[Thienylic acid, a new drug with saluretic and uricosuric activity. Preliminary data].

The effects of ticrynafen (250-500 mg) on salt-water and uric acid metabolism have been studied in 18 patients with no haemodinamic abnormalities or salt-water repletion (cardiac failure, oedema). The main results are: -- an effective natriuresis is observed in the first days and is attenuated thereafter. In subjects with a reduced GFR, a negative salt balance is obtained altough the volume of diuresis is not significantly increased. -- The potassium loss is variable according to dosage (maximum at 500 mg), renal function (low when reduced). -- The increase of urinary uric acid excretion and the lowering of blood uric acid concentration are rapid and prolonged. In conclusion, we confirm the effective natriuretic and uricosuric properties of ticrynafen.

[Acid-base modifications induced by treatment with concentration-reinfusion of ascitis fluid in cyrrhosis (author's transl)].

In 9 cyrrhotic patients with ascites we have studied the acid base status and the renal acidogenic capacity (urinary titrable acidity, ammonia) before, during and after reinfusion of concentrated ascitic fluid. Acid-base parameters have been evaluated also in the ascitic fluid and in the concentrated reinfusion fluid. The treatment does not determine any significant variation of acid base equilibrium in the cyrrhotic patients, while there is a remarkable loosing of CO2 with lowering of pCO2 in the concentrated ascitic fluid. We discuss the main physiopathological factors involved in such a type of treatment.

[Adherence to dietetic treatment, the nutritional metabolic status and the progression of chronic kidney failure].

Effects of two protein restricted diets on dietary compliance, nutritional and metabolic state, and progression of chronic renal failure (CRF) were investigated. Twenty-one patients with CRF were randomly assigned to either a conventional low protein diet (0.6 g of protein/kg b.w./day) or to a very low protein diet, providing 0.4 g of protein/kg b.w./day, supplemented with a mixture of essential amino acids which contained HIS, TYR and a high proportion of branched chain amino acids. Nutrition, assessed by body weight, anthropometry, serum protein levels and nitrogen balance studies, was maintained in all patients. Some metabolic abnormalities of CRF (i.e., secondary hyperparathyroidism, glucose intolerance) improved in both groups. The supplemented diet provided better adherence to protein prescription, corrected the depletion of VAL and LEU in muscle and was more effective than conventional diet in slowing the rate of progression of CFR.

[Mandibular laterodeviation of surgical significance].

The Authors look at the mandibular laterodeviations and their classification. They speak about the combined surgical-orthodontic treatment of this pathology and show some clinical cases from their clinical experience.

[Physiopathological and clinical features of hypernatremia].

The observation of hypernatriaemia in a large series under a variety of clinical conditions (gastrointestinal loss, increased "forced loss", and insufficiency parenteral feeding) is made the occasional for an overview of its main physiopathological and clinical features. From the physiopathological standpoint, the syndrome is rarely coupled with an absolute water deficiency. More commonly, sodium and water losses go together to bring about a fall in both intracellular and extracellular volume. The clinical significance of hypernatriaemia is essentially linked to increased osmolar content, not only of the extracellular liquid, but more particularly (due to water depletion) of the intracellular compartment, which underlies the establishment and progress of the anatomopathological lesions and the clinical symptoms.
