Nisoldipine as a vasodilator in endothelium-deprived large coronary arteries of isolated working rabbit hearts.

The effect of nisoldipine on the vasoconstrictor effect of histamine on coronary vascular resistance, cardiac performance and myocardial oxygen consumption was determined in this study. In a supported working heart preparation a perfluorochemical emulsion (FC-43) was used a perfusate. To enhance the sensitivity of the large coronary arteries to vasoconstrictor stimuli, the endothelium was removed mechanically from the large coronary arteries. The changes in coronary artery diameter were documented by colour arteriography. Nisoldipine reduced the constriction of coronary arteries and the decrease in coronary flow induced by histamine. Nisoldipine abolished or reduced the decrease in cardiac output, left ventricular end systolic pressure, dP/dt and myocardial oxygen consumption. In the supported working heart preparation, nisoldipine is an effective dilator of large coronary arteries deprived of endothelial lining.

Low oxygen cost of carbonic anhydrase-dependent sodium reabsorption in the dog kidney.

To examine the oxygen requirement of carbonic anhydrase-dependent sodium reabsorption in the proximal tubule, 18 anaesthetized dogs were studied under conditions of saturated distal NaCl reabsorption; the latter was accomplished by volume expansion (all groups) combined with infusion of loop diuretics (groups 1 and 3). Acetazolamide reduced HCO3- reabsorption by 602 +/- 32 mumol min-1 (55%, group 1) and by 777 +/- 103 mumol min-1 (66%, group 2). This was accompanied with a reduction in sodium reabsorption and oxygen consumption in a molar delta Na/delta O2 ratio of about 45 in both groups of dogs. The delta HCO3/delta O2 ratio averaged 16 +/- 1, which was not significantly different from the theoretical value of 18 expected for transcellular sodium transport by Na+, K+-ATPase. Mannitol (group 3) reduced NaCl reabsorption by 37 +/- 2% without affecting NaHCO3 reabsorption or oxygen consumption significantly. We conclude that carbonic anhydrase-dependent NaCl reabsorption in the proximal tubules is passive, and that NaHCO3 reabsorption is the only important active sodium transport which is sensitive to inhibition of carbonic anhydrase.

Posttranslational regulatory system for nitrogenase activity in Azospirillum spp.

The mechanism for "NH4+ switch-off/on" of nitrogenase activity in Azospirillum brasilense and A. lipoferum was investigated. A correlation was established between the in vivo regulation of nitrogenase activity by NH4Cl or glutamine and the reversible covalent modification of dinitrogenase reductase. Dinitrogenase reductase ADP-ribosyltransferase (DRAT) activity was detected in extracts of A. brasilense with NAD as the donor molecule. Dinitrogenase reductase-activating glycohydrolase (DRAG) activity was present in extracts of both A. brasilense and A. lipoferum. The DRAG activity in A. lipoferum was membrane associated, and it catalyzed the activation of inactive nitrogenase (by covalent modification of dinitrogenase reductase) from both A. lipoferum and Rhodospirillum rubrum. A region homologous to R. rubrum draT and draG was identified in the genomic DNA of A. brasilense as a 12-kilobase EcoRI fragment and in A. lipoferum as a 7-kilobase EcoRI fragment. It is concluded that a posttranslational regulatory system for nitrogenase activity is present in A. brasilense and A. lipoferum and that it operates via ADP-ribosylation of dinitrogenase reductase as it does in R. rubrum.

Changes in hyperfrontality of cerebral blood flow and carbon dioxide reactivity with age.

We evaluated the topographic distributions of regional cerebral blood flow in 51 normal subjects (mean age 41 years) by the xenon-133 inhalation technique. Forty-five of these subjects were divided by age into young normals less than 30 years old (mean age 24 years), middle-aged normals 30-50 years old (mean age 40 years), and elderly normals greater than 50 years old (mean age 62 years); there were 15 subjects in each group. The distributions of vascular CO2 reactivity to hypocapnia were also evaluated in 20 of the normal subjects (mean age 34 years), including 11 younger normals less than 30 years old (mean age 24 years) and nine older (middle-aged or elderly) normals greater than or equal to 30 years old (mean age 45 years). The hyperfrontal distribution of regional cerebral blood flow observed in the young and middle-aged normals was not observed in the elderly normals. The hyperfrontal distribution of vascular CO2 reactivity observed in the younger normals was absent in the older normals. In addition, the correlation between regional cerebral blood flow and vascular CO2 reactivity observed in the younger normals was disturbed in the older normals. The data show a hyperfrontal distribution of regional cerebral blood flow in normal subjects that diminishes during the fifth and sixth decades, along with a distribution of vascular CO2 reactivity in younger normal subjects that is not homogeneous throughout the frontoparietal regions.(ABSTRACT TRUNCATED AT 250 WORDS)

The influence of barbiturates on cerebral metabolism in patients with borderline cerebrovascular reserve during intraoperative transient carotid occlusion.

