Effect of pravastatin on progression and regression of coronary atherosclerosis and vessel wall changes in carotid and femoral arteries: a report from the Regression Growth Evaluation Statin Study.

Few data are available about the potential benefit of serum cholesterol reduction in the broad range of patients with coronary atherosclerosis and normal to moderately elevated serum cholesterol levels. REGRESS is a double-blind, placebo-controlled, multicenter study to assess the effect of a 2-year treatment with the 3-hydroxy-3-methylglutaryl co-enzyme A reductase inhibitor pravastatin on progression and regression of coronary atherosclerosis using quantitative coronary arteriography in 885 male patients with a total serum cholesterol value of 155-310 mg/dl (4-8 mmol/liter). Among symptomatic men with significant coronary atherosclerosis and normal to moderately raised levels of serum cholesterol, patients treated with pravastatin had less progression of coronary atherosclerosis and fewer new cardiovascular events than patients in the placebo group. Ultrasound examinations of carotid and femoral arteries were performed in 255 patients. Changes in intimal-medial thickness also showed a treatment effect from pravastatin; however, on a per patient basis, there was no correlation with the treatment effect in the coronary arteries.

Effects of lipid lowering by pravastatin on progression and regression of coronary artery disease in symptomatic men with normal to moderately elevated serum cholesterol levels. The Regression Growth Evaluation Statin Study (REGRESS).

BACKGROUND: Intensive lowering of serum cholesterol may retard progression of coronary atherosclerosis in selected groups of patients. However, few data are available on the potential benefit of serum cholesterol reduction in the broad range of patients with coronary atherosclerosis and normal to moderately elevated serum cholesterol levels who undergo various forms of treatment. The Regression Growth Evaluation Statin Study (REGRESS) addresses this group of patients. METHODS AND RESULTS: REGRESS is a double-blind, placebo-controlled multicenter study to assess the effects of 2 years of treatment with the 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitor pravastatin on progression and regression of coronary atherosclerosis in 885 male patients with a serum cholesterol level between 4 and 8 mmol/L (155 and 310 mg/dL) by quantitative coronary arteriography. Primary end points were (1) change in average mean segment diameter per patient and (2) change in average minimum obstruction diameter per patient. Clinical events were also analyzed. Of the 885 patients, 778 (88%) had an evaluable final angiogram. Mean segment diameter decreased 0.10 mm in the placebo group versus 0.06 mm in the pravastatin group (P = .019): The mean difference between treatment groups was 0.04 mm, with a 95% CI of 0.01 to 0.07 mm. The median minimum obstruction diameter decreased 0.09 mm in the placebo group versus 0.03 mm in the pravastatin group (P = .001): The difference of the medians between the treatment groups was 0.06 mm, with a CI of 0.02 to 0.08 mm. At the end of the follow-up period, 89% (CI, 86% to 92%) of the pravastatin patients and 81% (CI, 77% to 85%) of the placebo patients were without new cardiovascular events (P = .002). CONCLUSIONS: In symptomatic men with significant coronary atherosclerosis and normal to moderately elevated serum cholesterol, less progression of coronary atherosclerosis and fewer new cardiovascular events were observed in the group of patients treated with pravastatin than in the placebo group.

Cerebral blood flow during migraine attacks without aura and effect of sumatriptan.

OBJECTIVE: To study regional cerebral blood flow (rCBF) during migraine attacks without aura and after treatment with sumatriptan. DESIGN AND INTERVENTION: We performed three technetium Tc99m hexemethyl-propyleneamineoxime single photon emission computed tomography scanning procedures in patients with migraine who participated in a double-blind, placebo-controlled, randomized clinical trial (1) outside an attack, (2) during an attack, and (3) after treatment of the attack with 6 mg of subcutaneous sumatriptan. SETTING: University hospital. PATIENTS: We studied 20 patients with migraine without aura, 15 of whom were evaluated under all three conditions and five of whom were evaluated under only two conditions. OUTCOME MEASURES: The single photon emission computed tomographic images were evaluated semiquantitatively with regard to (1) the degree of asymmetry of the rCBF between the headache side and the nonheadache side and (2) the ratio of the rCBF in regions of interest to the rCBF in two reference areas (cerebellum or frontal cortex). RESULTS: We found no significant rCBF asymmetries outside or during the attack or after treatment with sumatriptan, and there were no significant changes of the rCBF ratios during the attack (compared with outside the attack) or after treatment of the attack (compared with during the attack). CONCLUSION: Migraine attacks without aura and treatment of the attacks with 6 mg of subcutaneous sumatriptan are not associated with detectable focal changes of the rCBF.

