A multicentre trial to evaluate the efficacy and tolerability of alpha-glycerylphosphorylcholine versus cytosine diphosphocholine in patients with vascular dementia.

An open clinical trial was carried out to compare the efficacy and the tolerability of 1 g/day alpha-glycerylphosphorylcholine (alpha-GPC) with 1 g/day cytosine diphosphocholine (CDP) both given intramuscularly for 90 days in 120 patients with mild to moderate vascular dementia. The clinical evaluation, carried out at the start as well as halfway through (45 days) and at the end of treatment (90 days), was expressed by psychometric tests (modified Parkside behaviour rating scale, Sandoz clinical assessment geriatric scale, word fluency test, Hamilton's rating scale of depression, narration subtest of Wechsler memory scale). Both treatments produced a definite symptomatic improvement and showed a very good tolerability. The results suggest that in most tests alpha-GPC possessed a statistical higher efficacy and an overall more satisfactory activity assessed by both patients and investigators compared with CDP.

No effect of cloricromen on some coagulation parameters in patients with ischaemic cerebrovascular disease.

Cloricromen is a new drug that inhibits platelet aggregation in man and in experimental thrombosis. Twenty patients with a history of atherothrombotic stroke received cloricromen (100 mg, twice daily) for 30 days in order to evaluate its effects on plasma fibrinogen, antithrombin III, and other variables of the haemostatic system. A statistically significant decrease in the prothrombin time (P < 0.01) was found only after 30 days of therapy. This decrease was transient and disappeared 15 days after the end of treatment. No statistically significant changes in plasma fibrinogen levels, antithrombin III, partial thromboplastin time, or platelet count were observed compared with baseline values. No side-effects were reported. This study did not reveal an effect of cloricromen on coagulative variables in patients with cerebrovascular occlusive disease.

[Rehabilitative approach to ataxia].

The author, after visiting the most common methods in use to value the ataxic patient, gives a video camera system as a good valuation device, good to give objective, complete and verifiable data. This kind of evaluation must offer the formulation of a personalized treatment plane and can demonstrate the improvements obtained during the treatment.

[An accessory musculus soleus (symptomatic and asymptomatic). The CT findings].

The authors have reviewed 90 CT exams of the foot to identify the presence of the accessory soleus muscle, an anatomic variant corresponding to the finding of a medial backtibial tumefaction, often aching, especially of the Achilles-calcaneal-plantar system. In these cases, US is generally required to demonstrate the alteration, which was an occasional finding in the past, and to exclude the presence of inflammatory or expansile processes of the soft tissues. The accessory soleus is characterized by the presence of accessory muscle fibers, which, in variable number, caudally make for the medial aspect of the calcaneus from the antinode of the soleus muscle. The anatomic variant is much better evidenced by CT, more frequently employed than MR imaging, thanks to its easier approach. As a matter of fact, both methods are capable of defining the relationships with the adjacent tendinous-vascular-nervous structures, involved by mechanical friction in the event of muscle hypertrophy due to overload. Indeed, the review of CT examinations has pointed out a relative relationship between volumetric changes in the soleus and nonphysiological functional demands, giving further evidence of its specific role in the efficiency of the Achilles-calcaneal-plantar system. Therefore, the report in asymptomatic cases is an important prognostic factor which allows the correct clinical picture of subsequent painful symptoms.

[Ultrasonography and computerized tomography in giant Baker's cyst].

The US study of the posterior compartment of the knee usually allows the clinical suspicion of popliteal cyst to be confirmed. Nevertheless, in case of giant Baker's cysts--which are, at any rate, less frequent--US diagnosis is more difficult. CT can help determine the benign nature of the lesion, and allow an accurate spatial evaluation. Moreover CT, when accurately performed, demonstrates the caudal or cranial cystic spread, together with its clear separation from adjacent muscular tissues. Direct coronal scans are more useful to obtain better spatial definition than reformatted images.

Effects of triglycylvasopressin on portal pressure and portal bile acid concentration in normal and cirrhotic rats.

The effects of triglycylvasopressin on portal vein pressure and portal bile acid concentration after oral loading with chenodeoxycholic acid were evaluated in normal and cirrhotic rats. Triglycylvasopressin significantly reduced the portal vein pressure and portal bile acid concentration in both populations (normal: portal vein pressure 21.2%, p less than 0.05; portal bile acid concentration 43.2%, p less than 0.005; cirrhotic: portal vein pressure 19.8%, p less than 0.025; portal bile acid concentration 20.3%, p less than 0.05). Both in normal and cirrhotic rats, portal vein pressure and portal bile acid concentration resulted correlated. These results support that the combined determination of portal vein pressure and portal bile acid concentration could represent an interesting approach to the study of drugs which are potentially active on portal circulation and could indirectly provide information as to portal flow.

[Longitudinal study of 5 patients with "primary empty sella" (author's transl)].

The clinical symptoms and the radiographic findings of five female patients with "primary empty sella" were studied. The patients were treated with corticosteroid therapy and the clinical syndrome, with the exception of optic subatrophy of the left eye in one patient and of both eyes in another patients, improved in all of them. In subsequent controls, the radiographic findings did not show any evolution suggesting that the cause of the empty sella syndrome is a potentially regressive element (endocranial hypertension?). The small number of our cases does not prove with any certainty that corticosteroid treatment, was beneficial but as improvement occurred and in all the subjects this therapy should be tried in other patients with "empty sella" syndrome. The possible improvement of the empty sella syndrome has to be taken into account before deciding on surgical intervention.

[Varicose disease of the lower limbs: analysis of the most common predisposing factors in a sample of 589 patients from eastern Sicily. A retrospective study].

Authors analyze, retrospectively, the principal predisposing factors to varicose illness: age, sex, pregnancy, oral contraceptives, and labour activity, in 589 patients. They consider the correlation between subjective symptomatology and the time of varicose appearance, in order to individualize a pre-clinical status.

[Survey of childhood handicaps in the Unità Sanitaria Locale No. 22 "Valtiberina"].

An epidemiological research about the infantile handicaps has been carried out to make a quantitative and etiological valuation of the existing cases in the social, economic and cultural sphere of U.S.L. number 22 "Valtiberina". Considering an infantile population of 5,063 subjects, 55 cases, corresponding to 1.08% have been examined; 32 of them are male children and 23 are female children. The research which has been carried out shows that there are more cases of psychological handicaps (35 cases out of 55) rather than of organic handicaps. It seems that it is possible to find a connection between this high percentage and the social, economic and cultural situation of the territory. At the same time an efficacious work has been carried out by the Social and Medical Services of U.S.L. and by the school to recognise, to point out, to diagnose and to study these pathologies. Such a study can result useful if we want to plan a campaign to prevent and to treat as soon as possible the infantile handicaps.

[The course of varicose disease. Parallel retrospective study of 2 ethnic groups (Catania-Basel)].

Authors presents a retrospective study with two different ethnic groups, one of them from Catania and the other from Basilea, with varicose veins and subjected to a saphenectomy. They studies the following parameters: age at the entrance, and when varicose veins appeared; correlation between varix appearance date and subjective symptomatology; and rapport between varix complications and its duration. The possible incidence of "ambiental factor" and contrasts between the two groups are considered.
