[Acute cerebrovascular disease at the internal medicine hospital].

On the basis of the medical reports of 255 patients with an acute cerebral insult the clinical symptoms, the courses of the illness as well as the diagnostic procedure were discussed. 76 of 255 patients (29.8%) are taken ill before they reach their 65th year, which stresses the sociomedical importance of acute cerebrovascular syndromes. With patients under 65 years the clinical syndrome corresponds to the following stages of the cerebrovascular insufficiency: stage IIa-14.5%, stage IIb-43.4%, stage III and IV-36.8%. A brain embolism is found in 5.3% of the cases. If there is an acute cerebrovascular insufficiency the ultrasonic Doppler detection as a noninvasive procedure should be the beginning of the paraclinical diagnosis. Should the ultrasonic Doppler detection give an indication of extracranial stenosis or obstruction of the A. carotis, then an angiographic diagnosis ought to be started in order to make possible a surgical treatment of the A. carotis.

[Transabdominal ultrasound tomography of the lumbar intervertebral disks and the lumbar canal].

The transabdominal sonographic examination of the lumbar intervertebral discs and the lumbar canal was carried out on 50 patients, for whom no existed clinical symptoms for a lumbosacral nerve root lesion. The sonographic device used was a mechanical sector scanner with a transducer with a frequency of 3.5 MHz. The transversal representation of the lumbar intervertebral discs and the lumbar canal was successful mainly at the height of the intervertebral space L4/5 (96%), less frequent at the height L3/4 (74%), L5/S1 (70%) and L2/3 (18%). The intervertebral disc and the intraspinal space at the height of the disc can be assessed in a transversal section, and thus the noninvasive sonographic method represents an improvement of the conventional X-ray-examination of the lumbar spine.

[Spinal ultrasound tomography in the diagnosis of lumbar intervertebral disk displacement].

By comparative sonographic and myelographic investigations of 61 patients with lumbar disc herniations and other nerval compressive syndromes, the value of the spinal sonography in the diagnosis of lumbar disc herniation is demonstrated. The transversal transabdominal representation of lumbar disc herniations is successful in 82% of all cases. The sensitivity of sonography amounts to 0.73 in the segment L5/S1 and to 0.92 in the segment L4/5. In connection with the neurological investigation and X-ray films of the lumbar spine, the spinal sonography as a noninvasive procedure can represent an useful alternative to the myelographic diagnosis of the lumbar disc herniation.

Protection by antiarrhythmic drugs against ischemia-induced arrhythmias occurring during coronary artery occlusion in the rat.

Ligation of the left coronary artery in rats provides a relatively simple model for the assessment of antidysrhythmic compounds. This procedure results in severe arrhythmias in the initial postligation period (extrasystoles, bigeminae, tachycardia and fibrillation). Using this model we have compared the effectiveness of antiarrhythmic drugs which will be referred to class I, II and IV according to the VAUGHAN WILLIAM'S classification. All drugs have been shown to be antiarrhythmic against early occlusion arrhythmias. Especially the antiarrhythmics prevent the rhythm disturbances tachycardia and fibrillation. We conclude from the results that the coronary artery occlusion in rats is a valuable model to use in the screening of drugs for possible antidysrhythmic activity and with differing electrophysiological profiles.

[Cardiac effects of Cordemcura in vitro].

Cordemcura has a dose-dependent inotropic effect in the Langendorff-perfused guinea pig heart, in the isolated spontaneously active atria of the guinea pig as well as in the isolated electrically stimulated rabbit papillary muscle. Tachyphylaxy symptoms and arrhythmogene side-effects have not been observed. The dose-dependent coronardilatory effect caused by Cordemcura as observed at the isolated heart of a guinea-pig appears intensely. The positive inotropic effect by Cordemcura on an isolated electrically stimulated papillary muscle of a rabbit is linked with a significant decrease of the relaxation time t2, whereas there is no change in the time to peak force t1. Both mean velocity of force development S1 as well as the mean velocity of relaxation S2 are increased by Cordemcura.

[Modification of smooth muscle contractility by Cordemcura].

Cordemcura causes a dose-dependent inhibition of noradrenaline contractions of isolated rabbit aorta as well as of isolated guinea pig ductus deferens. In both organ preparations analogous intensity of effect has been observed. In different intestinal isolated organs Cordemcura also produces a dose-dependent inhibition against different agonists. The Ba2+-induced contraction is inhibited to approximately the same extent as various receptor-induced contractions. Consequently a specific receptor function on smooth muscle has to be excluded with regard to Cordemcura. Furthermore Cordemcura has a bronchodilatory effect. This property could be proved by the protection effect as observed in the inhalative histamine-induced bronchoconstriction in guinea pig.

[Spectrum of the adverse effects of Cordemcura].

