In the case we present, a physician's order for the treatment of a patient with severe burns unfolds into an ethical dilemma for an occupational therapist. Several conflicting thoughts-in terms of the appropriateness and plan for treatment, while trying to maintain the patient's best interest as the central focus-come to mind. We examine the therapist's ethical responsibilities in light of considerations of futility, nonmaleficence, financial costs, and team relations. Several options for responding to the case are suggested.
BACKGROUND: Giant luteinized follicle cyst is a recently recognized cause of ovarian enlargement during pregnancy and the puerperium. Only rare cases of this clinical condition have been reported in the literature, and the cytologic features have not been previously described. CASE: A 34-year-old, pregnant woman presented at 10 weeks' gestation with a large, right ovarian cyst. Clinical management was initially conservative, but four weeks later the patient presented with acute abdominal pain. At laparotomy, the ovarian cyst was aspirated and a cystectomy performed. On cytologic examination, the presence of a luteinized follicle cyst of pregnancy was suggested. Subsequent histologic examination confirmed the diagnosis. CONCLUSION: This case illustrates the clinicopathologic features of a luteinized follicle cyst of pregnancy with special emphasis on the cytologic characteristics. The cytologic features are sufficient to establish the diagnosis in conjunction with the ultrasonographic appearance.
Otosclerosis is one of the common causes of hearing loss. The incidence varies between 0.1% and 2%. In Lebanon otosclerosis is a common entity that has not been well evaluated. To the best of our knowledge there is no epidemiologic analysis of the incidence or outcomes of otosclerosis in Lebanon. We collected the number of stapedectomies performed for otosclerosis in different hospitals between Jan. 1994 and Dec. 1995. We also retrospectively reviewed the charts of 71 cases who underwent stapedectomy at Hôtel-Dieu de France-St Joseph University Hospital Medical Center, Beirut, Lebanon. Between 1992 and 1996 the incidence of otosclerosis in Lebanon as revealed through stapedectomy is 5/100,000. We report also on the pathology, technique, complications and outcomes of stapedectomy surgery for otosclerosis in Hôtel-Dieu Hospital. Further epidemiologic studies and screening is required to reveal the exact incidence of this common entity that could be underdiagnosed or untreated in our country.
OBJECTIVES: Chronic suppurative otitis media (CSOM) is a common disease especially in developing countries. In Lebanon, CSOM is a frequent complaint. This retrospective study is an objective evaluation of the cases we operated in HDF. METHODS: One hundred and forty cases of CSOM were collected between 1993 and 1997: 88/140 cases were simple CSOM and the others 52/140 cases had cholesteatomas. RESULTS: There were no immediate postoperative complications reported (< 6 weeks). Beyond 6 weeks, with a mean follow-up of 20 months, 15% of the patients were lost for follow-up and we had 93.4% of success rate in CSOM and 88% in cholesteatomas. The recurrence rate of cholesteatoma was 14%. Children's cholesteatoma (below 15 years of age) were more aggressive than adult's cholesteatomas. In CSOM without cholesteatoma, the hearing gain was 5 dB. In CSOM with cholesteatoma the mean hearing gain was 4 dB. In conclusion the study revealed the importance of this pathology in our country with the relative high incidence of cholesteatoma compared to simple CSOM, this could be explained by the chronicity of the disease and the lack of preventive measures.
In this study, we are presented the clinical and the aetiologic features of 47 patients having hypercalcemia (total serum calcium > or = 2.7 mmol/l). Our results show a positive significantly correlation between the degree of the hypercalcemia and the severity of the clinical symptomatology. As for to the aetiology, the multiple myeloma, the hyperparathyroid and the neoplastic process constituted 78% of the total aetiology.
We report on an unusual tumor of the jugular foramen in a 26-year-old male. The lesion showed moderate enhancement on CT scan and a higher signal intensity on T2-weighted MR images than on T1, with a massive enhancement after gadolinium. Tumor was entirely removed surgically and histology was consistent with a low-grade myxoid chondrosarcoma, an exceptional tumor in this location. The differential diagnosis includes paragangliomas, schwannomas, metastases and meningiomas. Radiological and histological features can also mimic chordomas. However, immunohistochemical analysis certifies the diagnosis.

Cardiac hydatic cysts are rare and represent 0.5 to 2% of all hydatic cysts in humans [1, 2]. They are usually found in the left or right ventricle, and exceptionally in the interventricular septum. An exclusively pericardia hydatic cyst is excessively rare. The case reported here emphasizes: That this hydatid cyst of the pericardium remained asymptomatic until rupture. The role of echocardiography as a diagnostic tool. The superiority of MRI to CT Scan in the establishment of a precise preoperative diagnosis.
