[Prognostic factors and results of treatment in endometrial carcinoma].

158 patients with endometrial cancer who were treated between 1980-1990 at the Department of Obstetrics and Gynecology, Hospital Berlin-Buch, were reviewed retrospectively with regard to prognostical and therapeutical aspects. The 5-year-survival rate of all patients amounted to 84%. The 5-year-survivals were 92.6% for stage I, 87.5% for stage II and 47.6% for stage III (old FIGO classification). Depth of myometrial invasion, lymph-vascular space involvement, lymph-nodal status, tumor type and grading are of dominant prognostic value. Surgery was the treatment of choice in all reviewed cases. No statistically significant difference was observed in the 5-year survival rate between vaginal and abdominal hysterectomy. The 5-year survival rate for cases with vaginal hysterectomy was 85.4% and for abdominal hysterectomy 87.8%.

Annual night visiting rates in 129 general practices in one family health services authority: association with patient and general practice characteristics.

BACKGROUND: Rates of night visiting by general practitioners have increased steadily over the last 30 years and vary widely between general practices. AIM: An ecological study was carried out to examine night visiting rates by general practices in one family health services authority, and to determine the extent to which differences in night visiting rates between practices could be explained by patient and practice characteristics. METHOD: The study examined the variation in annual night visiting rates, based on night visit fees claimed between April 1993 and March 1994, among 129 general practices in Merton, Sutton and Wandsworth Family Health Services Authority, London. RESULTS: Practices' annual night visiting rates varied from three per 1000 to 75 per 1000 patients. The percentages of the practice population aged under five years and aged five to 14 years were both positively correlated with night visiting rates (r = 0.38 and r = 0.35, respectively), as were variables associated with social deprivation such as the estimated percentage of the practice population living in one-parent households (r = 0.24) and in households where the head of household was classified as unskilled (r = 0.20). The percentage of the practice population reporting chronic illness was also positively associated with night visiting rates (r = 0.26). The percentages of the practice population aged 35 to 44 years and 45 to 54 years were both negatively associated with night visiting rates (r = -0.34 and r = -0.31, respectively) as was the estimated list inflation for a practice (r = -0.31). There was no significant correlation between night visiting rates and the distance of the main practice surgery from the nearest hospital accident and emergency department. There was also no association between night visiting rates and permission to use a deputizing service. In a stepwise multiple regression model, the multiple correlation coefficient was 0.56 with four factors (percentage of the practice population aged under five years, percentage aged 35-44 years, percentage who were chronically ill and estimated list inflation) explaining 32% of the variation in night visiting rates. CONCLUSION: Only about one third of the variation in night visiting rates between practices could be explained by patient and practice variables derived from routine data. Population-based research using data collected on individual patients and practices is required to improve current understanding of the patient and practice characteristics that influence the demand for night visits and of why night visiting rates vary so widely between practices.

Echocardiographic diagnosis of a cardiac fibrosarcoma in the right atrium of a sheep.

This is a case report of a six-year-old female White Alpine sheep with a cardiac fibrosarcoma in the right atrium. Clinically, the sheep had right-sided cardiac insufficiency with tachycardia, engorgement of the jugular veins, brisket edema, and ascites. Chronic congestion of the liver resulted in increased hepatic enzyme activity. Based on clinical findings, a tentative diagnosis of endocarditis or pericarditis was made. Radiography of the thorax revealed hydrothorax. An echogenic mass was observed in the right atrium via echocardiography; it was interpreted as a tumor or thrombus. Ultrasonography of the abdomen revealed severe ascites and chronic congestion of the liver attributable to right-sided cardiac insufficiency. The clinical and sonographic findings were verified at post mortem. The mass in the right atrium was a pedunculated fibrosarcoma.

[Evaluation of epidemiologic risk factors for endometrial carcinoma based on a case-control study].

In a case-control-study an epidemiological investigation of cancer of the endometrium was carried out. The study included 159 cases and 159 controls. It was shown, that woman in the sixth life decade with overweight, a smaller number of deliveries and a later menopause had a higher risk for endometrial cancer. Other risk factors are diabetes mellitus and hypertension. But it is necessary to see the relationship between the typical age for these two characteristics and the typical age for endometrial cancer. The intake of estrogens without enough gestagens during an estrogen replacement therapy was associated with an increased risk. Furthermore the patients with carcinoma of the uterine corpus had a higher incidence of malignant tumors in their families and more breast cancer in their own case history. A history of oral contraceptive use appeared to reduce the risk of endometrial cancer. In addition there is an negative association between smoking and endometrial cancer. Thus factors of high risk related to cancer of the endometrium could be defined. Preventive examinations of high risk groups could help to decrease the incidence of endometrial cancer.

Proteolytic processing and glycosylation of cathepsin B. The role of the primary structure of the latent precursor and of the carbohydrate moiety for cell-type-specific molecular forms of the enzyme.

