Ocular hypertelorism, facial basal cell carcinomas, and multiple odontogenic keratocysts of the jaws.

Dentists should be aware of the multiple jaw cysts of NBCS that may develop in patients as young as 7 to 8 years. As the jaw cysts frequently are the initial symptom, dentists have the opportunity to detect the syndrome early, before the patients seek medical treatment. Any young patient with multiple odontogenic keratocysts in the jaws can develop other conditions characteristic of the disease. A careful family history should be taken, and the patient should be monitored closely for subsequent clinical manifestations of the disease.

The dental management of the patient with dementia.

There has been a dramatic increase in the incidence of dementia as the percentage of aging Americans has increased. Approximately half of these persons are living at home with their spouses or in the homes of their children. This increasing group of patients will be seeking care from community-based dentists. An understanding of the diagnosis, the treatment, the course, and the prognosis of the disease process will better prepare dentists to meet the unique needs of this group of patients.

Alcoholism and dental management.

Alcohol is widely used and abused in the United States. Because alcohol affects all parts of the body, it is important for dentists as well as all other medical practitioners to be aware of the signs of alcoholism. Some dental problems occur more frequently in alcoholic patients, and often general dental care for these patients must be modified.

Dental management considerations for the patient with post-traumatic stress disorder.

Vietnam veterans with post-traumatic stress disorder are frequently encountered in the dental office. They are often anxious, hostile, depressed, withdrawn, or resistant to treatment. Their psychological set and relatively high incidence of alcohol and drug-related problems frequently require modification of their dental therapy. An attractive and biologically sound restoration of the orofacial structures may improve long-term rehabilitation by enhancing self-esteem and social interactions.

Asymptomatic red lesion in anterior floor of mouth: diagnosis and management.

We have presented a case illustrating that an asymptomatic, red lesion in the oral mucosa should be considered a malignant neoplasm, until proved otherwise. A biopsy and microscopic examination of such lesions are necessary to establish a definitive diagnosis. An appropriate method of obtaining a tissue specimen from the floor of the mouth has been described, and the importance of taking a comprehensive clinical history has been reviewed. The value of transmitting this information to the pathologist, who examines the removed tissue, has been stressed. Small, asymptomatic erythroplastic lesions of the oral mucosa that have not resolved or responded to treatment with two weeks should have a biopsy examination.

Mental nerve neuropathy: a complication of sickle-cell crisis.

Anesthesia of the mental nerve, a rare complication of sickle-cell disease, has been reported. This is the first documented and reported case occurring in the United States. A review of the basic pathophysiology of the disease has been presented.

Systemic lupus erythematosus.

A case of osteomyelitis of the mandible in a patient with systemic lupus erythematosus is described. Both the disease process and the treatment modalities must be understood for correct management.
