Clinical significance of myocardial squeezing of the coronary artery.

We investigated the coronary angiography (CAG) of 1,022 patients to clarify the clinical significance of myocardial squeezing and obtained the following results. Of 1,022 patients undergoing CAG, 164 patients (16.0%) had myocardial squeezing of the left anterior descending artery (LAD) and 16 patients (1.6%) had only the first septal perforating branches producing a squeezing of the artery during systole. The association of myocardial squeezing of the LAD with different diagnosis was anterior chest pain syndrome (26.6%), arrhythmia (23.6%), cardiomyopathy (21.7%), angina pectoris (15.8%) and myocardial infarction (6.9%). The degree of narrowing of the LAD was classified into 4 grades; 25-49%: 36 patients (22.0%), 50-74%: 82 patients (50.0%), 75-89%: 35 patients (21.3%) and greater than 90%: 11 patients (6.7%). The morphology of the vessel subjected to myocardial squeezing was classified into 4 patterns on CAG. Type A is localized narrowing. Type B is bead-like narrowing. Type C is diffuse narrowing. Type D is tapering and obstructive narrowing. All type D patients had the septal perforating branches. We investigated whether only myocardial squeezing has ischemic ST-T segment changes in the ECG. Fifty-five of 87 patients (63.2%) with myocardial squeezing of greater than 75% and organic stenosis of less than 50% of the LAD had ischemic changes in the ECG during exercise or pacing-induced tachycardia. There was no special feature between myocardial squeezing and subjective symptoms.

Role of protease--protease-inhibitor complexes in inflammation.

Neutrophils accumulate and release proteolytic enzymes at the site of acute inflammation. These proteases cause tissue damage but are inactivated by protease-inhibitors which have been thought to finish their activity. We paid attention to these protease--protease-inhibitor complexes and examined their roles. Trypsin was used as a proteolytic enzyme and was mixed with an excess of purified alpha 1-antitrypsin (alpha 1AT). The trypsin-alpha 1AT complex was separated from native alpha 1AT by column chromatography. Then the complex was incubated with human neutrophils and lysosomal enzyme release was examined. Significant enzyme release was observed when neutrophils were incubated with the complex, but the amount of enzyme release from the cells was not obvious when neutrophils were incubated with alpha 1AT or trypsin alone. The amount of lysosomal enzyme release was proportional to the amount of the complex added. Further study revealed that the complex did not have much influence on neutrophil chemotaxis or superoxide radical generation, but could activate complement by the classical pathway. These facts indicate that protease--protease-inhibitor complexes play a role in prolonging inflammation.

[External conduit repair for congenital heart diseases].

Between January 1970 and March 1984, external conduit repair was performed in eighty-five consecutive patients for various complex anomalies. The age at operation ranged from 2 months to 47 years, and 41 patients were in an age group of 5 to 9 years. Among 85 complex anomalies, 20 cases of d-transposition with VSD + PS, 33 cases of Tetralogy of Fallot with pulmonary atresia, 11 cases of truncus arteriosus and 11 cases of 1-transposition were included. From January 1970 to December 1975, conduits used were aortic homograft in 1, formalin treated porcine valved conduits in 27, and valved conduits treated with 0.65% glutaraldehyde in 3 and from January 1976 to November 1982, 42 Hancock composite grafts were used. Since December 1982, valved conduits made of preserved pericardium treated with glutaraldehyde were employed. In sixty-one patients survived from operation, actuarial survival rate was calculated as 87% at five years, 82% at ten to fourteen years. However, systolic pressure of right ventricle after repair mostly affected actuarial survival rate which maintained 93% at twelve years in patients with systolic pressure lower than 80 mmHg, and reduced to 65% at fourteen years in patients with that higher than 80 mmHg. Although external conduit repair was valuable for surgical treatment in complex anomalies, proper selection of conduit material, prevention of infectious endocarditis, and pseudoendothelial proliferation in late stage were the problems to be solved in the future.

Clinical and echocardiographic features of pulmonary valve endocarditis.

We studied the clinical and echocardiographic features of eight patients with infective pulmonary valve endocarditis. In two patients, the vegetation was limited to the pulmonary valve; in the six other patients, infective lesions were also present on the mitral or aortic valves. None of the patients were addicted to narcotics. Seven of the eight patients had underlying congenital heart disease. In six patients the organism responsible for the infective endocarditis was Streptococcus viridans. Two-dimensional echocardiography performed using a wide-angle sector scanner was more useful than M-mode echocardiography for evaluating patients with pulmonary valve endocarditis.

Studies on concurrent alpha- and beta-adrenoceptor blocking action of S-596 (arotinolol).

The effects of a therapeutic dose of oral S-596 upon the cardiovascular response to intravenous isoproterenol and noradrenaline were studied in 2 hypertensive and 4 normotensive subjects in order to evaluate the drug's mode of action. After oral administration of S-596, mean blood pressure rose slightly and the heart rate decreased. In addition, cardiac output decreased considerably and total peripheral resistance increased. However pulmonary arterial end-diastolic pressure and right atrial pressure were not affected by S-596 administration. Before S-596, intravenous isoproterenol increased both heart rate and pulse pressure in a dose-dependent manner. Similarly, intravenous noradrenaline increased both systolic and diastolic pressures. Following 15 mg of S-596, the effects of isoproterenol were antagonized such that the cumulative log-dose-response curves of the mean isoproterenol-induced increases in heart rate and reductions in diastolic pressure were shifted in parallel to the right. At the same time, the mean noradrenaline-induced increases in blood pressure were also antagonized in a competitive manner. The mean ratio of alpha- and beta-components was calculated as: (formula: see text) From this result it can be suggested that the alpha-adrenoceptor blockade potency of this drug is approximately one-eight of its beta-adrenoceptor blockade potency.

Experimental transplacental transmission of canine herpesvirus in pregnant bitches during the second trimester of gestation.

The effects of canine herpesvirus (CHV) on fetuses were studied after IV inoculation of pregnant bitches in the 2nd trimester of gestation. Cesarean sections were performed on 2 bitches that were inoculated with CHV on the estimated 30th day of gestation. Bitch M-1 had 2 mummified fetuses and bitch M-2 had 4 mummified and 2 dead fetuses and 3 live-born pups. Infection by CHV was confirmed histopathologically by the presence of focal areas of necrosis associated with intranuclear inclusion bodies in heart muscle sections of 1 dead fetus; CHV was not recovered from other organs. Abortion occurred between the 2nd and 3rd week after inoculation of another pregnant bitch inoculated with CHV on the estimated 30th day of gestation. Two bitches inoculated with CHV on the estimated 40th day of gestation gave birth prematurely to 10 pups. The detection of characteristic herpesviral lesions in various organs and the reisolation of CHV from the liver, spleen, kidneys, and lungs of premature pups indicated CHV infection. Transplacental infection of fetal pups by CHV resulted in their death and subsequent mummification. It appears that abortion and premature birth also may occur in pregnant bitches infected during the 2nd trimester of gestation.
