[Transient dilatation of intra- and extra-hepatic bile ducts. A case].

A case of transient dilatation of intra and extrahepatic bile ducts in the course of an infectious disease is described in a three-year-old girl. The ultrasonic bile duct abnormalities disappeared after three months. The mechanism involved remains uncertain; the hypothesis of a transient inflammatory obstruction is evoked.

[Accidental ingestion of caustics in Tunisian children. Report of 125 cases].

The authors reviewed 125 cases of accidental ingestion of caustic substances admitted to a general pediatrics department over the last four years. L'eau de Javel (bleaching agent with sodium hypochloride) was the most frequently encountered caustic substance (89%). Esophagogastric fibroscopy was performed in 100 cases and esophageal lesions were classified according to tree grades of severity. In 46 cases, fibroscopy was normal, while severe esophagogastric lesions (grades 2 and 3) were found in 26 cases. On follow-up, six patients developed esophageal stricture, three of them after concentrated, eau de Javel ingestion. Stricture was severe in four cases, and required colonoplasty of the esophagus; it was limited in two cases and required endoscopic dilatation only. The authors emphasize the frequency and the severity of lesions caused by chloride bleach (eau de Javel) and recommend that fibroscopy be carried out in all children following ingestion of any caustic substance, even in the absence of oropharyngeal burns.

[Femoral hypoplasia--unusual facies syndrome].

We report a new case of femoral hypoplasia-unusual facies syndrome (FH-UFS). A review of the literature disclosed fifty-five previously published cases. Both boys and girls can be affected. The syndrome includes bilateral femoral hypoplasia; facial dysmorphism with a cleft palate, micrognathia, a long philtrum, a thin upper lip, and a short broad-tipped nose; dysplasia of the hips; and hypoplasia of the fibulae. Other malformations may be found, including skeletal defects and visceral (especially cardiovascular and genitourinary) abnormalities. Etiopathogenesis of this syndrome remains unknown. Some investigators have suggested a link between the FH-UFS and caudal dysplasia in infants born to diabetic mothers.

[An association of Addison's disease, achalasia of the cardia and alacrimation. Apropos of 2 cases].

We report two cases of a syndrome combining Addison disease, cardial achalasia, and alacrimation in two girls aged 4 and 5 1/2 years. Moreover, these patients exhibited some unusual features: both also had mineralocorticoid deficiency, and one had loss of tongue papillae and absence of the pupillary reflex. This syndrome is infrequent. Etiology is unknown. Pathophysiology is under debate; autonomic nervous system dysfunction seems likely. We have analyzed the features of this syndrome on the basis of our two observations and the 15 previously reported cases.

[Thrombosis of the right ventricle in 2 infants with myocardiopathy].

We report two cases of left ventricular thrombosis in infants with myocardiopathy. Patients were aged ten and twelve months respectively. Two-dimensional echocardiography, performed because of the development of heart failure, evidenced an echogenic image within the left ventricle and significant dilatation of the left ventricular chamber. One patient developed a peripheral arterial thrombosis that resolved under anticoagulant therapy. The intracardiac thrombus disappeared under anticoagulant therapy after one month in one patient and six months in the other. Pathophysiology of the left ventricular thrombosis is discussed; the dilatation of the left ventricle apparently played a significant role. We emphasize the value of early initiation of preventive anticoagulant therapy in patients with myocardiopathy.

T-cell receptor antibodies in the immunohistochemical studies of normal and malignant lymphoid cells.

Three anti-T Cell receptor (TCR) antibodies, BF1, BF2, and WT31 were studied for their specificity and usefulness as immunohistochemical reagents. These antibodies were all satisfactory in the staining of normal peripheral lymphoid tissues and cortical thymic lymphocytes were reactive with BF1 and BF2 but not with WT31. Hassall's corpuscles in two of three thymuses studied reacted with all three antibodies. BF1 was superior to the other two antibodies, especially for lymphoid cells in cytospin preparations. Fifty-five lymphomas, 24 nonlymphoid malignancies, seven established cell lines, and five cases of large granular lymphocyte (LGL) proliferation with neutropenia were studied. The majority of non-T-cell lesions did not react with the antibodies. An occasional case showed weak reactivity which could be easily distinguished from the usual strong reaction with T-cells. Tumor cells from over 90% of snap frozen peripheral T-cell lymphomas reacted with BF1 and BF2. BF1 was the preferred antibody since it gave a stronger and more consistent reaction. It was also the antibody of choice in identifying T-lymphoblastic lymphomas. Michel's solution fixed tissue showed markedly diminished or absent reactivity with BF2 and WT31, but BF1 reactivity was less affected. Some unusual T-cell phenotypes, with respect to the pattern of expression of BF1 antigen and CD3, were observed. BF1 and anti-CD3, in combination, would be useful in identifying T-cell lesions with aberrant or unusual TCR-CD3 phenotypes.

