Using NEC MediFile 1000 and the public telephone system, the Department of Radiology of Gunma University Hospital and Saitama Medical School deliver image diagnosis to fellow clinicians and affiliated hospitals. The system consists of digitizing, filing, processing, retrieving and archiving units for radiographs and documents. Images were digitized by CCD and filed in 2 Gbyte double sided optical disk, which can accommodate 500 sheets of 14 x 17 films on 1500 of the smallest size slats without data compression. Image resolution was evaluated by ROC curve using multiple pulmonary nodules. The time taken for image transfer over 24 km was tested with various film sizes, image varieties and data compression rates.
We present a case of diffuse hepatocellular carcinoma of the liver with unusual radiological findings. On both CT and MR imaging, the posterior half of the right hepatic lobe showed a unique attenuation/intensity bordered by a clear margin, as if painted in two tones. It appeared to be analogous to "straight border sign," which is known to reflect hepatic venous stoppage. Autopsy revealed congestive hepatic infarction congruous with the area of altered attenuation/intensity caused by tumor thrombus in the corresponding branch of the hepatic vein.
A case of leiomyoma of the kidney is reported. A 54-year-old woman was found incidentally to have a solid left renal mass on abdominal ultrasound examination performed during a follow-up study for chronic hepatitis, type C. Computed tomography (CT), dynamic magnetic resonance imaging (MRI), and angiography revealed the hypervascular mass. Partial nephrectomy was performed under a preoperative diagnosis of renal cell carcinoma. Histopathology diagnosed the mass as renal capsular leiomyoma. We considered that the hypervascularity of the tumor was related to the presence of rich capillary beds.
We report a case of asymptomatic bronchial atresia of the anterobasal and laterobasal segments of the right lower lobe. Because of the absence of typical bronchial mucocele on chest radiograph, it was difficult to base the diagnosis on chest radiography alone. Helical CT images demonstrated the characteristic appearance of bronchial atresia, and three-dimensional (3D) CT bronchography using the volume rendering technique helped us to comprehend the spatial relationship between the dilated bronchus distal to the atresia and the more proximal bronchus.
A patient with extrahepatic portal vein aneurysm with massive thrombosis, congenital in nature, is reported. Reconstructed images using curved reformation software and three-dimensional images from helical CT were useful to delineate the shape of the portal vein aneurysm and extension of the thrombi.
