[Early combined carcinoma and malignant lymphoma in the stomach associated with tonsillar malignant lymphoma--report of a case].

This report deals with a histologic demonstration of a combined tumor of early signet ring cell carcinoma and lymphoma of the stomach in a 52-year-old Japanese man. The Gastric carcinoma was found by a systemic work up for a tonsillar lymphoma. The relationship between the malignant lymphomas of the stomach and of the tonsil is not clear in this case. The carcinoma and lymphoma of the stomach were both small, and the depth of invasion was localized to the mucosa and submucosa, respectively. The lymphoma was observed just beneath the cancer and was a diffuse, large cell type, and of the same histologic typing as the tonsillar lymphoma. Based on the broader size of the carcinoma, it is considered that the carcinoma probably induced the lymphoma in the stomach. This of an early combined tumor of carcinoma and lymphoma of the stomach is, to our knowledge, the first to be reported in the literature.

[Relation between prognosis and the Su-PS skin reaction in patients with non-resected primary lung cancer receiving OK-432].

Chemotherapy and administration of OK-432 were performed concomitantly in 49 cases of inoperable primary lung cancer, and the Su-PS skin reaction was measured. In the group of patients under 60 years of age, the maximum diameter of the reaction after chemotherapy tended to be large, and evaluation of the results showed that the maximum diameter tended to be large in the group showing tumor contraction effects. In a comparison of the prognosis between the group which survived for less than 6 months and that which survived for 6 months or longer, there was no significant difference between them before administration of OK-432, but when the maximum reaction diameter after chemotherapy was compared, that in the group which survived for 6 months or longer was significantly stronger (p less than 0.05). The Su-PS skin reaction at the time of administration of OK-432 is therefore considered to be a useful immunological parameter for predicting the results of treatment and also for prognosis.

Epidural ketamine for postoperative pain relief after gynecologic operations: a double-blind study and comparison with epidural morphine.

This double-blind study evaluates whether ketamine given epidurally is effective for postoperative pain relief, and compares the effects of epidural ketamine with those of epidural morphine. Sixty-eight patients undergoing abdominal gynecologic surgery were randomly assigned into six groups (control; ketamine 4, 6, and 8 mg in saline; 6 mg in 10% glucose; morphine 3 mg). All patients were anesthetized with thiopental, nitrous oxide, and enflurane, and drugs were administered epidurally at the end of the operation. The duration of analgesia in the ketamine groups did not differ from that in control patients and the difference in diluent had no observable effects. Significantly, none of the patients in the morphine group needed additional analgesics within 24 hr, whereas 85% in the other five groups did. We conclude that ketamine administered epidurally is inadequate for postoperative pain relief after gynecologic operations.

Mass spectrometric detection of the plasma prealbumin (transthyretin) variant associated with familial amyloidotic polyneuropathy.

A plasma prealbumin variant with a methionine-for-valine substitution at position 30 is closely associated with familial amyloidotic polyneuropathy (FAP) type I. Secondary ion mass spectrometry of the tryptic digest of a carrier's prealbumin could easily detect an abnormal peptide containing the substitution besides the normal peptide. This is a sensitive and reliable method for the diagnosis of FAP.

Characterization of a diphosphonopentaosylceramide containing 3-O-methylgalactose from the skin of Aplysia kurodai (sea hare)

The complete structure is proposed for a ceramide (Cer), bis(2-aminoethylphosphono)-pentaoside, isolated from the skin of Aplysia kurodai. This new phosphonoglycosphingolipid was purified using two systems of column chromatography on silicic acid. The purity of the glycolipid was confirmed by thin-layer chromatography, analysis of its composition, and proton magnetic resonance spectrometry. The component carbohydrates were glucose, galactose, N-acetylgalactosamine, and 3-O-methylgalactose. Most (90%) of the fatty acid was palmitic acid and the major sphingosine bases were octadeca-4-sphingenine (51%) and anteisononadeca-4-sphingenine (38%). 2-Aminoethylphosphonyl-6-galactose was identified after its partial hydrolysis. From studies by methanolysis, permethylation, mild acid hydrolysis, hydrogen fluoride treatment, chromium trioxide oxidation combined with thin-layer chromatography, gas liquid chromatography, gas chromatography-mass spectrometry, and proton magnetic resonance spectrometry, the structure of the glycolipid was concluded to be 3-OMeGal beta 1----3GalNAc alpha 1----3[6'-O-(2-aminoethylphosphonyl)-Gal alpha 1----2](2-aminoethylphosphonyl----6)Gal beta 1----4Glc beta 1----1Cer.

Fetal hemoglobin variants in 80,000 Japanese neonates: high prevalence of Hb F Yamaguchi (A gamma T 80 Asp----Asn).

A population-based screening of newborns for the structural variants of fetal hemoglobin was carried out in Osaka Prefecture, Japan, by isoelectric focusing of globin chains using dried blood on filter paper. Of 80,000 newborns, 18 had alpha globin variants and 55 had gamma globin variants. The incidence of gamma globin variants (1/1,455) was much higher than that of alpha globin variants (1/4,444). Structural studies were then carried out on the abnormal gamma globins in 36 samples, and revealed that 25 of them were Hb F Yamaguchi (A gamma T 80 Asp----Asn). The prevalence of this variant in Japanese was estimated to be as high as one per 2,100.

Accumulation of surfactant phospholipids in lipid pneumonia induced with methylnaphthalene.

