[Misdiagnosis of psychic symptoms of hyperthyroidism in affective psychosis].

A 71-year-old woman known to have suffered from a depressive illness for fifty years developed further psychological and somatic symptoms 18 months ago (agitation, increased impulsiveness, depressive mood, decreased appetite, diarrhoea and weight loss). Within five days there occurred nocturnal confusion with disordered word finding and counting. Biochemical tests indicated hyperthyroidism and scintigraphy showed multifocal activity increase. The psychological and somatic symptoms responded to thyrostatic treatment with carbimazole, 30 mg daily. This case demonstrates that added somatic causes of psychological symptoms should be thought of in previously known psychiatric illness.

Stress-related changes of saliva cortisol in VDU operators.

In the field of work physiology, catecholamines and cortisol are used as strain indicators, because their production is increased in stressing work situations. Short-term stress cannot be ascertained by measuring catecholamines in urine. We studied the excretion of cortisol in saliva during short-term stress. We examined 77 healthy subjects who worked at a video display unit (VDU) for 30 min. They had to carry out a task with high speed and accuracy demands. Saliva specimens were taken before and after VDU work. Saliva cortisol levels were determined by means of a sensitive enzyme immunoassay (EIA). Additionally physiological parameters (heart rate, blood pressure, respiratory rate) and task performance were measured, and the subjects were asked about their feelings of stress and psychovegetative disorders. Saliva cortisol concentrations were significantly increased after 30 min of VDU work. Heart rate, blood pressure and respiratory rate were also increased during the task. In persons with high heart rate and blood pressure levels saliva cortisol increased significantly stronger during VDU work than in persons with lower heart rate and blood pressure levels. The results indicate that saliva cortisol is a useful strain indicator in short-term stress situations.

Influence of clenbuterol, a beta-adrenergic agonist, on desipramine induced growth hormone, prolactin and cortisol stimulation.

We report herein the effects of the beta-adrenergic agonist clenbuterol on desipramine (DMI)-induced growth hormone (GH), prolactin (PRL) and cortisol secretion in healthy male subjects. In the first study, nine subjects were treated with either clenbuterol (0.04 mg, p.o.) or placebo. In the second study, 12 subjects received either DMI (50 mg, i.v.) alone or in combination with clenbuterol (0.04 mg, p.o.) given 60 min prior to DMI administration. Clenbuterol alone had no influence on GH, PRL, or cortisol concentrations, compared to placebo. DMI alone caused GH stimulation (mean maximum = 15.7 +/- 3.4 ng/ml), which was significantly lower after combined administration of DMI and clenbuterol (mean maximum = 7.7 +/- 1.6 ng/ml) (p less than or equal to 0.01). DMI-induced PRL and cortisol stimulation was not influenced by clenbuterol pretreatment. These results indicate the inhibiting influence of noradrenergic beta-receptors on GH stimulation.

The influence of psychotropic drugs and releasing hormones on anterior pituitary hormone secretion in healthy subjects and depressed patients.

Pharmacoendocrinological studies have shown that psychotropic drugs with different actions have different effects on anterior pituitary hormone secretion in man. Substances with different effects on the central nervous system are characterized by a different pharmacoendocrinological profile. Studies with various receptor blockers have shown varying influences on the DMI-induced growth hormone, prolactin, and ACTH/cortisol secretion. Growth hormone stimulation was shown to be mediated by alpha 2-receptors and inhibited by beta-receptors. Investigations in male and female endogenous depressive patients demonstrated a significantly blunted growth hormone response to DMI compared with age- and sex-matched healthy subjects. A comparative study in male endogenous depressive patients showed a significantly diminished growth hormone stimulation both after DMI and after growth hormone-releasing hormone compared to healthy male subjects. In further tests a simultaneous application of four releasing hormones (GHRH, CRH, GnRH, TRH) was used. These investigations showed a significantly lower GH stimulation in endogenous depressive patients compared with age- and sex-matched healthy subjects, but not in neurotic depressive or schizophrenic patients. Cortisol stimulation was similar in all groups of patients and healthy subjects. TSH stimulation was significantly lower in endogenous depressive and schizophrenic patients than in healthy subjects. Somatomedin-C concentrations were significantly elevated in endogenous depressed patients compared with healthy subjects. The blunted growth hormone response in endogenous depression could be explained by inhibitory influences such as increased somatomedin-C concentrations or a hyperactivity of central beta-adrenergic-receptors.

