was evaluated for the clinical efficacy and safety in patients documented or suspected of systemic fungal infection (SFI) complicated with hematological malignancies. The data was also evaluated according to serological tests and fungal culture. Out of a total of 79 patients, the clinical efficacy of itraconazole was evaluated in 52 patients, comprising 4 patients with proven SFI, 33 with clinical SFI, and 15 with suspected SFI. The overall efficacy was 67.3% (35/52), in which 25.0% in the established SFI group (1/4), 78.8% in the clinical SFI group (26.33), and 53.3% in the suspected SFI (8/15). When assessed according to the severity of SFI, the effectiveness rates were 75.0% in the mild group (15/20), 76.2% in the moderate group (16/21), and 36.4% in the severe group (4/11), resulting in significantly higher rates in the mild and the moderate groups than in the severe group (p = 0.0479). Out of the 10 patients who were switched from the previous antifungals, 7 patients were judged as showing marked or good responses. All of these patients were switched from intravenous fluconazole therapy. With respect to the safety of itraconazole, among 79 patients eligible for safety evaluation, adverse events such as hepatic dysfunction, increase in GOT and/or GPT, nausea and vomiting, and chest pain were observed in 6 patients (7.6%), and itraconazole may have been associated with them. As a conclusion, itraconazole proved to be effective and safe on SFI developed in patients with various hematological malignancies.

Anti-glomerular basement membrane nephritis after extracorporeal shock wave lithotripsy.

Rapidly progressive glomerulonephritis was observed in a 37-year-old woman following the administration of extracorporeal shock wave lithotripsy (ESWL) for a single stone in her right kidney. The renal biopsy specimen showed diffuse cellular crescents in all glomeruli, with linear deposits of immunoglobulin G and complement component C3 along the glomerular basement membrane (GBM). Circulating anti-GBM antibodies were detected by enzyme-linked immunosorbent assay. Thus, the patient was diagnosed with anti-GBM nephritis. It is suggested that ESWL produced an alteration in the GBM leading to the production of anti-GBM antibodies.

Thiol compounds rescue growth inhibition by retinoic acid on HTLV-I (+) T lymphocytes; possible mechanism of retinoic-acid-induced growth inhibition of adult T-cell leukemia cells.

We demonstrated significant growth inhibition by retinoic acid (RA) of HTLV-I (+) T-cell lines (ATL-2 and HUT102), but not HTLV-I (-) T-cell lines (MOLT-4 and Jurkat). We hypothesized that the mechanism of growth inhibition by RA depends on an imbalance in redox potential. To examine the effect of exogenous thiol compounds for the growth of HTLV-I (+) T-cell lines by RA, HTLV-I (+) T-cell lines were cultured with several thiol compounds (thioredoxin, L-cystine, and GSH), following addition of 13-cis RA or ATRA, respectively, in cultured with thiol free medium. Unexpectedly, thiol compounds alone did not restore growth inhibition of HTLV-I (+) T-cell lines. However, when those cells were preincubated with thiol compounds for 24 hours, no growth inhibition by 13-cis RA or ATRA was observed. These results suggest that thiol compounds are associated strongly with sensitivity to RA of HTLV-I (+) T cells, but not of HTLV-I (-) T cells and that thiol compounds serve an important role on HTLV-I (+) T cells.

Reduced expression of calponin h1 in leiomyosarcoma of the uterus.

To determine the genetic differences between various smooth muscle tumors of the uterus, the expression of calponin h1 (cytoskeletal protein) was examined in normal myometrium and in smooth muscle tumors of the uterus (leiomyosarcoma, smooth muscle tumors of uncertain malignant potential, cellular leiomyoma, bizarre leiomyoma, and ordinary leiomyoma) using immunohistochemistry and Western blotting. Analysis of calponin h1 transcripts using the reverse-transcription-PCR was also performed. Immunohistochemically, calponin h1 was expressed in normal myometrium and in all leiomyomas; however, its expression was markedly weaker in leiomyosarcoma. The results of both Western blotting and the analysis of calponin h1 transcripts were compatible with the immunohistochemical results. It is suggested that reduced expression of calponin h1 is associated with leiomyosarcoma of the uterus, and that calponin h1 expression may serve as a valuable molecular maker in the differential diagnosis of smooth muscle tumors of the uterus.

