Infantile cystinosis and insulin-dependent diabetes mellitus.

A 13-month-old infant was admitted to our Institution because of difficult metabolic control of diabetes mellitus. Clinical and laboratory findings revealed that the child was affected by both insulin-dependent diabetes mellitus and nephropathic cystinosis. Treatment with indomethacin was associated with growth improvement at an early stage of renal insufficiency, but not in more advanced renal failure.

Platelet-derived growth factor(s) mitogenic activity in patients with myeloproliferative disease.

Platelet-derived growth factor has been invoked in the pathogenesis of medullary fibrosis during myeloproliferative disorders. In this study we compared the mitogenic activity of heat-stable platelet-growth factor(s) from 13 patients suffering from myeloproliferative disorders with that of a normal group. The test was carried out on Go growth arrested Balb/c 3T3 fibroblasts incubated with various concentrations of platelet extracts, determining the entrance into the S phase by means of [14C]thymidine uptake. The incorporation curves of [14C]thymidine by the fibroblast culture, under the effect of pathological extracts, were consistently lower than the control curve, indicating a lower level of PDGF(s) in platelets from patients. The greatest depression of this activity was found to be associated with highest degree of medullary fibrosis (agnogenic myeloid metaplasia patient group), in agreement with the hypothesis that fibroblast activation within bone marrow during myeloproliferative disorders might be correlated with a PDGF(s) release in the bone marrow environment.

Possible sources of error in the calculation of ferrokinetic parameters using Ricketts' triexponential model.

The triexponential model developed by Ricketts et al. in 1975 for the calculation of ferrokinetic parameters has been widely used to obtain quantitative information about effective and ineffective erythropoiesis. However, a calculation error in one of the formulae of this method may have a considerable effect on those results which depend on the value of the third exponential constant, gamma 3. Moreover, the analysis of plasma 59Fe activity needs to be based on good counting statistics, which lambda-counting seldom provides after the first 24 h; therefore, 59Fe beta-counting is considered to be the best method.

Different patterns of platelet turnover in chronic idiopathic thrombocytopenic purpura.

Platelet turnover, platelet production, platelet mean life span (MLS), platelet count, mean platelet volume (MPV) and platelet-associated antibodies have been examined in 26 patients with chronic idiopathic thrombocytopenic purpura (ITP) and in 1 patient with hypomegakaryocytic thrombocytopenia (HT). 15 ITP patients had normal or increased platelet turnover and platelet production, while 11 had subnormal values despite shortened MLS, while the patient with HT had normal MLS. The differences between the two groups with high and low platelet turnover were statistically significant. No correlation was found between kinetics parameters and bone marrow pattern in a total of 19 patients examined. These data suggest that in some cases of chronic ITP, the pathogenesis of thrombocytopenia can be due not only to the peripheral destruction of platelets, but also to a deficient platelet production by megakaryocytes. Since the number of megakaryocytes in bone marrow slides is not decreased in the low turnover compared with the high turnover group, it is possible that an impaired pattern of megakaryocyte maturation be the cause of the low platelet production in these patients, unlike in the HT patient where megakaryocytes are almost absent.

1-(2-Chloroethyl)-cyclohexyl-nitrosourea-induced remission in essential thrombocythemia.

14 patients with essential thrombocythemia (ET) and 1 patient with agnogenic myeloid metaplasia received (1-(2-chloroethyl)-cyclohexyl-nitrosourea (CCNU), 80 mg/m2, p.o., in a single dose once a month up to three times. Remission was achieved in 13 patients (86.6%) and easily maintained by low-dose busulfan (2 mg twice a week). The data on the treatment of ET reported in the literature have been reviewed and compared to our results.

Scanning electron microscopy in the diagnosis of bronchogenic squamous cell and small cell anaplastic carcinoma.

The authors believe that differential diagnostic aspects between poorly-differentiated squamous cell carcinoma (PDS) and small cell anaplastic carcinoma (SCA) may be shown by scanning electron microscopy (SEM) analysis of 'thick' sections from routine paraffin-embedded tissues. In 8 'sample' cases features were demonstrated which were typical of PDS (well-defined outlines of cells, intercellular bridges) and of SCA (fairly smooth cell surface, nuclei in relief, a spongy, granular-like appearance of cytoplasm).

Bronchogenic carcinoma: survival after surgical treatment according to stage, histologic type and immunomorphologic changes in regional lymph nodes.

The survival rates of 90 patients who underwent operation for bronchogenic carcinoma were analyzed statistically according to sex, age, tumor stage and histologic type, types of surgical procedures (lobectomy or pneumonectomy), and to immunomorphologic parameters of immunologic activity in peribronchial and hilar lymph nodes. The Stage I group survived significantly longer than did the Stage II and III groups, the lobectomized patients survived significantly longer than the pneumonectomized patients. The absence or presence of lymph node metastases was one of the major determinants of survival. The mean values (percent of total node cut surface) of lymph node sinus histiocytosis and of paracortical area could be correlated directly to survival in each of the histologic tumor-type groups, while the development of follicular cortex and germinal centers correlated inversely with survival. Increased survival might be associated with changes concomitant with immune reactivity in lymph node T-cell areas and with the sinus histiocytosis pattern, the latter representing probably a tumor--host reaction of the delayed hypersensitivity type. By contrast, increased activity of lymph node B-cell areas tended to correlate with poor prognosis of lung cancer patients. Moreover, patterns showing germinal center hyperplasia were statistically associated with lymph node neoplastic invasion, while the pattern with sinus histiocytosis and expanded paracortical areas was statistically associated with tumor-free lymph nodes.

[Correlations between membrane and bubble oxygenators in patients treated with or without methylprednisolone. Haemodynamic and metabolic parameters (author's transl)].

We studied 64 patients who underwent cardiac surgery. Hemodynamic and metabolic variables-cardiac index (CI), total peripheral resistances (TSR), oxygen consumption and (VO2), urinary output, alveolar-arterial oxygen difference (delta A-a O2) - were related with oxygenator (bubble or membrane oxygenator) and with treatment (with or without methylprednisolone). 31 patients were perfused with membrane oxygenator (MO), 33 with bubble oxygenator (BO), 13 patients MO and 18 patients, BO were treated with methylprednisolone (mps). The analyzed variables were sampled 15 minutes after the start and one to five times during the perfusion. At first data were analyzed in terms of mean values in whole set of time intervals. Only delta A-a O2 showed statistically significant difference between the MO and the BO group (282 +/- 110 versus 361 +/- 82, p less than 0.005). Between nomps-treated and mps-treated patients statistically significant difference were found in TSR (2.134 +/- 362 versus 1.785 +/- 317, p less than 0.005) and in delta A-a O2 (293 +/- 92 versus 355 +/- 108, p less than 0.02). Secondo analysis is concerning time change of variables. The time behaviour of TSR is similar in all groups studied, but a statistically significant lower value was found in mps-treated patients in every time interval. The VO2, in the last time interval, is statistically higher in the mps-treated than in the nomps-treated patients (p less than 0.05) and furthermore, for the same temperature change, the rise of VO2 is higher in the mps-treated (p less than 0.001). The delta A-a O2 shows time increase both in the MO and in the BO group, but the MO patients start from statistically significant lower values than the BO and the behaviours remain parallel for all analyzed time. The results are interpreted and discussed in relation to hemodynamic and metabolic patterns of perfusion as good or bad tissue perfusion.