Prednisone treatment of elderly-onset rheumatoid arthritis. Disease activity and bone mass in comparison with chloroquine treatment.

OBJECTIVE: Prednisone is frequently used in the treatment of elderly-onset rheumatoid arthritis (RA), but the balance between efficacy and toxicity, including the effect on bone mass, has not been investigated in long-term studies. This prospective, randomized study was undertaken to compare disease activity and bone mass during long-term treatment with prednisone versus chloroquine in this patient population. METHODS: Patients with active RA diagnosed at age > or = 60 were randomized to receive prednisone (15 mg/day for 1 month, with the dosage tapered as low as possible thereafter) (n = 28) or chloroquine (n = 28). Patients who did not show a response received other second-line drugs as an adjunct to prednisone or as a replacement for chloroquine. Bone mass was measured by dual-energy x-ray absorptiometry. The study duration was 2 years. RESULTS: During the 2 years, treatment with other second-line drugs was needed for 12 patients in the prednisone group (43%) and 8 in the chloroquine group (29%). Functional capacity and disease activity improved significantly in both groups and did not differ significantly between the groups, except for a greater improvement in the prednisone group at 1 month. Radiographic scores for joint destruction progressed similarly in both groups. There was a nonsignificant excess bone loss of 1.8% in the spine and 1.5% in the hip in the prednisone group, compared with the chloroquine group. CONCLUSION: Neither treatment was entirely satisfactory since a significant number of patients needed an additional second-line drug over the 2-year period.

Clinical value of immediate thallium-201 reinjection imaging for the detection of ischaemic heart disease.

Immediate reinjection of thallium-201 after acquisition of the stress images followed by imaging 60 min later has been proposed as a novel time-saving and patient-convenient approach for the evaluation of myocardial ischaemia and viability. The objective of this study was to establish the clinical value of thallium-201 stress immediate reinjection imaging in the evaluation of patients with suspected and known coronary artery disease. Quantitative planar thallium-201 stress immediate reinjection imaging was performed in 138 patients with undiagnosed chest pain, of whom 114 underwent cardiac catheterization and 24 had a < 5% likelihood for coronary artery disease; 56 patients had previously suffered a myocardial infarction. Sensitivities and accuracies were determined for coronary artery disease, reversible myocardial ischaemia, and myocardial viability. Of the 138 patients studied for coronary artery disease, thallium stress scintigraphy identified 85 of 93 patients with angiographically proven coronary artery lesions (overall sensitivity 91%). The normalcy rate in the low-likelihood patients was 88% (21 of 24 patients) and the overall diagnostic accuracy was 83%. In the 56 patients who had suffered a previous myocardial infarction, and in whom the suitability for reversible ischaemia was sought, the overall patient-related sensitivity and accuracy were 93% and 92% respectively. In the 56 patients with a previous myocardial infarction a total of 168 vessel regions were studied to detect viability. Of the 142 normal or hypokinetic regions, 127 regions were scintigraphically identified as viable (sensitivity 89%). The thallium stress immediate reinjection imaging protocol is an appropriate clinical approach in patients with undiagnosed chest pain. The combined identification of stress-induced ischaemia and viability by thallium stress immediate reinjection imaging provides adequate algorithms allowing optimal management and risk assessment in the individual cardiac patient.

Levels of circulating intercellular adhesion molecule-1 and -3 but not circulating endothelial leucocyte adhesion molecule are increased in patients with rheumatoid vasculitis.