The lysosomal cysteine proteinase cathepsin B is synthesized in cultured human hepatoma HepG2 cells as an inactive 44 kDa precursor and subsequently processed to the mature single-chain enzyme with a molecular mass of 33 kDa. Intralysosomal conversion into the two-chain form results in subunits of 27 kDa, 24 kDa (heavy chain) and 5 kDa (light chain). Enzymic deglycosylation reveals that the 27 kDa polypeptide is the glycosylated variant of the carbohydrate-free 24 kDa heavy-chain form. The intracellular transport to the lysosomes is dependent upon mannose 6-phosphate-containing N-linked oligosaccharides. Receptor-mediated endocytosis of human skin-fibroblast-derived procathepsin B by HepG2 cells resulted in processed molecular forms that are not distinguishable from endogenous cathepsin B, thus favouring rather a cell-type-specific processing than structural differences due to the source of the proenzyme. The conversion step of single-chain catehpsin B into the two-chain enzyme is inhibited in vivo by the irreversible cysteine-proteinase inhibitors Z-Phe-Ala-CHN2 and, albeit weaker, Z-Phe-Phe-CHN2. Both substances have no effect on the activation of procathepsin B to the mature enzyme. The carbohydrate moiety of cathepsin B exerts no significant influence on the stability and the enzymatic activity of the enzyme.

[Linear relations of brain pulse amplitude to mean intracranial pressure].

A catheter for intracranial pressure monitoring was introduced into the right lateral ventricle in 71 neurosurgical patients. In 63 patients (88.7%) a significant linear relationship was found between the cerebral pulse amplitude (PA) and the mean intracranial pressure (Pm). In 8 patients (11.3%) no linear relationship was established between PA and Pm. PA- and Pm-analyses performed subsequent to these compliance tests are discussed on the basis of the pathophysiology of the intracranial space.

[Anti-arrhythmic action of some amidinohydrazone substituted benzophenones. 1. Synthesis of new amidinohydrazone and N-phenylamidinohydrazone substituted benzophenones].

The title compounds are synthesized as a rule by condensation of substituted benzophenones and derivatives of aminoguanidine in the presence of up to 2.5 moles of an anorganic acid. They can be obtained alternatively via corresponding hydrazones, thiosemicarbazones or methylthiothiocarbonylhydrazones.

[Potential cardiotonics. 14. Synthesis and in vitro positive inotropic actions of 4-morpholino-5-(4-pyridinyl)pyridine derivatives].

By the reaction of 2-chloro-5-(4-pyridinyl)pyridines 1-6 with morpholine as well as by derivation of the 2-morpholino-pyridine-3-carboxamide 8 the 3-substituted 2-morpholino-5-(4-pyridinyl)pyridines 7-14 were prepared. The evaluation for positive inotropic properties in spontaneously beating isolated guinea pig atria gave for the 3-cyano derivative 7 (AWD 122-14) the best activity. The potency is comparable to that of milrinone and is due to partial by inhibition of phosphodiesterase III (PDE III).

[Antiarrhythmic amidinohydrazone substituted benzophenones. 2. Preparation and determination of configuration of (Z)- and (E)-2-amino-5-chlorobenzophenone amidinohydrazone salts].

The configurational isomers of the 2-amino-5-chlorbenzophenoneamidinohydrazones could be obtained as TLC pure samples by fractionating crystallization of the product mixture. The determination of their configuration by comparison of the UV spectra with the corresponding oximes could be confirmed by X-ray structure analysis.

[Extending informative value of intracranial pressure measurement by cerebral pressure amplitudes (Pa)--analysis of average intracranial pressure (Pm)].

One aim of the intracranial pressure measurement is observing constantly patients with intracranial space-occupying factors and therefore to act diagnosticly and therapeutically correct at the right time. Through the continual registration of the intracranial pressure amplitude in relation to the medium intracranial pressure it is earlier possible to forecast intracranial pressure rise and to deal with this than with just the simple measurements of medium intracranial pressure. With 40 neurosurgical patients such examinations were undertaken after implantation of a ventricle catheter. The value of this method was proven during the continual observation of the patient.

[Improved patient monitoring during intracranial pressure measurement by calculating a theoretical pressure-volume curve].

Patients with an acute "brain damage" where an intracranial pressure rise can be suspected due to the pathophysiological context are an indication for intracranial pressure measurement. For registrating the reduction of the intracranial reserve space before the rise of the medium pressure, volume-loads from the side-ventricles were undertaken with 54 neurosurgical patients. A better bedside monitoring is possible though the mathematical registration of those factors which indicate the size of the functional intracranial reserve space. With the calculated parameters of the reserve space (pressure-volume-index) a theoretical pressure-volume-curve was demonstrated.

Responses of normal and rheumatic human articular chondrocytes cultured under various experimental conditions in agarose.

The purpose of the present study was to test if agarose could support the maintenance of normal and arthritic human chondrocytes in culture, and under which experimental conditions they could be successfully grown. Cultures of chondrocytes isolated from articular cartilage from patients with rheumatoid arthritis (RA), juvenile rheumatoid arthritis (JRA), and healthy controls were assessed by light microscopy, alcian blue staining, formazan uptake and incorporation of radiosulfate into the extracellular matrix. The results showed that both normal and arthritic chondrocytes proliferated, and synthesized proteoglycan (PG) in agarose in short term and long term culture. Proliferation and PG synthesis occurred at a slower rate in chondrocytes from adult rheumatic patients than from healthy controls. Supplements to the medium influenced chondrocyte proliferation, PG synthesis and release into the medium. Serum from RA patients stimulated chondrocyte responses more than normal human serum (NHS), and NHS promoted PG synthesis more than fetal calf serum (FCS). Exposure to inflammatory synovial fluid (SF) enhanced PG synthesis of healthy chondrocytes, but suppressed it in arthritic chondrocytes. We conclude that species-specific serum is optimal for chondrocyte cultures, and that disease related culture conditions change the chondrocyte response. As metabolic responses of human chondrocytes are maintained in agarose, this culture system appears as a suitable in vitro tool for further studies of human joint disease.