Sixty patients with borderline or frank insufficiency of their cerebrovascular reserve undergoing carotid endarterectomy were given enough short acting barbiturates, during cross clamping of the internal carotid artery, to cause EEG burst suppression. Arterial and venous concentrations of lactate were measured in blood samples taken before, 30 min after occlusion of the artery and 10 min after reperfusion. The amount of lactate produced depended on the length of time of burst suppression; the shorter it lasted, the larger the amount of lactate produced. Only in patients with the shortest burst suppression times did the blood lactate levels rise above normal levels. The lactate levels can be used as indicators of how effective barbiturates are being in preventing ischaemia during the time of carotid artery occlusion.

[Depth EEG and evoked potentials using an intravascular catheter electrode in the baboon].

Obtaining an intracranial EEG, as well as a depth recording of the somatosensory evoked potentials, via a vascular approach are described. Experimentally, a directable microcatheter was placed in the middle cerebral artery of baboons. Using the catheter guide wire as electrode, intracranial electrical activity was recorded from within the cerebral vessels. Significantly, depth recordings of the SEP showed waves of thalamic origin not present in surface recordings. Advantages of this method over surface recordings are the greater gain of information possible and, over other techniques of depth recording, the comparibly lesser invasiveness.

[SPECT in reversible symptoms of cerebrovascular diseases].

In 16 patients with reversible symptoms of cerebrovascular disease a HMPAO- and/or IMP-SPECT was performed. 12 of these patients were suffering from TIA, 4 from PRIND. Using HMPAO-SPECT in 8 out of 9 patients with TIA and in 1 of 2 with PRIND a hypoperfusion could be demonstrated; IMP-SPECT showed a pathological pattern in 3 of 5 patients with TIA and in none of 2 patients with PRIND. A semiquantitative evaluation showed a tracer accumulation reduced by 13 +/- 12% (HMPAO) and 8 +/- 7% (IMP), respectively, in the clinically involved hemisphere, compared to the contralateral side. In circumscript SPECT lesions a reduction by 21 +/- 8% (HMPAO) or 17 +/- 7% (IMP) was observed. The interhemispheric ratio from the HMPAO-SPECT showed a significant correlation to that of the 133Xe-rCBF measurement (r = 0.86; p less than 0.001). SPECT was positive in a higher percentage than TCT, Doppler sonography, radioangiography and 133Xe-rCBF measurement. This does not imply a higher specificity or sensitivity, because a slight inhomogeneous SPECT pattern may occasionally be observed even in normal persons.

[Nuclear medicine diagnosis of Alzheimer type dementia].

Eleven patients with Alzheimer type dementia were investigated by single-photon-emission computed tomography (SPECT) using 99mTc-hexamethylpropylene-aminoxim (HM-PAO). Nine had a typical, symmetrically reduced enrichment in the parieto-temporo-occipital area. In three of these nine patients a similar pattern was recorded additionally in the frontal, in the remaining two in the temporal or parietal region. In a semi-quantitative evaluation radioactivity concentrations in the cerebrum and cerebellum were compared. It demonstrated that there was in relation to the cerebellum a reduced concentration in seven of the eight studied cerebral regions. The relative blood flow of the parieto-temporo-occipital region was most markedly affected and in patients with Alzheimer type dementia was reduced by 25% below that of normal subjects. The results indicate that cerebral SPECT with HM-PAO is a reasonable method of investigation to confirm the diagnosis of "Alzheimer type dementia" and exclude other cerebral disease, especially if pathognomonic findings are to be obtained.

Semiquantifying regional cerebral blood flow by 123I-amphetamine (IMP) SPECT in cerebrovascular disorders: correlations with CBF indices by 133Xe inhalation method.

