Bone metastasis in hepatocellular carcinoma. A report of five cases and a review of the literature.

Hepatocarcinoma occurs frequently throughout the world. Bone metastases are rare although incidence has increased because of progress in diagnosis and treatment. The authors report 5 cases of bone metastases and review the literature. The spine is the most frequent localization of bone metastases. Radiotherapy is the treatment of choice for this lesion. Surgery should be used to prevent and treat complications such as nerve compression and pathologic fracture, only if the coagulative pattern and the conditions of the patient allow it. The authors recommend the use of long intramedullary nailing when localization of the disease is in the femur, with prophylactic stabilization of the neck in diaphyseal metastasis.

[Health surveillance and prevention in the service industries: the problems and outlook].

The great expansion of the services industry in the developed countries of the world is producing profound changes in the composition of the work force and, consequently, also in the health needs that are traditionally part of occupational medicine. Of all the medical disciplines, occupational medicine is most strongly affected by social and economic changes. In recent years experience has been acquired both at national and international level that can identify the new health needs in practical terms. The aim of this paper is to describe such experience with particular reference to the situation in Italy. The authors describe the shift of prevalent diseases from occupational to work-related diseases, the problems concerning the human, social and economic cost of these diseases, prevention and control strategies, relationships between company health services and the national health service and possible synergic action of the public and private sectors.

[Essential mixed cryoglobulinemia with liver involvement: a still open problem].

Essential mixed cryoglobulinemia (EMC) is a syndrome characterized by cryoglobulinemia and clinical features including purpura, arthralgia, asthenia (Meltzer-Franklin syndrome) without evidence of any systemic disease Liver involvement in the course of EMC is described in 50-84% of patients. It consists of mild silent hepatosplenomegaly and slightly rise of serum amino transferase. Eleven patients with clinical and laboratory findings suggestive for EMC (five type II and six type III) underwent percutaneous liver biopsy to evaluate the degree of liver involvement. Two liver cirrhosis, two chronic active hepatitis, one chronic persistent hepatitis and a case of hepatic steatosis were found. A type III cryoglobulinemia was present in four of the six patients with liver involvement. All the patients were Hbs Ag negative but three of them were Hbs Ab positive. The pathogenesis of liver involvement in the course of EMC is still now uncertain. The authors believe that a previous HBV infection plays no role in the pathogenesis of EMC syndrome. This syndrome must be considered different from mixed cryoglobulinemia secondary to chronic liver disease. They suggest that liver biopsy is mandatory during the course of EMC even when clinical and laboratory data are silent.

[New problems in occupational medicine: findings on the risk of coronary heart disease in the services industry].

The distribution of the major coronary risk factors: total and HDL cholesterol, blood pressure, smoking and body mass index, was studied during the course of a routine screening of 1404 male bank employees in Northern Italy. The recent Italian and international consensus conferences and particularly the results from the Italian areas of the WHO-MONICA Project were taken as reference points. There was a widespread coronary risk in the bank employees who had a 5-10% excess of cholesterol, corresponding to a 10-20% increase in the theoretical risk of coronary heart disease. Compared to the data produced by the Lipid Research Clinics (USA), coronary risk appeared higher for all the surveyed factors in the Italian general population, and particularly in bank employees. Greater stress at home and at work and above all the change from the traditional, poor Mediterranean diet to a richer diet might provide an explanation for these results. A new perspective is thus opened for occupational health due to the new health problems produced by the cultural and technological changes occurring in the country.

[Dermatopathies in the domestic worker: the Lombard experience].

In order to identify the etiopathogenesis of cutaneous diseases in housewives, a questionnaire was made, and the gathered data are here described. 1,824 people have answered this questionnaire: 1,007 have reported lesions with a suspicious immunological pathogenesis. The commercial products which were reported as a cause of the dermatitis have been 834. In a group of 142 housewives (86 with allergic contact dermatitis and 56 with irritative contact dermatitis), the allergic sensitizations to the chemicals in the detergents were tested. 21% of the cases had positive cutaneous reactions for nickel salts and 8.4% for chromium salts. The analysis in atomic absorption method of these metals in 34 commercial products has shown their presence in almost all the samples. Hypothesis are formulated to find the source of these polluting substances. The aims and the methods of a investigation carried out on 1.800 housewives, who are going to be controlled in order to estimate the prevalence, the clinical kinds and the etiopathogenesis of dermatitis related to house work, are described. The study will be carried out at the Occupational Health Institute "Clinica del Lavoro L. Devoto" of Milan and at the Dermatological Departments of the Universities of Bari and Florence.