The aim of this study was to investigate whether levels of circulating adhesion molecules reflect vascular inflammation in rheumatoid vasculitis (RV). Levels of circulating intercellular adhesion molecule-1 (cICAM-1), c-ICAM-3 and circulating endothelial leucocyte adhesion molecule (cE-selectin) were determined in 14 patients with RV and compared to 47 patients with rheumatoid arthritis (RA) and 100 healthy donors (HD). Enzyme-linked immunosorbent assays were used to quantify cICAM-1, cICAM-3 and cE-selectin. We found that in RV significantly (P < 0.0001) elevated levels of cICAM-1 and cICAM3, but not cE-selectin, were found when compared with RA patients. Levels > 2 S.D. above the mean level of HD were present for cICAM-1, cICAM-3 and cE-selectin in 57, 71 and 21%, respectively of patients with RV and 2, 21 and 44%, respectively of the RA patients. Increased levels of both cICAM-1 and cICAM-3 were found in 43% of the RV patients and in none of the RA patients. Comparison of the serum levels of patients studied in an active and inactive phase of RV revealed significantly lower levels of cICAM-3 levels in the inactive phase. In conclusion we find that determination of cICAM-1 and cICAM-3 may be useful as a marker of vascular inflammation in patients with RV.

In-patient treatment for active rheumatoid arthritis: clinical course and predictors of improvement.

The objective was to determine the clinical course and predictors of clinical response in patients with rheumatoid arthritis (RA) who are hospitalized for active disease and functional deterioration. Sixty-three patients who were admitted to a rheumatology clinic for multidisciplinary treatment were prospectively evaluated at 2-weekly intervals. During the admission period, which lasted for a mean of 47 +/- 24 days, patients improved significantly according to variables of disease activity, functional status and emotional status. The change in the variables appeared to be linear with time. Twenty-seven patients (43%) fulfilled the Paulus 20% criteria for clinical response during treatment. A long disease duration, a high Larsen erosion score, a high level of disease activity at admission and the institution of a disease-modifying anti-rheumatic drug within 3 months before admission were associated with a clinical response. In patients hospitalized for active disease, improvement appeared to be linear with time during admission. Patients with long-standing, destructive disease and a high level of disease activity improved most during hospitalization.

Age and gender affect different characteristics of slow waves in the sleep EEG.

Low-frequency EEG was analyzed quantitatively during 2 nights in 40 females and 34 males aged 26 to 101 years. Analyses were based on Rechtschaffen and Kales NREM sleep stages, on absolute low-frequency amplitude (i.e. power in the range of 0.2-2.0 Hz) and on low-frequency continuity. The latter parameter describes how much (0-100%) of the current slow-wave activity is continued in the near-future EEG. Such continuation can occur through closed loops in the underlying neuronal network and cells. These loops are slow, thus corresponding to slow-wave frequencies, and can consist of electrophysiological, chemical and/or other pathways. The continuity percentage then monitors the relative activity of these loops. It does not depend directly on absolute EEG amplitudes. All analyzed parameters, including amplitude-independent continuity, decreased substantially and significantly with increasing age. The amplitudes of low-frequency EEG in females were significantly and substantially (40%) larger than those in males. However, the amplitude-independent continuity percentage did not differ between the genders. These findings support the notion that gender-related anatomical differences have a general effect on EEG amplitude, including during slow-wave sleep. Aging, however, specifically affects the neurophysiological slow-wave-generating mechanism.

The effect of methacholine-induced acute airway narrowing on lung sounds in normal and asthmatic subjects.

The association between lung sound alterations and airways obstruction has long been recognized in clinical practice, but the precise pathophysiological mechanisms of this relationship have not been determined. Therefore, we examined the changes in lung sounds at well-defined levels of methacholine-induced airway narrowing in eight normal and nine asthmatic subjects with normal baseline lung function. All subjects underwent phonopneumography at baseline condition and at > or = 20% fall in forced expiratory volume in one second (FEV1), and in asthmatic subjects also at > or = 40% fall in FEV1. Lung sounds were recorded at three locations on the chest wall during standardized quiet breathing, and during maximal forced breathing. Airflow-dependent power spectra were computed using fast Fourier transform. For each spectrum, we determined the intensity and frequency content of lung sounds, together with the extent of wheezing. The results were analysed using analysis of variance (ANOVA). During acute airway narrowing, the intensity and frequency content of the recorded sounds, as well as the extent of wheezing, were higher than at baseline in both groups of subjects. At similar levels of obstruction, both the pitch and the change in sound intensity with airflow were higher in asthmatics than in normal subjects. Wheezing, being nondiscriminative between the subject groups at baseline, was more prominent in asthmatics than in normal subjects at 20% fall in FEV1. We conclude that, at given levels of acute airway narrowing, lung sounds differ between asthmatics and normal subjects. This suggests that airflow-standardized phonopneumography is a sensitive method for detecting abnormalities in airway dynamics in asthma.(ABSTRACT TRUNCATED AT 250 WORDS)