Lipid analyses were carried out on the lungs of female B6C3F1 mice treated with methylnaphthalene. Cholesteryl ester, which could not be detected in lungs of control animals, was present in lungs of treated animals. Cholesterol and dipalmitoylglycerophosphocholine (DPPC) content was increased about five times in lungs of treated mice compared with control mice, and the content of a minor phospholipid was increased six times. The latter phospholipid was purified by high performance liquid chromatography and identified as phosphatidylglycerol by thin layer chromatography and by fast atom bombardment-mass spectrometry. Both DPPC and phosphatidylglycerol are known to be pneumonal surfactants produced from type II pneumocytes. Therefore, the accumulation of these lipids in lung tissue was assumed to be caused by the proliferation of type II cells induced by the administration of methylnaphthalene. The results provide important information concerning the underlying mechanism of endogenous lipid pneumonia in mice.

Increased cadmium resistance of skin fibroblasts from Menkes disease patients.

Cultured skin fibroblasts from patients with Menkes disease and Wilson disease were analyzed as to their sensitivities to copper and cadmium by means of a colony-forming ability and cell growth study. All the Menkes strains exhibited about 3-fold higher levels of resistance to cadmium, whereas the cytotoxicity of copper did not differ among the Menkes, Wilson and normal fibroblast strains. The resistance to cadmium of Menkes skin fibroblasts may provide a diagnostic marker of Menkes disease and useful or valuable model for the understanding of detoxification system against heavy metals.

Structural studies on glycolipids of shellfish. V. Gala-6 series glycosphingolipids of the marine snail, Chlorostoma argyrostoma turbinatum.

A series of glycosphingolipids and phosphonoglycosphingolipid containing only galactose as the sugar component were isolated from the marine snail, Chlorostoma argyrostoma turbinatum. The structures of these lipids were studied by methylation analysis, hydrogen fluoride degradation, proton magnetic resonance spectroscopy and fast atom bombardment mass spectrometry, and characterized as follows: the glycosphingolipids galactosyl beta(1-1)ceramide, galactosyl beta(1-6)galactosyl beta(1-1)ceramide, galactosyl beta(1-6)galactosyl beta(1-6)galactosyl beta(1-1)ceramide and galactosyl beta(1-6)galactosyl beta(1-6)galactosyl beta(1-6)galactosyl beta(1-1)ceramide, and phosphonoglycosphingolipid N-methylaminoethylphosphonyl galactosyl(1-1)ceramide. The main molecular species of the ceramide moiety were hexadecanoyl-octadecasphingenine and hydroxyhexadecanoyl-octadecasphingadienine in all of these sphingolipids.

[Combination therapy of high-dose carboquone and OK-432 in unresectable non-small cell lung cancer].

A combination of OK-432 and high-dose Carboquone (12-22 mg/m2) was administered to 17 patients with unresectable non-small cell lung cancer. The response rate was 42.9% (CR-1, PR-5) among 14 patients in whom measurement of tumor diameter was possible. With regard to hematologic adverse effects, 13 patients had a WBC count of less than 3,000, and 6 patients had a platelet count of less than 50,000. Duration of WBC nadir was not longer than 3 days, and there were no cases of infectious complication or bleeding tendency. Other side effects were all transient.

Quantitative approaches to the study of lymphatic contractile activity in vitro and in vivo: potential role of this dynamic 'lymph pump' in the re-expansion of the vascular space following hemorrhage.

Few investigators have considered a dynamic role for the lymphatic vessel in regulating the movement of fluid and protein from the interstitium back to the bloodstream. This view is based on the assumption that lymphatics are passive conduits and that hydrostatic pressure gradients and external compression forces acting on the vessels are primarily responsible for the movement of lymph. However, it is becoming increasingly evident that the intrinsic contractile capabilities of lymphatic vessels provide a major part of the propulsive force. Lymphatics have noradrenergic innervation and respond to a variety of humoral factors and inflammatory mediators suggesting that the pumping activity is centrally regulated and in addition, may respond to local factors. In this article, we will discuss what is known of the regulation of this 'lymph pump'. Techniques that permit analysis of contractile activity and fluid pumping in vitro and in vivo will be reviewed. Of particular interest is a sheep model that allows the quantitation of pumping activity in vivo without the complication of variable lymph inputs. While there is little information available at this time concerning the potential role of the 'lymph pump' in pathophysiological states, some preliminary experiments from our own laboratory suggest that an independently regulated 'lymph pump' may play an important role in hemorrhagic shock.

[Traumatic aneurysm occurring after surgical procedure of large cerebral aneurysm].

A case of intracranial traumatic aneurysms occurring after surgical treatment of a large cerebral aneurysm is reported. A 56-year-old man was admitted to our department with complaints of headache, nuchal pain and nausea. Left carotid angiography (Lt-CAG) revealed a large aneurysm, measuring 20 mm in maximum diameter, of the azygos anterior cerebral artery. Successful clipping operation was performed on day 17 of subarachnoid hemorrhage. Unfortunately, small cortical branches were pulled out during the procedure from the right pericallosal artery. The postoperative Lt-CAG showed formation of two other aneurysms. Second operation was done on day 28 after the first operation. These aneurysms were located at the previously injured sites on the right pericallosal artery. From the history sited above, we diagnosed them as traumatic aneurysms. The second operation resulted in successful obliteration of these two traumatic aneurysms. Literature review yielded 25 similar cases, and the authors discuss the etiologic factors of the traumatic aneurysm due to surgical procedure.