A "back door" approach to analysis of ethanol-associated risks and behavior.

Ethanol has been detected in the majority of medical examiner cases involving nonnatural death and in a substantial number of natural deaths. The higher the serum alcohol concentration (SAC), the greater were the odds that death was due to external (traumatic) causes. We identified 149 medical examiner cases that were positive for ethanol and negative for 11 major drug of abuse groups, and studied those cases to further clarify ethanol-related risk factors and behaviors. Based on terms of frequency of occurrence, the data identify SACs of 201 mg% to 300 mg% as a particularly high risk factor for sudden, unexpected, traumatic death. Traumatic deaths associated with SACs below 100 mg% were infrequent, and attendant opinions concerning the contributory role of ethanol must be made with circumspection. SACs below 150 mg% could not be used to predict whether or not the decedents had been engaged in active or sedentary behavior prior to death because both situations occurred with equal frequency. At SACs between 151 mg% and 350 mg%, active situations were observed three times as often as sedentary situations. When SACs were 350 mg%, sedentary situations were observed twice as often as active situations. Suicide victims and driver fatalities rarely showed SACs above 350 mg%; therefore finding such SACs in apparent suicidal or driver fatality situations should prompt a thorough investigation to rule out other possibilities.(ABSTRACT TRUNCATED AT 250 WORDS)

[Etiological study of acute gastroenteritis in children in Tunisia].

This study was aimed at identifying the relative importance of the various agents responsible for gastroenteritis and to contribute to the etiopathogeny of this condition. Four hundred and two samples of stools of patients and 209 of control subjects, all under 3 years of age, were submitted to bacteriological, parasitologic and virologic examinations. An enteral etiology of gastroenteritis was found in 50% of the children. It consisted of enteropathogenic bacteria in 11%, rotaviruses in 29% and both associated in 19% of cases. There was a significant difference between patients and controls for bacterial (p less than 0.0001) and viral (p less than 0.00001) infestation. The most frequently isolated bacteria consisted overall of Escherichia coli. Eighty-seven isolated strains of E. Coli were tested for their enterotoxinogenic capacities. Four strains were bearing the attachment factor (EAF) and 14 were toxinogenic. Shiga-like toxin (or vero toxin) was detected in 4 strains. Campylobacter was isolated as frequently in patients as in controls and was often associated with other bacteria or rotaviruses. The presence of parasites and candidas was not significantly different in patients and in controls. Giardia intestinalis was the most frequently isolated parasite.

[In vitro study of the effects of oxolinic acid at sub-inhibitory concentrations on the activity of hemagglutinins and adhesion to uroepithelial cells by Escherichia coli isolated from urine].

The aim of the present study was to investigate the effects of sub-MIC doses of oxolinic acid (quinolone), widely used in the treatment of urinary tract infections, on both haemagglutinating activity and adhesion capacity of 13 Escherichia coli strains isolated from urine during acute cystitis or pyelonephritis. All these strains adhered to uroepithelial cells and showed mannose-sensitive and/or mannose-resistant haemagglutinating activity. Sub-MIC doses of oxolinic acid induced filaments in most of the bacterial cultures; however, inhibition of haemagglutination and adhesion was variable in vitro. When inhibition did take place in any one strain, both haemagglutination and adhesion were affected. These results confirm those of other authors and indicate that the effect of sub-MIC doses of a given antibiotic is strain-specific; they also indirectly show the heterogeneity of E. coli strains isolated from urine. It thus seems unlikely that, in clinical conditions, a single antibiotic is capable of reducing adhesion, given the diversity of the adhesins found in pathogenic E. coli strains.