Site-directed mutagenesis of amino acid residues of protein phosphatase 1 involved in catalysis and inhibitor binding.

Site-directed mutagenesis of selected residues of mammalian protein phosphatase 1 (PP-1) has been carried out to further define the mechanism of catalysis, activation by divalent cations, and inhibition by toxins and inhibitory proteins. Mutation of active site residues predicted to bind metals (N124D and H248N) resulted in a large loss of enzyme activity and decreased affinity for metal ions; mutation of residues predicted to bind phosphosubstrate (R96A or R221S) led to a large loss of enzyme activity; and mutation of active site residues (D95A and D208A) resulted in a large loss of enzyme activity. Mutants N124D, H248N, R96A, and R221S exhibited large decreases in sensitivity to the toxins calyculin A, okadaic acid, and microcystin and to thiophospho-DARPP-32. Mutation of Y272 (Y272F) had little effect on activity but resulted in a large decrease in sensitivity to okadaic acid and calyculin A. Mutant D208A exhibited a decrease in sensitivity to okadaic acid and calyculin A, but, paradoxically, the sensitivity to inhibition by thiophospho-DARPP-32 was increased. Mutation of acidic groove residues (E256R, E275R, E252A:D253A, and E252A:D253A:E256R) exhibited little change in enzyme activity and no change in sensitivity to toxins, but increased sensitivity to thiophospho-DARPP-32. These results suggest that toxins and phospho-DARPP-32 interact at the active site of PP-1 in a similar fashion despite their differences in structure. In addition, acidic groove residues appear to influence the interaction of the phosphoinhibitor with the active site of PP-1.

Immunochemical fecal occult blood test is not suitable for diagnosis of hemorrhoids.

PURPOSE: This study was conducted to clarify the diagnostic value of an immunochemical fecal occult blood test for hemorrhoids. PATIENTS AND METHODS: In a case-control study, an immunochemical fecal occult blood test with a 2-day method was carried out on 82 subjects with hemorrhoids, on 82 subjects with colorectal cancer, and on 82 healthy subjects. In a population-based cross-sectional study, 29,714 subjects who received an immunochemical occult blood screening with a 2-day method were divided into two groups, according to the results of a questionnaire on hemorrhoids, and the positivity rate of an immunochemical test as well as the predictive value for colorectal cancer were compared in the two groups. Moreover, both an immunochemical occult blood test with a 2-day method and colonoscopy were conducted at the same time on asymptomatic subjects during a medical checkup. RESULTS: In the case-control study, the test was positive in 13.4% subjects with hemorrhoids, in 84.1% subjects with colorectal cancer, and in 4.9% healthy subjects, respectively, showing a significant difference in the detection rate between the two diseases (P < 0.001). In the population screening program, the test was positive in 6.9% subjects with hemorrhoids and in 6.5% subjects without hemorrhoids, and the predictive value was 3.2% in subjects with and without hemorrhoids, respectively, indicating no significant difference in the positivity rate as well as the predictive value between the two groups. Among 232 subjects in a medical checkup, 28 patients with hemorrhoids and 21 patients with colorectal polyp 1 cm or larger were diagnosed by colonoscopy, and the occult blood test was positive in 16.7% patients with hemorrhoids and in 52.4% patients with colorectal polyp, respectively. There was a significant difference in the sensitivity between the two disease groups (P < 0.05). CONCLUSIONS: These findings indicate that the immunochemical fecal occult blood is unsuitable for the diagnosis of the patients with hemorrhoids and an examination of the colorectum is necessary in cases where the occult blood test is positive but there is a sign of hemorrhoids.

Soluble intercellular adhesion molecule-1 levels of patients with acute myeloid leukemia after allogeneic bone marrow transplantation.