Serum values of pepsinogens and antibodies to Helicobacter pylori in relation to a history of peptic ulcer disease and NSAID use.

The present study was undertaken to investigate whether patients with a history of peptic ulcer disease (PUD) have serological features indicative of the presence of more severe gastritis, compared to patients without a history of PUD. In addition we investigated whether current chronic use of non steroidal anti-inflammatory drugs (NSAIDs) was associated with serological features indicative of a chemical (type C) gastritis. In total 850 consecutive outclinic patients were interviewed by a standard questionnaire with emphasis on demographic data, diagnosis, chronic use of NSAID, and history of PUD. Serum pepsinogen A (PgA) and pepsinogen C (PgC), the PgA:PgC ratio, and IgA and IgG antibodies to H. pylori were measured in all patients. After controlling for age, sex, H. pylori seropositivity, and presence of RA, a correlation was found between a decreased pepsinogen A:C ratio and the use of NSAID. Patients with a history of PUD have serological features indicative of a persistently more severe gastritis. Secondly we found serological evidence for the existence of a chemical (type C) gastritis in NSAID users.

Analysis of continuous nocturnal penile rigidity measurements with the use of the RigiScan summary analysis software program.

The evaluation of the RigiScan nocturnal recordings of 324 patients, 18 to 72 years old (mean age 49 years), according to guidelines based on normal data, were compared with analysis of these recordings by using the RigiScan Summary Analysis Software Program. This enabled us to provide normal values for this program. We conclude that it should be possible to extend this RigiScan software program in such a way that a probability of normality is given to its user. In patients with corporal veno-occlusive dysfunction a significant lower total event duration was found when compared with the results of patients with normal and abnormal findings.

Case-control analysis of allogeneic bone marrow transplantation versus maintenance chemotherapy for relapsed ALL in children.

In a retrospective study, the results of maintenance chemotherapy and allogeneic bone marrow transplantation (BMT) for children who reached a second complete remission (CR2) of their acute lymphoblastic leukemia (ALL) were compared. Case-control analysis was performed comparing 25 allogeneic transplant patients (cases) with 97 patients treated with maintenance chemotherapy (controls), who were matched for site of relapse, duration of CR1 and leukemia-free interval from onset of CR2. Until the first relapse, the children were treated according to standard protocols. The majority of patients suffered from a bone marrow relapse, mostly occurring more than 24 months after the onset of CR1. Remission reinduction treatment was heterogeneous. Patients treated with allogeneic BMT received high-dose chemotherapy and total body irradiation prior to BMT. Maintenance chemotherapy in controls was given for approximately 2 years. Following BMT, relapse rate was lower but the treatment-related mortality was higher compared with maintenance chemotherapy, resulting in leukemia-free survival rates at 4 years of 44% and 24%, respectively (not significant, NS). Case-control analysis of leukemia-free survival showed a hazard ratio of 0.756 in favor of BMT compared with chemotherapy (NS). If bone marrow relapses and central nervous system relapses were analyzed separately, a tendency to better leukemia-free survival was present after BMT compared with maintenance chemotherapy for patients with a relapse in the central nervous system, but for an isolated bone marrow relapse, no differences in leukemia-free survival were seen between the two groups of patients.

Immediate 201Tl reinjection: an alternative approach in myocardial perfusion scintigraphy? A comparison with contrast angiographic findings.

