[Measuring ionized calcium (Ca2+) transport during intermittent peritoneal dialysis (IPD)].

The aim of studies was an evaluation of dialysate/plasma ratio and peritoneal balance of Ca2+ during IPD performed using 1.5 g% glucose and 1.75 mmol/l Ca2+ dialysis solution. Net ultrafiltration rate and EPBF were simultaneously examined. Studies were carried out using dialysis solution without any drugs except heparin (n = 10) and with intraperitoneal administration of sodium nitroprusside (n = 11), chlorpromazine (n = 10) and isoprenaline hydrochloride (n = 10) in the doses of 5, 2.5 and 0.5 mg/l of dialysis solution, respectively. During IPD conducted with 1.75 mmol/l Ca2+ dialysis solution, in 2-7% of patients hypercalcaemia (Ca2+ > 1.5 mmol/l) was present. It was promoted by Ca2+ absorption from dialysis solution to blood, observed in all patients. Chlorpromazine but not sodium nitroprusside or isoprenaline hydrochloride influenced significantly peritoneal transfer of Ca2+, decreasing its absorption from the peritoneal cavity. Chlorpromazine-induced changes in peritoneal transfer of Ca2+ were not a result of the drug influence on EPBF, which was not significantly different in patients dialysed without and with drugs. Ultrafiltration rate induced by 1.5 g% glucose dialysis solution did not exert significant influence on Ca2+ transport.

[Peritoneal blood flow, peritoneal transfer and continuous ambulatory peritoneal dialysis].

The aim of studies was an evaluation of effective peritoneal blood flow (EPBF) in patients treated with continuous ambulatory peritoneal dialysis (CAPD) and determination if there is a dependence between EPBF, peritoneal transfer and CAPD adequacy. In 14 patients (3 women and 11 men) treated with CAPD through 8.0 +/- 6.1 months the examinations of EPBF using diffusive peritoneal mass transfer coefficient of CO2, CAPD adequacy and peritoneal transfer during the first 2.5 minute dwell of dialysis solution in peritoneal cavity were performed. In 10 patients above mentioned examinations were repeated after 4.7 +/- 0.3 months of CAPD treatment. EPBF after 14 +/- 6.2 months of CAPD treatment showed increasing tendency (460 +/- 250 ml/min) comparing to values obtained at the beginning of treatment (285 +/- 150 ml/min). There was no significant dependence on peritoneal transfer rates of small molecules (sodium, potassium, urea and uric acid) in first 2.5 minutes dwell of dialysis solution in peritoneal cavity on EPBF. At EPBF greater than 200 ml/min excretion of total protein with dialysate and clearances of urea and creatinine showed positive dependence on EPBF. The obtained results indicate that both transperitoneal excretion of total protein and CAPD adequacy are dependent on peritoneal blood flow.

[Evaluation of skeletal muscle function during calcitriol treatment in patients dialyzed for uremia].

The purpose of the present study is to investigate whether Rocaltrol therapy in uremic patients undergoing dialysis treatment can improve muscle function. In 8 uramic patients (2--on CAPD, 6-on HD treatment) calcitriol (Rocaltrol, Roche) was given in the dosis at 1 mcg/day during 15 months. At the beginning of therapy and every month the following parameters were determined in serum: creatinine (Cr), calcium (Ca), phosphate (PO4) and parathyroid hormone (PTH). At the beginning and at the end of treatment the quantitative electromyography (EMG) was performed. We observed a slight increase of serum Cr during 15 months of treatment in 4 patients. Serum Ca, PO4 and PTH did not changed significantly. The EMG revealed abnormal polyphasic motor nerve unit potentials of brief durations, decrease in the amplitude and fibrillations potentials. The EMG findings did not change after Rocaltrol therapy, but all patients on physical examinations exhibited disappearance of clinical manifestations of uremic myopathy. In conclusion, our findings suggest that vit. D deficiency is one of the causes of uremic myopathy and a careful treatment with Rocaltrol can diminish muscle weakness in uremic patients.

Does calcitriol therapy improve muscle function in uremic patients.

The purpose of the present study is to investigate whether calcitriol therapy in uremic patients undergoing dialysis treatment can improve muscle function. In 8 uremic patients [2 on continuous ambulatory peritoneal dialysis (CAPD), 6 on hemodialysis (HD) treatment], calcitriol (Calcitriol, Roche) was given in the dose of 1 microgram/day for 15 months. At the beginning of therapy and every month, the following parameters in serum were determined: creatinine (Cr), calcium (Ca), phosphate (P), and parathyroid hormone (PTH). At the beginning and at the end of treatment, quantitative electromyography (EMG) was performed. We observed a slight increase of serum Cr during 15 months of treatment in 4 patients. Serum Ca, P, and PTH did not change significantly. The EMG revealed abnormal polyphasic motor nerve unit potentials of brief durations, a decrease in the amplitude, and fibrillation potentials. The EMG findings did not change significantly after calcitriol therapy, but all patients on physical examinations exhibited the disappearance of clinical manifestations of uremic myopathy. In conclusion, our findings suggest that vitamin D deficiency is one of the causes of uremic myopathy and a careful treatment with calcitriol can diminish muscle weakness in uremic patients.