[The isolated presence of anti-HBc antibodies: prevalence and interpretation based on the results of viral DNA research and anti-HBs antibodies measurement after vaccination].

In order to study the significance of the isolated presence of anti-HBc antibodies, we have looked for the 3 classic serological markers of the hepatitis B (HBs antigen, anti-HBs and anti-HBc antibodies) in 1,586 hospital agents who are to vaccinate in the framework of a campaign of systematic vaccination of the hospital personnel of university hospitals of Sfax for a period of 18 months. We identified subjects who presented isolated anti-HBc antibodies (33 individuals = 2.08%). In these subjects'serum, we performed a research of the DNA of hepatitis B virus (HBV) with a PCR hybridization technique using a couple of primers. One week after administration of a vaccine dose, we also measured anti-HBs antibodies in their sera. Among the tested 18 personnel with anti-HBc isolated antibodies, 11.1% had low rates of anti-HBs antibodies indicating that there is presumably primary antibody response and therefore a false positivity of anti-HBc antibodies in pre-vaccinal serology; while 11.1% others had higher rates of anti-HBs antibodies corresponding to a secondary antibody response, which witness a previous HBV immunisation. The research of the HBV-DNA was positive in 11.1% of tested personnel, testifying a presumably chronic portage of the virus with low rates of the HBs antigen undetectable with the serological techniques. For the remainder subjects with isolated anti-HBc antibodies (66.7%), the interpretation remains ambiguous.

[Prevalence of anti-CMV antibodies in blood donors in the Sfax region (value in blood transfusion)].

Detection of anti-CMV antibodies was carried out in sera of healthy blood donors, divided into groups of 20 according to age and sex. Sera were tested for anti-CMV by an ELISA test (Enzygnost anti-CMV/IgG-Behring). Among 280 sera, 272 were positive for IgG to CMV (97.14%). The prevalence of those antibodies was high in all age stratum (95-100%) but was higher in women than in men (98.57% versus 95.71%). The titre of IgG to CMV was superior to 12 Ul/ml in 56.43% of CMV positive donors. So, the leucocyte removal is the only alternative for the prevention of post-transfusional CMV infection. The high percentage of donors with anti-CMV antibodies level more than to 12 Ul/ml allow to consider the use of plasmapheresis for preparing specific immunoglobulins to CMV.

[Significance of "anti HBC alone" serological profile in 284 patients suspicious of being infected with hepatitis B virus].

The "anti-HBc alone" serological profile is a frequent finding in hepatitis B virus infections, but little is known about its clinical significance. The aim of this study was to explore the 'anti-HBc alone' serological profile obtained by immunosorbent assay (ELISA) in 284 patients suspicious of being infected with hepatitis B virus. Sera were screened for following serological markers: HBs Ag, anti-HBc and anti-HBs antibodies using immunoradiometric assay (IRMA) and for HBV DNA using polymerase chain reaction. Among 284 studied sera with 'anti-HBc alone' serological profile, 124 were positives for anti-HBs antibodies by IRMA and corresponding to a recovered form of hepatitis B. Nineteen sera were negatives for anti-HBc antibodies, suggesting false positive results by ELISA. Two sera were found positives for HBs Ag by IRMA, which are related to authentic hepatitis B. HBV DNA was positive in 4 sera, suggesting occult hepatitis B. This study indicates that "anti-HBc alone" serological profile is most often correlates with recovered hepatitis B infection, but it can mask an occult hepatitis B.

[Study of the sensitivity of non-typhoid salmonellae isolated at Sfax (Tunisia)].

From 1982 to 1986, 122 strains of non-typhoid Salmonellae were isolated at the University Hospital of Sfax (Tunisia). Both tests: serotype classification and sensitivity have demonstrated that the most frequent serotypes: S. wien and S. typhimurium have the highest degree of multiresistance. Ampicillin, cotrimoxazole and chloramphenicol are the most affected while cefotaxime, amikacin and colistin are still saved. Facing these facts, the authors insist on the seriousness of the excessive use of antibiotherapy which leads to the selection of such strains.