To examine the capacity of detecting regional cerebral blood flow (rCBF) disturbances in stroke, measurements with 133Xe inhalation and 123I-amphetamine (IMP) SPECT were performed within 1-2 days in 19 patients. IMP SPECT images in transverse sections consisting of outer and inner areas were correlated with 32 regions of 133Xe rCBF. 133Xe rCBF was assessed by ISI, CBF15, and F1. Inter hemispheric ratio correlation showed outer IMP uptake correlated better with 133Xe CBF indices than the inner one. With higher ratios of 133Xe rCBF. IMP uptake ratios were variable, suggesting less usefulness of this parameter in evaluation of mild ischemia. Values of IMP in outer regional hypo and hyperemic areas correlated better with those of 133Xe rCBF than the inner ones. In regional ratios of hypo and hyperemic areas from hemispheric mean, outer IMP uptake correlated with 133Xe CBF indices, reflecting regional disturbances of fast clearing tissue perfusion. Regression lines between ratios in 133Xe CBF and IMP uptake were located below one to one correlation, and dissociated more for hyperemic regions. IMP SPECT correlated better with 133Xe rCBF for tissues with reduced perfusion but underestimated hyperemic regions as measured with the 133Xe method. The data suggest outer cerebral IMP uptake evaluated by SPECT could reflect flow disturbances in the brain cortex.

Influence of amino acids on nitrogen fixation ability and growth of Azospirillum spp.

The utilization of amino acids for growth and their effects on nitrogen fixation differ greatly among the several strains of each species of Azospirillum spp. that were examined. A. brasiliense grew poorly or not at all on glutamate, aspartate, serine, or histidine as the sole nitrogen and carbon sources. Nitrogen fixation by most A. brasiliense strains was inhibited only slightly even by 10 mM concentrations of these amino acids. In contrast, A. lipoferum and A. amazonense grew very well on glutamate, aspartate, serine, or histidine as the sole nitrogen and carbon sources; nitrogen fixation, which was measured in the presence of malate or sucrose, was severely inhibited by these amino acids. It was concluded that growth on histidine as the sole source of nitrogen, carbon, and energy may be used for the taxonomic characterization of Azospirillum spp. and for the selective isolation of A. lipoferum. The different utilization of various amino acids by Azospirillum spp. may be important for their establishment in the rhizosphere and for their associative nitrogen fixation with plants. The physiological basis for the different utilization of glutamate by Azospirillum spp. was investigated further. A. brasiliense and A. lipoferum exhibited a high affinity for glutamate uptake (Km values for uptake were 8 and 40 microM, respectively); the Vmax was 6 times higher in A. lipoferum than in A. brasiliense. At high substrate concentrations (10 mM), the nonsaturable component of glutamate uptake was most active in A. lipoferum and A. amazonense.(ABSTRACT TRUNCATED AT 250 WORDS)

Abnormalities of interictal cerebral perfusion in classic but not common migraine.

Regional cerebral blood flow (rCBF) was measured as gray matter blood flow using the 133Xe inhalation technique in 50 pain-free headache patients: Eight patients having classic migraine with normal computed tomograms were matched to patients having common migraine and to normal controls. Interictal rCBF was determined at least 6 days after the last migraine attack and more than 24 hours before the next one. There were no between-group differences for age, PCO2, mean hemispheric blood flow, interindividual and intraindividual variabilities, hyperfrontality, or rCBF symmetry. However, when subjects were classified as to overall abnormal perfusion, a significant number (n = 4, p less than 0.04) of patients with classic migraine had rCBF abnormalities, whereas only one such patient was seen in the group with common migraine. Patients with classic migraine had abnormal mean hemispheric blood flows or disturbed intrahemispheric rCBF patterns. Oligemic and hyperemic regions topographically corresponded to the clinical symptoms in one patient. We conclude that during migraine attacks and interictally there is an instability of rCBF control in patients with classic but not common migraine.

A controlled study on the effect of pentoxifylline and an ergot alkaloid derivative on regional cerebral blood flow in patients with chronic cerebrovascular disease.

Regional cerebral blood flow (rCBF) in 90 patients with CBF decreased due to vascular diseases was studied by using the xenon 133 inhalation technique and a 32-detector setup. Whereas 30 patients received their standard basic therapy only and were regarded as controls, 30 others received 3 x 2 mg/day of an ergot alkaloid (co-dergocrine mesylate), and 30 others received 3 x 400 mg pentoxifylline (slow-release formulation)/day orally. Therapy was performed for eight weeks and CBF measured before start of treatment, after a four-week treatment period, and at the end of the study. CBF did not change significantly in the control group; both the pentoxifylline and the ergot alkaloid group presented with a significant increase in the CBF. This positive effect was significantly more pronounced in the pentoxifylline group and affected more ischemic than other brain tissues. In addition, symptoms like sleep disturbances, vertigo, and tinnitus improved significantly during the pentoxifylline observation period.