Thallium-201 (201Tl) stress-redistribution/reinjection imaging provides a reliable diagnostic approach in patients evaluated for myocardial ischaemia. Immediate reinjection of 201Tl after completing the stress images followed by imaging 1 h later shortens the investigation time to 2.5 h and may provide a novel approach. In this study, we validated the immediate reinjection approach in the assessment of myocardial perfusion and viability using contrast angiographic findings as the reference standard. We studied 114 patients with documented cardiac catheterization data using quantitative planar 201Tl stress/immediate reinjection imaging. The 201Tl findings were compared with contrast angiographic data, coronary arteriographic findings and electrocardiographic data. Myocardial regions were considered to be viable if they (1) showed normal or hypokinetic wall motion, (2) were supplied by patent coronary arteries or by occluded arteries with adequate collateral circulation, or (3) were not related to electrocardiographic infarct site. Myocardial regions were considered to be non-viable if they (1) showed akinetic or dyskinetic wall motion, (2) were supplied by occluded vessels with inadequate collateral circulation, or (3) were related to electrocardiographic infarct site. Of 280 regions with normal or hypokinetic wall motion, 47 (17%) regions showed persistent 201Tl defects compared with 21 of 26 (81%) regions with akinetic/dyskinetic wall motion (P < 0.0001). Of 318 regions supplied by patent vessels, 65 (20%) regions showed persistent 201Tl defects compared with 14 of 22 (64%) regions subtended by occluded vessels (P < 0.0001). When the 201Tl findings were compared with electrocardiographic infarct site, 37 of 259 (14%) remote regions showed persistent defects compared with 42 of 83 (51%) infarct-related regions (P < 0.0001). We conclude that persistent defects on 201Tl immediate reinjection images are significantly more often associated with severe left ventricular wall motion abnormalities, coronary artery occlusion and infarct-related regions. In particular, agreement between the wall motion approach and scintigraphic findings was excellent, with 21 of 26 akinetic or dyskinetic segments showing persistent defects and 233 of 280 normal or hypokinetic segments measured as viable by the reinjection technique, resulting in a sensitivity of 81%, a specificity of 83% and an overall accuracy of 83%. These findings show the potential of immediate 201Tl reinjection imaging for assessing myocardial perfusion and viability.

Frequencies of HPRT mutants and micronuclei in lymphocytes of cancer patients under chemotherapy: a prospective study.

Fifteen cancer patients, including 10 testicular carcinoma patients, were treated with several types of combination chemotherapy. Blood samples were collected before, during and after chemotherapy. Subsequently, lymphocytes were analyzed for frequencies of HPRT mutants (MF) and micronuclei (MNF). Significantly elevated MFs were detected in eight patients. Mean expression time (+/- SD) for mutations was 98 +/- 54 days (range: 42-172 days). In some patients, enhanced MFs persisted for a period of 430-490 days after cessation of chemotherapy. In five patients MNFs were increased 2-6-fold and the enhancement was fairly persistent. Ifosfamide and cyclophosphamide appeared to be the most mutagenic and clastogenic constituents of the chemotherapy, while evidence for adverse effects of adriamycin, 4-epi-adriamycin and bleomycin was equivocal. Results indicate that the clinical use of mutagenic drugs must be weighed against the risks of persistent genetic damage and secondary malignancies in cured patients and their potential offspring. Further studies are necessary to determine the true risks and incidence of such abnormalities following chemotherapy for curable forms of cancer.

Rating impairment and disability in Parkinson's disease: evaluation of the Unified Parkinson's Disease Rating Scale.

Although the Unified Parkinson's Disease Rating Scale (UPDRS) is widely used to monitor disease progression and drug efficacy, no attempts have been made to evaluate its scientific and clinical quality. Poor clinical sensibility of items in the activities of daily living (ADL) section and redundancy of items in the motor examination (ME) section prompted us to determine if the number of items in these sections could be reduced to a smaller, equally sensitive set that would describe the data as reliably as did the original set of items. Therefore, we assessed 111 patients with idiopathic Parkinson's disease on the UPDRS and Hoehn and Yahr staging scales. Our results show that the ADL (13 items) and ME (14 items) sections can be reduced to 8 items each without loss of reliability or validity. Our report is preliminary and needs validation.

A prospective study of effects of Belsey MK IV antireflux surgery on endoscopic esophagitis, lower esophageal sphincter pressure, and 24-hr pH measurements. Relation to symptom improvement.