Side-effects of Cordemcura were verified in a relatively extensive repertoire of methods. Investigations of blood pressure activity, effect on heart frequency as well as PQ and QT times after continuous infusion, influence on diuresis and antidiuresis and behaviour to achieve results of a possible activity of the central nerve system were given priority. The proved vasodilatory qualities of Cordemcura become also evident in the rat blood pressure reduction due to higher dose as a result of i.v. injection. No hypotensive effect could be observed after oral administration of the drug. Heart frequency as well as PQ and QT times after i.v. infusion of 1 X 10(-4) mol/kg/min Cordemcura over a period of 15 min remained unchanged. Doses going far beyond therapeutically applied doses indicate a reduction of diuresis after oral administration of Cordemcura for rats. Only due to these high doses unspecific changes in the behaviour of rats and mice occurred. No specific central nerve system activity could be observed.

Reproductive outcomes in women exposed to solvents in 36 reinforced plastics companies. I. Menstrual dysfunction.

Recent studies have suggested that exposure to styrene monomer and other solvents is associated with menstrual disturbances. This is the first U.S. study to evaluate the effects of exposure to styrene on dysmenorrhea, intermenstrual bleeding, secondary amenorrhea, menstrual blood clots, and hypermenorrhea. A comprehensive questionnaire was administered to 1,535 blue-collar workers. Multiple logistic regression analyses did not reveal a positive association between styrene exposure and any menstrual abnormality. Risk factors associated with menstrual disturbances included chronic illness with secondary amenorrhea (p less than or equal to .005), nulliparity with both decreased blood clots (p less than or equal to .005) and hypermenorrhea (p less than or equal to .001), smoking with blood clots (p less than or equal to .005), and age with hypermenorrhea (p less than or equal to .001). The overall prevalences of menstrual disorders were 14% for dysmenorrhea, 16% for intermenstrual bleeding, 7% for secondary amenorrhea, 40% for menstrual blood clots, and 31% for hypermenorrhea.

[Synthesis and spectrum of pharmacologic action of aryl-arylamino- and aryl-aryliminoacetonitriles].

Aminoacetonitriles were synthetized from arylaldehyde, arylamine and cyanhydrogene. Their dehydrogenation led to the iminoderivatives. The compounds were evaluated for spasmolytic activity in vitro. Some aryl-arylaminoacetonitriles showed significant inhibition of the histamine-induced contraction in guinea-pig ileum and acetylcholine-induced contraction in rat ileum. Structure-activity relationships are discussed.

[Spasmolytic activity of (3,4-dichloroanilino)phenylacetonitrile (H 74) and its analogs].

The influence of smooth muscle contractility of (3,4-dichloranilino)phenyl-acetonitrile (H 74) was studied in comparison with the analogs (4-chloranilino)phenyl-acetonitrile (H 47) and (3,4,5-trichloranilino)phenyl-acetonitrile (H 84) and several standard pharmaca. H 74 and H 84 showed a spasmolytic action that was almost equal to that of methphenethamine and papaverine. Other investigations have revealed that H 74 was free from any side effect. It is concluded that chloranilino-phenyl-acetonitriles cause a nonspecific inhibition of smooth muscle contractility.

[Contribution to the atypical macroglia in nonhepatic diseases (author's transl)].

Frequency and appearance of atypical macroglia in the frontal cortex and striatum in 400 human brains of cases with non-hepatogenic diseases were investigated and compared with results in 150 human brains of cases with severe liver diseases. Quantitative and qualitative differences between the non-hepatogenic (I---III) and hepatogenic (IV) groups were statistically significant. Evidence for the so-called Alzheimer-II-cells could only be obtained in metobolic disturbances of hepatogenic origin and in a few cases of cerebral coma.

Kinetic enzymatic method for automated determination of glucose in blood and serum.

Studies are reported on the reaction kinetics of the glucose assay according to Trinder which involves the specific oxidation of glucose by glucose oxidase and the determination of the hydrogen peroxide released by means of phenol and 4-aminophenazone in the presence of peroxidase. The results have been used to develop a general kinetic fixed-time method for the analysis of glucose in whole blood and serum. The single reagent method has been adapted to the ENI GEMSAEC centrifugal analyzer and to the Abbott ABA-100 analyzer. The procedures exhibited excellent precision and the results correlated well with those obtained by the hexokinase method, Linearity was achieved from 3 to 64 mmol/1 glucose for the GEMSAEC method, and from 3 to 33 mmol/1 glucose for the ABA-100 method. Reagent or sample blank corrections were not necessary. There were no interferences from various drugs, hemoglobin, bilirubin, or lipemia.

Experimental evaluation of the spasmogenicity of dopamine on the basilar artery.

Arteriograms of the basilar artery reveal that dopamine given intracisternally to dogs can generate cerebral vasospasm. This finding supports a recent hypothesis of others that dopamine may play a role in the pathogenesis of vasospasm, especially since many substances are known which fail to produce such spasm. Compared to blood or prostaglandin E2, however, the spasm induced by dopamine was delayed in onset, less in incidence, and usually less intense. Possible explanations for such experimental differences are discussed.