Concentrations of serum-soluble intercellular adhesion molecule-1 (ICAM-1) were measured for 10 patients with or without chronic graft versus host disease after allogeneic bone marrow transplantation. Levels of soluble ICAM-1 were higher among patients with chronic graft versus host disease than among those without it, a statistically significant difference. The results indicated that measurement of serum-soluble ICAM-1 is useful for prediction of chronic graft versus host disease.

Clinical diagnostic accuracy of faecal occult blood test for anal diseases.

PURPOSE: To investigate the diagnostic accuracy of faecal occult blood test (FOBT) for anal diseases and to evaluate the relationship of this disorder to the results of FOBT. METHODS: In a hospital-based retrospective study, FOBT using faecal samples collected on two consecutive days was performed on patients with anal disease, such as internal as well as external haemorrhoids and anal fissure, patients with colorectal cancer and healthy subjects to evaluate the diagnostic accuracy of FOBT. In a screening-programme-based cross-sectional study, the subjects who underwent colorectal cancer screening by FOBT using the 2-day method were divided into two groups according to the results of a questionnaire on anal diseases; the positive rate of FOBT as well as the predictive value for colorectal cancer were determined in the two groups. RESULTS: Among 300 cases, including 100 anal diseases, 100 colorectal cancers and 100 healthy subjects, the test was positive in 12 anal diseases, 82 colorectal cancers and six healthy subjects. The sensitivity to anal diseases and colorectal cancer was calculated to be 12% and 82%, respectively, and the specificity was 95%. A significant difference was noted in the sensitivity between anal diseases and colorectal cancer (P < 0.001). Among 21,724 subjects, who underwent colorectal cancer screening, 2395 subjects were positive for anal diseases, and 19,329 subjects were negative. The positive rate of FOBT was 7.2 for the anal diseases group and 6.8 for the non-anal diseases group, and the predictive value for colorectal cancer was 2.9 and 3.0, indicating no substantial difference between the two groups. CONCLUSIONS: These results reveal that FOBT is inferior for the diagnosis of anal diseases, and anal diseases have little relationship to the results of FOBT.

Effects of a GnRH analogue on human smooth muscle cells cultured from normal myometrial and from uterine leiomyomal tissues.

Leiomyomas are tumours of uterine smooth muscle tissue that are oestrogen and progesterone dependent. When explants of these tumours were grown in culture, the proliferating tissue formed characteristic ball-like aggregates (BLA), rather than the usual hill and valley (HV) pattern of growth of normal myometrial tissue in culture. Immunocytochemical staining with fluorescein isothiocyanate (FITC)-labelled gonadotrophin-releasing hormone (GnRH) revealed that both myometrial and leiomyomal cells have membrane receptors for this hypothalamic releasing hormone. Furthermore, polymerase chain reactions (PCR) with primer sets that were specific for GnRH receptor mRNA, as well as GnRH mRNA, showed that transcripts for both of these nucleic acids are present in myometrial and leiomyomal tissues. The treatment of cultured explants of leiomyomal tissue with a GnRH analogue (buserelin, HOE766) diminished the formation of BLA, but this synthetic hormone had only a moderate effect on the HV topography of normal myometrial tissue. A colorimetric assay indicated that GnRHa inhibited cell proliferation in leiomyomal tissue in a dose-dependent manner. Western blotting, to detect the expression of G1 phase cell cycle-related gene products, showed that cyclin E and p33cdk2 formation in leiomyomas were inhibited by high concentrations of GnRHa. In conclusion, GnRHa might suppress leiomyomal growth by interfering with the expression of cell cycle factors.

Case report: a case of primary ileal carcinoma in a young woman and a review of the Japanese literature.

We report a case of primary ileal carcinoma in a young woman, which was diagnosed definitively before operation. A 29-year-old woman presented with abdominal pain, diarrhoea and bloody stools. Colonoscopic and radiographic studies revealed that there was a 7.5 x 7 cm tumour (well-differentiated adenocarcinoma) at the terminal ileum, forming an ulcerated lesion at the centre. The tumour had invaded the caecum, the right urinary tract, the right ovary and a portion of the sigmoid colon. Fifty-three cases of primary ileal carcinoma were reported in Japan between 1982 and 1994 and their clinical features are reviewed herein.
