[Expression of tissue factor and vascular endothelial growth factor in colorectal carcinoma].

Tissue factor (TF) and vascular endothelial growth factor (VEGF) play an important role in tumor progression and metastasis. We analyzed their expression in the carcinoma and normal mucosa of 53 colorectal cancer patients. VEGF levels were significantly higher in the tumor and correlated with TF expression. No correlation was found with tumor stage. TF may influence tumor growth and metastasis by modulating VEGF expression and neoangiogenesis.

[Anatomical hepatic resection using radiofrequency thermoablation in the treatment of primary or secondary liver tumors].

Radiofrequency thermoablation (RFA) in performing parenchymal liver section has been tested. Eleven patients with primary cancer or colorectal cancer metastasis underwent anatomical major liver resection after vascular control. Parenchymal section was performed by knife after coagulative necrosis. No deaths, clinical or technical complications or blood losses occurred. Such technique is feasible and effective.

Communicating oesophageal duplication: a case report.

Duplications of the oesophagus are rare congenital abnormalities and rarely communicate with the oesophageal lumen. They are commonly associated with other congenital malformations, such as spinal deformities, congenital heart disease, vertebral anomalies, malrotation of the bowel, Meckel's diverticulum. During a percutaneous endoscopic gastrostomy, performed because of a neurological dysphagia, the endoscopy revealed a very rare case of a 26-year old man affected by Klippel-Trenaunay syndrome, with an asymptomatic oesophageal duplication that communicated proximally and distally.

[Computerization of a clinical chart of perinatal data].

A computerized protocol, PADOVA-BPD, has been set-up to study prematures affected by RDS, PDA and/or BPD. This protocol consists of a Data Bank, BPDdata, where the information of traditional clinical chart has been stored and of the BPDPadova program, written for this purpose in FORTRAN 77, that allows a wide band of clinical and statistical analysis.

Radiation treatment plus CCNU plus the radiosensitizer lonidamine in malignant gliomas operated.

From June 1988 to January 1990, 28 patients with primary brain tumours were operated and treated with radiotherapy (RT) (50 Gy whole brain + 10 Gy boost to tumour bed) + cyclohexylnitrosourea (CCNU) 130 mg/msq p.o. every 6 weeks + the radiosensitizer Lonidamine (LND) (150 mg T.I.D. for the whole duration of treatment). Myelotoxicity of this regimen was acceptable, with two cases of grade IV leukopenia and thrombocytopenia requiring discontinuation of treatment. LND was discontinued in 6 patients for major toxicity (myalgias and/or testicular pain), and 3 additional patients required dose reduction of this drug. The median follow-up time of the patients on study was 12 months. The median survival time (MST) was 5 months for grade IV astrocytomas (n = 8) and 16 months for grade III lesions (= 20). No correlation was seen between survival of patients and DNA content, measured by flow cytometry, or levels of O6- alkylguanine-DNA alkyltransferase, an enzyme that repairs the CCNU-induced DNA damage.

[Primary hepatic actinomycosis: a clinical case report and review of the literature].

Primitive actinomycotic abscess of the liver is extremely rare: in the world literature are reported twenty cases. The authors report a case of a 73-year-old patient admitted at our hospital in poor general conditions, with twelve kilograms weight loss in the last year, recurrent fever and tenderness in right hypochondrium. CT scan, ultrasonography and angiography showed the presence of a seven-centimeters hypovascularized mass in the fourth segment of the liver. The patient was submitted to surgical segmentectomy of the fourth segment. The histologic examination and the histochemical colorations made on the specimen have given the diagnosis of hepatic actinomycotic abscess. No clinical evidence of other intra-abdominal or extra-abdominal localizations were found. From 1928 to date only twenty similar cases have been reported in the world literature. The mean time between the beginning of the symptoms and the diagnosis was 11.1 months (range: 2-54 months): this to confirm the diagnostic difficulties of the disease. The hepatic lesions are described as single in eight cases and multiple in thirteen. Thirteen patients have been treated only with medical therapy: nine recovered and four died for disease spread. Of the six patients with resectable lesion treated surgically, five recovered and one died for pulmonary recurrence.

[The echo-guided percutaneous therapy of dysontogenetic liver cysts].

