The management of non-traumatic cardiac arrest in the operating room with cardiopulmonary bypass.

We present a case of a 29-year-old woman whom, while undergoing an elective gynecological procedure, acutely arrested. Closed chest cardiopulmonary compressions were not effective. Fortuitously, the cardiac surgical team was in an adjacent operating room, about to start an elective bypass case. After sternotomy, the patient was placed on cardiopulmonary bypass within 20 min of the arrest. The patient achieved return of spontaneous circulation and was ultimately discharged with only mild extremity weakness. The etiology of the arrest was never fully explained. Open chest massage and cardiopulmonary bypass should be considered early in the management of unexpected cardiac arrest, especially in the operating room where surgical expertise should be immediately available. Surgeons and anesthesiologists need to be aware of, and consider, the possibility of employing these techniques.

Grip strength testing reliability.

The purpose of this study was to investigate the test-retest reliability of isometric grip strength testing using the Jamar dynamometer. Thirty-three subjects, 16 men (mean age = 37.4 years) and 17 women (mean age = 35.5 years), were assessed on two separate occasions using four methods to determine grip strength score. The methods used were (1) the score of one trial, (2) the mean score of two trials, (3) the mean score of three trials, and (4) the highest score of three trials. All five handle positions of the Jamar dynamometer were used during testing. Although still within the limits of acceptability, position 1 was significantly less reliable when compared with the other handle positions. Because the four methods of determining grip strength score were not significantly different in reliability, it was not possible to recommend any method as being more accurate. To analyze curve shape further, grip strength curves for the strongest and weakest men and women were compared by transforming points on the curves into percentages of the maximum value. Transformed curves for both strong and weak subjects were almost identical, indicating that, for this sample, "flattening" of the grip strength curves was a function of weak grip. The present data do not support the use of raw score curve shape as an indicator of sincerity of effort during grip strength testing.

An electronic cryoprobe for cryosurgery using heat pipes and thermoelectric coolers: a preliminary report.

A hand-held fully electrically powered and programmable cryoprobe for general-purpose cryosurgery and cryotherapy has been developed. By combining the technologies of thermoelectric cooling and heat pipes, the temperature at the tip of the probe can easily reach -50 to -60 degrees C. It can hold below -40 degrees C when it cools a load of 10 W at the tip. Previous efforts developing cryoprobes made of thermoelectric modules have been hindered by the inherent characteristics of commercially available thermoelectric coolers: low efficiency, size and inflexible shape and very sensitive to heat intensity and thermal insulation. Matching thermoelectrics with heat pipes uses the advantages of both technologies. In the cryoprobe the heat pipe is used to focus and transport the cooling power of multi-thermoelectric modules. The heat flux for the thermoelectric modules is reduced and their efficiencies are increased. The transport of heat by a heat pipe also allows flexible access to treated spots of patients.

Antenatal intraventricular haemorrhage.

Germinal matrix-intraventricular haemorrhage (IVH) is a well described complication of premature birth. We present two infants with severe IVH in the absence of other serious neonatal illness. These lesions probably occurred antenatally and prior to the onset of labour. Unrecognised intrauterine stresses may cause intraventricular haemorrhagic or ischaemic lesions in the immature brain. Timing of these lesions is important for both clinical and indeed medico-legal reasons.
