Epidemiology of an outbreak of erythema infectiosum in Tokyo.

Endemic outbreaks of erythema infectiosum were observed in various areas of Japan from 1977 to 1981. Even in a limited district of Tokyo, we recorded 395 cases until June 1981, so this might be the largest epidemic in Japan. Most cases occurred from January to June. The peak incidence was at the age of 7 years and the average age was 8.9 years. The outbreak also occurred among our hospital nurses, but no severe case was observed in hospitalized patients. Furthermore, the symptoms in adults were as mild as in children, suggesting the little participation of immunity in this disease. By epidemiologic survey, erythema infectiosum was distinct from rubella.

Hypersensitivity to mosquito bite and malignant histocytosis.

The follow-up data from 21 cases in Japan of severe local and constitutional hypersensitivity to mosquito bite (mosquito allergy) were collected. Three cases are now cured and no longer show hypersensitivity and 9 cases died, in 7 of these death being attributed to malignant histiocytosis (MH) or related conditions. In another case a systemic malignant neoplasm was suspected but was not confirmed as MH. Since both mosquito allergy and MH are exceedingly rare conditions, the association of the two is unlikely to be coincidental, and the possibility of a participation of viral infection is suspected as the cause of the systemic histiocytoses including MH.

[Pyoderma gangraenosum and cardio-vasculopathies, particularly Takayasu arteritis. Review of the Japanese literature (author's transl)].

Three cases of pyoderma gangraenosum were associated with Takayasu arteritis (pulseless disease), characterized by disappearance of pulse, hypertension and/or aortic insufficiency. Another 11-year-old boy, suffering from a serious congenital heart malformation, presented skin lesions of pyoderma gangraenosum. In spite or rather low incidence of association of ulcerative colitis (10.4 p. 100), Japanese cases show frequent association of cardiovascular diseases, especially Takayasu arteritis (27.1 p. 100).

Divided nevus spilus and divided form of spotted grouped pigmented nevus.

Two cases of divided nevus spilus and a case of divided form of spotted grouped pigmented nevus are reported. In this group, flat melanotic macules around the eyes were located on the opposite parts of the upper and lower eyelids. The macules were divided by the palpebral fissure when the lids were open, and the two halves of the lesions formed a unit when the lids were closed. Case presentation as such may be intriguing in the clarification of the developmental relation between aberrantly differentiated melanin synthesizing cells and normal melanocytes.

[Pseudoxanthoma elasticum associated with Marfan syndrome (author's transl)].

We described a 26-year-old man whose brother has a mottled fundus. Many maternal relatives suffer from myopia and heart disease, and one cousin has arachnodactylia. The patient is very tall and shows various abnormalities such as arachnodactylia, highly arched palate, funnel chest and scoliosis. He also shows hyperextensibility of joints, inguinal hernia, dissecting aneurysm of ascending aorta, dilatation of aortic ring, aortic and mitral insufficiency, myopia and striae distensae. These findings are that of Marfan's syndrome. At the same time, we noticed yellowish papules on the neck, axillae, inguinal region and inner aspect of thighs. Brown patches were found around umbilicus. These skin lesions as well as angioid streaks of fundus are typical for pseudoxanthoma elasticum. This case should be an association by chance of these two genetic affections.

Fine structure of unmyelinated nerves in neonatal skin.

The fine structure of unmyelinated cutaneous nerve fibers in newborns was examined in lesions of adnexal polyp of neonatal skin. In the neonatal cutaneous nerves, unmyelinated fibers outnumbered myelinated ones. The unmyelinated fibers consisted of Schwann cells, axons and basal lamina. Their ultrastructural organization was basically similar to that in the adult skin. However, some of the unmyelinated fibers contained axonal bundles which suggested a state of cytoarchitectual immaturity or incomplete growth. Phagocytosis of melanosomes by Schwann cells was also observed.

Neonatal sebaceous glands: fine structure of sebaceous and dendritic cells.

Ultrastructural features of sebaceous glands in newborns were examined in adnexal polyp lesions of neonatal skin. Material for electron microscopy was obtained from the nipple areola of 10 Japanese newborn babies of both sexes, less than 8 days of age. The cell organization of the neonatal sebaceous acini consisted of undifferentiated, differentiating, and mature sebaceous cells, and the sequence of sebaceous transformation seemed to be consistent with that described in the postnatal acini. The sheet of continuous basal lamina covering the sebaceous acini was neither distorted nor convoluted. This configuration in the basal lamina revealed no convincing evidence for physiological involution of the neonatal sebaceous cells were identified at the outer periphery of the sebaceous glnads. Melanocytes in symbiosis with the sebaceous acini in concurrent presence of Langerhans cells.

Melanogenic melanocytes in human sebaceous glands.

Electron microscopic observations revealed for the first time a small number of active melanocytes synthesizing distinctive melanin-containing organelles (melanin granules)in the ducts and acini of human sebaceous glands.

Adnexal polyp of neonatal skin.

The adnexal polyp of neonatal skin is a small, usually solitary tumour of the skin occurring mostly on the areola of the nipple of the neonate. Our statistics reveal an incidence of 4-0% in 3,257 newborn infants. Histologically it always contains hair follicles, eccrine glands and vestigial sebaceous glands, hence the term 'adnexal polyp of neonatal skin' is proposed. No similar report has been hitherto recorded.