[Peritoneal blood flow and peritoneal transfer parameters during intermittent peritoneal dialyses performed with administration of sodium nitroprusside or chlorpromazine].

Effective peritoneal capillary blood flow (EPBF) was evaluated by calculation of diffusive mass transport coefficient of CO2 during intermittent peritoneal dialysis performed with intraperitoneal administration of sodium nitroprusside (NP, 5 mg/l of dialysis solution) or chlorpromazine (Cl-Prom, 2.5 mg/l of dialysis solution). Peritoneal transfer rates of substances of different molecular size and charge were simultaneously examined and compared with EPBF. During NP or Cl-Prom administration, EPBF was respectively 220 +/- 40 (n = 11) and 224 +/- 29 (n = 10) ml/min. These values were insignificantly higher than those obtained during dialyses without drugs (169 +/- 16 ml/min, n = 10). NP and Cl-Prom did not cause any significant changes in peritoneal transfer of CO2. Transfer rates of HCO3 and tCO2 were increased only with NP. Both drugs enhanced peritoneal removal of Na+, K+, urea, uric acid and total protein, but with similar EPBF during the use of both drugs, the effect of NP was more pronounced. The obtained results indicate that EPBF is not a major factor responsible for enhanced peritoneal transfer rates during dialyses performed with NP or Cl-Prom.

[Effect of furosemide added to peritoneal dialysis fluid on hydroxyproline excretion in dialysate and urine].

The effect of furosemide (1500 mg/21 of peritoneal dialysis fluid) on excretion of hydroxyproline in dialysate and its renal clearance was investigated in 12 patients with end-stage renal disease on regular dialysis treatment. The 37 +/- 8% and 20 +/- 6% increase of D/S ratio of hydroxyproline after 5 and 8 hours of dwell, respectively, and 52 +/- 24% increase of renal clearance of hydroxyproline were induced by furosemide, which indicate its significant influence on peritoneal and renal removal of hydroxyproline.

[Comparison of indices of kinetic modelling of urea and creatinine as well as evaluation of nutritional status of patients treated with continuous ambulatory peritoneal dialysis].

The purpose of this study is to evaluate the adequacy of continuous ambulatory peritoneal dialysis (CAPD) using urea and creatinine kinetics as well as nutritional indices in 5 male patients treated with CAPD (111 +/- 9 ml of dialysis solution per kg b.w. per day) during 6.9 +/- 1.3 months. For kinetic evaluation of CAPD the following parameters were used: weekly Kt/V, normalized protein catabolic rate (NPCR, g/kg b.w./day), total creatinine clearance (Cl cr, l/wk/1.73 m2) and efficacy number (1/g cr/day). Dietary protein intake (DPI, g/kg b.w./day), daily energy intake (DEI, kcal/kg b.w.), lean body mass (LBM, % b.w. and plasma albumin (PA, g/dl) were indices of nutrition. Results (mean +/- SD) of kinetic parameters indicate adequate (A), marginal (M) or inadequate (NA) evaluation of CAPD: Kt/V of 1.5 +/- 0.3 (M/NA), NPCR of 0.76 +/- 0.13 (M/NA), Cl cr of 64 +/- 25 (A/M) and EN of 5.9 +/- 0.9 (A/M). Urea and creatinine kinetics were accompanied by DPI of 1.07 +/- 0.35, DEI of 34 +/- 12 and PA of 3.73 +/- 0.38. These parameters, although obtained in overweight patients (LBM of 77 +/- 11% b.w.), indicate marginal nutritional status. Our results show that urea kinetics (Kt/V and NPCR) are more rigorous parameters in evaluation of CAPD adequacy than creatinine kinetics (Cr Cl and EN).

[Effect of sulfinpyrazone on peritoneal and renal clearance in patients with chronic renal failure treated by intermittent peritoneal dialysis].

The paper was performed in order to evaluate the influence of sulfinpyrazone given orally in the doses of 400 mg to 16 uraemic patients during intermittent peritoneal dialysis on transperitoneal and renal transfer of different molecular weight substances. It was shown that sulfinpyrazone causes the increase in dialysate excretion of uric acid, sodium and protein but does not influence the transperitoneal transfer of water, potassium, creatinine, glucose, heparin and acetic acid. Renal function determined by diuresis volume as well as clearance of sodium, potassium, creatinine, uric acid and protein remained unchanged after the drug administration. Thus, when sulfinpyrazone is used during peritoneal dialysis a possibility of its action on the peritoneal membrane permeability should be taken into consideration.
