[Evaluation of the results of endoscopic internal urethrotomy in the treatment of urethral stricture. Apropos of 163 controlled cases].

Results are reported of 294 internal urethrotomies performed over 5 years to treat 196 urethral strictures. In this series the predominant etiology was sclero-inflammatory (55% of cases) but there was also a fairly high incidence of iatrogenic (52 cases) and post-traumatic (30 cases) lesions. The length of the narrowed segment varied between 0.5 and 3 cm, the most frequent site the bulbar region. Results at 1-year follow up or longer were assessed by quality of micturition and flowmeter curve. Of the 163 cases controlled good results exceeded 60% of patients treated by this method. Mastery of internal urethrotomy has allowed almost complete elimination of blind instrumental dilatation, and has markedly decreased number of urethroplasties since adoption of this therapeutic procedure.

[Echographic aspects of fused kidneys. Apropos of 2 cases].

Coalescent kidney is a rare variety of fused kidneys, clinical manifestations observed with the greatest frequency being a more or less painful abdominal mass, digestive disturbances and a complicating infection, calculus, renal failure or hematuria. Straight abdominal films (SAF), ultrasound tomography, SAF after colon insufflation and IVU can usually establish the diagnosis. Results of ultrasound imaging of this rare variety of ectopic kidney have apparently not been published. Apart from any complicating factor, the coalescent kidney corresponds to a discoid shaped subumbilical prespinal mass of oblong section. It appears as two highly echogenic central zones encircled by a less dense zone. No separation exists between the right and left portion, and furthermore a kidney cannot be identified in either lumbar fossa. These findings are useful for identification of a retroperitoneal mass, ultrasound imaging being of importance when IVU supplies insufficient data.

[Total hematuria with normal intravenous urography. Apropos of 36 cases].

The authors present a critical study and retrospective of 36 cases of hematuria without urographic anomalies. Renal angiography was performed in 69% of the patients. 84% of these angiograms were negative. Only 2 identified the cause of the bleeding. 28% of the patients underwent disappointing surgery. In 40% of the cases the appearance of the kidney was normal. Only one of the 6 nephrectomies was justified. The authors recommend the use of modern and non invasive methods of investigation.

[Multilocular cyst of the kidney: diagnostic problems. Apropos of a case in an adult].

The authors report a case of multilocular cyst of the kidney in an adult. The diagnosis was made, despite the fact that hydatid disease was endemic in the area, on the basis of the finding at urography of a renal mass with the density of water, the finding at echotomography of a multiloculated mass of mixed nature and finally the absence of any blood supply at arteriography. Excision of the multilocular mass left in place a normal kidney with a good result confirmed after 18 months' follow-up. This case is used as a basis to describe the differential diagnosis in the context of the results of special investigations: I.V.U., echotomography, CT scan and arteriography. Diagnoses which must be eliminated are: renal adenocarcinoma in its necrosed form, solitary renal cyst, angiomyolipoma, nephroblastoma and, in endemic areas obviously, hydatid cyst, in particular in its multivesicular form. Despite the multiplicity of investigations, the diagnosis is often uncertain until the time of surgery. Treatment is excision of the abnormal mass with preservation of healthy parenchyma.

Angiography↗

[Contribution of echotomography in the diagnosis of hydatid cysts of the kidney. Apropos of 43 confirmed cases].

Hydatid cysts of the kidneys were explored by ultrasound in 43 patients between 1978 and 1982, diagnosis being confirmed by operation. Cases included 33 adults and 10 children under 15 years. Ultrasound images of renal hydatid cysts are analyzed, and correlated with the classification into 5 types proposed in the literature and based on their natural history. This non-invasive technique, when associated with intravenous urography, is of primary diagnostic importance, particularly in countries where these cysts are endemic. Ultrasound imaging assists positive diagnosis in a large number of cases, acts as a guide to complementary explorations when these are required, and enables detection of extrarenal locations of hydatidosis. Differential diagnosis of ultrasound images is discussed, and emphasis placed on the advantages of using this technique in countries where the affection is endemic, particularly its low cost.

Effects of ten milligrams of ampicillin per day on urinary tract infections.

A total of 20 patients with symptomatic urinary tract infections (greater than 10(5) colony-forming units of Escherichia coli per ml of urine in addition to pyuria) received 10 mg of ampicillin and 2 liters of fluid daily for 3 days. After 2 days, 16 patients had culture-negative urine without pyuria. A total of 18 similar patients received only 2 liters of fluid for 3 days. On day 4, all had greater than 10(5) colony-forming units of E. coli per ml of urine and greater than 10(4) leukocytes per mm3 of urine.