Congenital hepatic cysts are a frequent disease, symptomatic in 16-18% of the cases. The Authors report their personal experience about 4 cases and they make a review about 56 cases treated with ultrasonically-guided therapy. In 20 cases the therapy was simple aspiration, in 39 was alcohol sclerotherapy and in 1 was external percutaneous drainage. All the 20 cysts treated with simple aspiration recurred after 0.5-24 months. All the 39 patients treated with alcohol sclerotherapy presented a complete remission of symptoms and are disease-free after 1-48 months. The patient treated with percutaneous drainage, is disease-free after 10 months from the procedure. The alcohol sclerotherapy is the treatment of choice for the congenital hepatic cysts. The simple aspiration is an effective procedure only from a diagnostic point of view but not therapeutic. The external percutaneous drainage is the treatment of choice in case of infected cysts. Surgery is indicated only in the cysts with acute presentation, with biliary content and in case of failure of alcohol sclerotherapy.

[The role of echo-guided percutaneous cholecystectomy in acute cholecystitis].

Personal experience in the treatment of acute cholecystitis with percutaneous cholecystostomy in high risk patients and in elderly patients is reported. Between January 1989 and November 1990, 28 patients affected by acute cholecystitis were treated with percutaneous cholecystostomy at Emergency Surgery Department, Verona University Hospital. The patients treated included 13 men and 15 women; 8 of them were under 70 years old, 5 between 70th and 75th and the remaining patients over 75 years old. The suspected clinical diagnosis of acute cholecystitis was confirmed in all cases by ultrasonography (accuracy 95.4%). The percutaneous cholecystostomy was successful in 26 over 28 cases. In all these cases patients had a sudden improvement of their clinical conditions. In one case we failed because the guide-wire slipped out of the gallbladder and we couldn't perform a second attempt for the patient's refuse; in an other case there was the dislodgment of the catheter after less than 12 hours from the cholecystostomy and the patient was operated on. Twenty-two of 26 patients whose conditions were improved by percutaneous cholecystostomy, subsequently underwent elective cholecystectomy. In 2 cases of acalculous cholecystitis the patients did not undergo the operation; in 2 cases because of the elderly age of the patients and their bad cardiorespiratory conditions we preferred not to perform the operation. We had not major complications; 6 patients complained pain irradiating to right shoulder which disappeared within 30-60 minutes from the end of the procedure.(ABSTRACT TRUNCATED AT 250 WORDS)

[The conservative treatment of hepatic abscesses with echo-guided transcutaneous drainage].

Liver abscesses are a rare disease with 80-100% mortality in untreated cases. The advent of ultrasound in the clinical practice has improved the prognosis. Surgery for this disease has a percentage of complications of 15-52% and a mortality of 12-33%. In a review of the literature about 200 liver abscesses treated with ultrasound-guided percutaneous drainage the complications were 7.5% and the mortality was 3%. The authors report their personal experience about 21 hepatic abscesses in 13 patients. Nineteen abscesses were treated with ultrasound-guided percutaneous drainage and two with antibiotics. Six patients had pyogenic liver abscesses, two amebic and in five patients the cultures were sterile. In 9 cases the location was in the right lobe of the liver, in 3 was in the left lobe. One patient had multiple abscesses. The size of the abscesses ranged from 0.5 to 13 cm. We drained with Seldinger technique or direct procedure with Trocar's needle under ultrasound guidance without serious complications and without mortality. In 6 cases the patients had pain during the procedure and in 3 cases medical therapy was necessary. In one case we had a pleuritis for the puncture of costophrenic space. All the patients treated had a complete resolution of the abscesses. The ultrasound-guided percutaneous drainage is the treatment of choice for liver abscesses. But other reports with more patients are necessary to clarify the indications and the ultrasound criteria of recovery.

[Leiomyoma of the nipple (a case report and review of the literature)].

The leiomyoma of the nipple is a very rare affection. No more than 20 cases are reported in the literature of the last 50 years. This neoplasm presents a difficult differential diagnosis with the remaining phlogistic and neoplastic breast diseases. We report a personal case with a review of the literature.

[The therapy of symptomatic and/or complicated dysontogenetic liver cysts].

Congenital hepatic cysts are a frequent disease, symptomatic in the 16-18% of the cases. The authors report a review of the literature of the last 20 years about 135 treated surgically patients with symptomatic and/or complicated hepatic cysts. They analyse the different models of treatment both surgical and conservative, referring in special way about the alcoholization of the cysts. The authors finally report their personal experience about eight cases.

[The perforation of a para-Vater's duodenal diverticulum (a report of 2 clinical cases)].

Duodenal diverticulum is not a rare disease, with a frequency of 1-3% in radiologic review and of 2.8-20% in pathology review; The spontaneous perforation of a perivaterian duodenal diverticulum is a rare complication, with about 100 cases reported in the literature. The post-traumatic perforation of a perivaterian diverticulum is an exceptional event, reported in single cases of the oriental literature. The authors report their personal experience about two cases of diverticular duodenal perforation, one spontaneous, the other post-traumatic, discussing the surgical problems that this rare complication present.