[Task specificity and reproducibility of the psychophysiologic reaction pattern].

This paper presents results from investigations of physiological activation processes concerning two subjects. Firstly: Different tasks cause different reactions of the activation variables. In our experiment the heart rate raised stronger in the mental arithmetic task than in the trakking task. Conversely, blood pressure and respiratory frequency showed higher values in the tracking task. Secondly: By analyzing the stabilities for repeated investigations we obtained the following results: a) The reliability of the physiological strain variables is higher than that of the difference values. b) Habitual values are predicted best by the second investigation. c) If the experiment is often repeated, one can determine intraindividual correlations between activation variables for every single person.

[Psychophysiologic particularities in patients with functional syndromes].

Patients with gastroenterological or cardiovascular functional syndromes (n = 109) were examined by means of an extensive psychological standardized diagnostic programme and psychological strain analysis was carried out under psychogenic stress in laboratory. The results were compared with healthy probands (n = 51) likewise examined. Patients with functional syndromes significantly discriminated of healthy probands by more psychosocial risk constellation under strain caused by more distress and in parameters of performance and in psychophysiological field in some strain variables (heart rate, blood pressure, acral vasomotricity) and their dynamics.

[Digital image intensifier radiography--application in angiography, urography and bone diagnosis].

In a retrospective study, the clinical usefulness of digital image intensifier radiography was analysed. It could be demonstrated that for angiographic procedures the image quality of a 1024(2) matrix is superior to a 512(2) matrix. For digital excretory urography it could be seen that in 80 to 90% the images were at least sufficient to analyse the renal pelvis. Digital image intensifier radiography of bones was diagnostically sufficient in 76% and analysis of bone architecture was possible in 85%.

[Arterial digital subtraction angiography of the lower extremities using a high-resolution 1024 image matrix].

The quality of radiographs representing corkscrew vessels and collateral vessels that developed after occlusion of the superior femoral artery, using digital angiography technique with a 1024 matrix, is compared with that of a 512 matrix. The images obtained with the 1024 matrix were superior to those obtained with a 512 matrix. Using a small cannula for the application of the contrast agent, the quality of the images representing the collateral vessels was very good in 83% of the images produced with the 1024 matrix and in 70% of the 512 matrix images.

[Observations on the temporal sequence of reported pregnancy risks].

A prospective study was conducted with 383 pregnant women concerning the occurrence in respect of time, the duration, and the mutual interactions of pregnancy risks. 35 per cent of the women with hemorrhages during early pregnancy later had premature labour pain (p less than 0.01). The relative risk of premature labour is greater by the factor 2.2 in pregnant women with hemorrhages during early pregnancy than in women without haemorrhages. There was no difference between the investigated groups in respect of onset and duration of these two risk factors. 54 per cent of the pregnant women in whom circular sature of the cervix according to Shirodkar had been performed, also had premature labour pain (p less than 0.001). After the performance of the Shirodkar suture, premature labour followed in 40% of the pregnant women during the further course of pregnancy, corresponding to a relative risk of 2.3 compared with the group without Shirodkar. Premature labour with subsequent Shirodkar cerclage occurred earlier (21st week) and lasted for a longer time (13 weeks) than with the reverse sequence or if premature labour occurred alone. If cervical insufficiency occurred first, followed by premature labour, the time of birth was definitely much earlier (36 weeks) than with the controls. There was no evident connection between the risks "infections of urinary tract" and "premature labour". On the other hand, 32 per cent of the pregnant women with urinary tract infections suffered an EPH gestosis, corresponding to a relative risk of 2 (p less than 0.001). The results show that clear and specific pointers towards the possible occurrence of other risks can be derived from the presence of certain pregnancy risks based on specific findings.
