In France, 30% of adults smoke, and males smoke a little bit more than females. Young people smoke more and more, and 50% of 18-24 years-old males and females smoke. Tobacco causes 60,000 deaths each year in France. Publicity is now forbidden (Evins's law). Price of cigarette has been multiplied by two. Interdiction of smoking in public and closed places is however insufficiently respected. Budget devoted to information, education, and prevention, is also dramatically insufficient. By contrast, due to vigilance of the Comité National Contre le Tabagisme, tobacco sales decreased from 1991 to 1997 of 11.1%, and cigarette sales of 14.5%. French situation must be replaced in European and international context. European regulation on smoking interdiction increases the impact of the French legislation.
All cases of primary pleural cancers that were reported during a 10-y period (i.e., 1978-1987) in New Caledonia were recorded and compared with five cancer registers. There was an excess of primary pleural cancers in the area of New Caledonia, and it was particularly evident among the Melanesian ethnic group. Neither geographical nor occupational factors accounted for these results, but an environmental factor could not be excluded.

Stereotactic radiosurgery was used in the treatment of 126 patients with acoustic neurinomas up to 30 mm in diameter from 1969 to 1984. Adequate follow-up data (mean follow-up period 4.7 years) were available for 111 (116 ears) of these 126 patients; of these 111 patients, 64 (65 ears) had a pure-tone threshold of less than 90 dB before the treatment and were followed up audiologically. Preserved hearing was found in 26% of the ears one year postoperatively. Shrinkage of the tumour was obtained in 44% and arrest of its growth in 42%. There was no mortality related to the radiosurgical treatment. Transitory facial weakness was noted in 15% of the patients (3% in 1983-84). Eighteen per cent of the patients had some, usually transitory, trigeminal dysfunction. The stapedius reflex threshold was improved in 13 ears (20%). In one patient the audiological tests became pathological in the contralateral ear during growth of a new tumour. Initially the stapedius reflex threshold was elevated, and 11 months later the BRA pattern also became abnormal.
The commonly used parameters of the stapedius reflex responses in the diagnosis of sensorineural hearing disorders are the reflex threshold level and the persistence of the reflex response on prolonged stimulation, i.e., the reflex decay test. With the object of establishing the sensitivity of these reflex parameters to identify tumours affecting the eighth nerve in the early stages, a series of 97 such cases were examined. The mean hearing loss of the affected ears was 32 dB; 28 of the cases still had normal thresholds of hearing; 95 of the ears demonstrated elevated reflex thresholds and/or pathologic reflex decay. To check the specificity of the test, a control series with sensorineural hearing loss and reflex threshold elevation or pathologic reflex decay was analysed with respect to clinical diagnoses. In every third case of reflex threshold elevation and in every second case of reflex decay, the medical diagnosis confirmed a disease known to be associated with retrocochlear dysfunction. It was also noted that none of the cases with brain stem lesion demonstrated reflex decay--an observation which supports the idea of the reflex decay phenomenon as specifically associated with eighth-nerve lesions.