A prospective study on the effect of the Belsey MK IV operation on esophagitis, lower esophageal sphincter pressure (LESP), and acid reflux as monitored on 24-hr pH recording was conducted to investigate the association between objective changes and the effect of operation on reflux-associated symptoms. Thirty-one patients were included. The effect of surgery on symptoms was recorded in all patients, and 22 patients agreed to undergo endoscopy, manometry, and 24-hr pH recording. Follow-up ranged from three to nine years (mean five years); 87% reported long-lasting improvement (50% free of symptoms, 37% major improvement, no medication needed). The combination of symptomatic improvement and absence of esophagitis was found in 70%. LESP significantly increased [8 +/- 6 mm Hg preoperatively, 14 +/- 5 mm Hg postoperatively (P < 0.001)] to a level above 5 mm Hg in 96% of the patients. No endoscopic esophagitis was found in 17 of 20 patients (85%; P < 0.05) (two patients refused endoscopy). The 24-hr pH monitoring normalized in 11 of the 20 patients (55%) (one registration failed). The operation-induced rise in LESP correlated with improvement on endoscopy (r = 0.51; P < 0.002) and with reduction of reflux parameters (number of episodes with pH < 4: r = 0.64; P < 0.05, percentage of total time pH < 4: r = 0.42; P = 0.07). A rise in LESP must be an important aim of antireflux surgery. The Belsey MK IV does not induce a rise to a level that causes severe dysphagia or bloating, but the trade-off is less control of acid reflux.(ABSTRACT TRUNCATED AT 250 WORDS)

Medium latency stretch reflexes in young-onset Parkinson's disease and MPTP-induced parkinsonism.

It is still unclear why components of the phasic stretch reflex are increased in Parkinson's disease (PD). To study the role of aging, we assessed medium latency (ML) stretch reflexes in 19 Parkinson patients (8 with young-onset PD, 11 with late-onset PD) and 23 normal subjects (10 young, 13 old). To assess the contribution of supraspinal dopaminergic influences, we also studied 5 young parkinsonian patients with a selective central dopamine deficiency induced by 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP). ML responses were recorded from the medial gastrocnemius muscle which was stretched by 4 degrees toe-up rotations of a platform upon which subjects were standing. ML responses were significantly enhanced in late-onset patients compared with older controls. In contrast, ML responses did not differ between young-onset patients and young controls. This observation could be attributed to the significantly different influence of age on ML-amplitudes in patients and controls. Thus, ML-amplitudes declined with age in controls, whereas they increased with age in PD possibly because older patients were more severely affected than younger patients. Thus, the difference between young-onset and late-onset PD seems to be related to the opposite effects of increasing age and disease severity in patients and controls. ML-amplitudes did not differ between patients with MPTP-induced parkinsonism, patients with young-onset PD and controls, suggesting that supraspinal dopaminergic systems are not critically involved in control of ML responses. This result also indicates that the severity effect in PD may be related to disruption of non-dopaminergic pathways which occurs in late-onset, but not young-onset patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Quantitative analysis of planar technetium-99m Sestamibi myocardial perfusion images. Clinical application of a modified method for the subtracton of tissue crosstalk.

Application of the Thallium-201 quantitative analysis method to Tc-99m Sestamibi planar myocardial perfusion images results in oversubtraction of tissue crosstalk, defined as 'activity within the myocardial image, which originated outside the heart'. A modified algorithm, specific for Tc-99m Sestamibi, was applied in 60 patients and the results of quantitative analysis to estimate the risk of coronary artery disease were compared with coronary angiographic findings and with visual analysis. New crosstalk planes were generated based on the results in 20 male healthy volunteers. The results in our study group indicate a higher diagnostic accuracy when circumferential profiles are generated on the basis of maximal counts compared to mean counts. Quantitative analysis was able to predict the presence or absence of CAD as accurately as visual analysis. Odds ratios were 1.07 and 1.32 respectively (P < 0.001) for both quantitative maximal uptake and visual uptake analysis. Sensitivity of visual analysis alone was 89%, increasing to 96% with combined visual/quantitative analysis. Specificity of visual analysis decreased from 79% to 64% with combined analysis.
