Differential regulation of effector responses of cell mediated immunity in experimental salmonellosis.

Delayed type hypersensitivity (DTH) and protective cell mediated immunity showed different profiles with respect to time following intraperitoneal immunization with live Salmonella enteritidis. Whereas the DTH response decreased with time the Protection Index increased. The decline in DTH response was found to be associated with suppressor cells generated by intraperitoneal immunization and could be prevented by cyclophosphamide treatment prior to immunization. It was concluded that the two effector responses of cell mediated immunity were under differential regulation.

Transient neurologic disturbances, brain tumors, and normal computed tomography scans.

During a 4-year period, four patients presented with transient disturbances in neurologic function that were diagnosed as seizures in two and transient ischemic attacks in the other two. Computed tomography (CT scan), both with and without contrast, was normal in all four patients. Isotopic brain scans (3 patients), cerebral angiograms (4 patients), and lumbar punctures (4 patients) were normal. Electroencephalograms (EEG) were normal in two patients and abnormal in two patients (consisting of focal slowing). Within 4.5 months, all patients developed symptoms and signs of a brain tumor, and in all four, CT scan now revealed a large mass lesion which at surgery was shown to be a malignant astrocytoma. These four patients constituted 4% of the total number of patients with malignant astrocytomas that were seen at the NYU Medical Center during this same time period. It is stressed that the CT scan may be normal early in the course of patients with brain tumors, particularly if they present with a transient disturbance in neurologic function. The first evidence of the tumor in such patients may be a slow-wave abnormality on the EEG. Patients who are suspected of having a brain tumor should, if the initial CT scan is normal, have the scan repeated later.

Pergolide for the treatment of pituitary tumors secreting prolactin or growth hormone.

We gave pergolide mesylate, a new long-acting ergot derivative with dopaminergic properties, to 47 patients with hypersecretion of prolactin or growth hormone. Single doses produced long-lasting reductions of serum prolactin levels; after 24 hours, the values remained depressed at a mean of 28.8 per cent of the base-line value. Among 41 patients (22 women and 19 men) with hyperprolactinemia who took pergolide for three months or more, prolactin levels fell to normal in 37 and remained slightly elevated in 2. In the two patients in whom the levels fell to only 38 to 52 per cent of base line, treatment was regarded as a failure. The level of growth hormone fell to a mean of 52.8 per cent of base line in patients with acromegaly who were taking 100 micrograms of pergolide per day. Among patients for whom adequate CT scans were available, definite tumor shrinkage occurred in 10 of 13 with macroadenomas and definite or probable shrinkage in 5 of 9 with microadenomas. Menses returned in 76 per cent of treated women and testosterone levels rose in 10 of 14 men. We conclude that pergolide reduces hypersecretion and shrinks most prolactin-secreting macroadenomas. In some patients long-term pergolide therapy may be superior to surgery and x-ray treatment.

Long term survival among patients with malignant brain tumors.

Eight of 57 patients (14%) with malignant astrocytomas lived at least 2 years. The mean survival time was 143 weeks (range, 104 to 250 weeks). All of the patients were treated with operation, radiation, and chemotherapy. Four of the 8 patients died because of tumor recurrence, 1 died from a second primary tumor, 2 died of cases unrelated to the tumor, and 1 is still alive. Diffuse cortical dysfunction associated with cortical atrophy that could not be related to tumor regrowth and was not explained by focal deficits, psychotic of depressive thought disorders, metabolic or endocrine abnormalities, or hydrocephalus developed in the 3 longest-surviving patients. The diffuse dysfunction was initially apparent only through psychometric testing, but eventually became so disabling as to result in 2 of the 3 patients retiring from work. Although small, but gratifying, gains have been made in the treatment of patients with malignant brain tumors, accompanying these gains have been problems of a magnitude that is only now beginning to be appreciated.

Spirorchidiasis in loggerhead sea turtles (Caretta caretta): pathology.

Loggerhead sea turtles (Caretta caretta) from the Atlantic seaboard (Florida to Massachusetts) were examined at the Marine Pathology Laboratory, University of Rhode Island, from March through December, 1980. Three genera of blood flukes (spirorchids) were found in 14 (33%) of the 43 turtles. Gross signs in heavily infected animals included cachexia, anemia and enteritis. Histopathological lesions were similar to those present in homeotherms with schistosomiasis. Granulomatous gastritis, enteritis, hepatitis, pneumonitis, and nephritis were present. Acute and chronic vasculitis accompanied metastasis of eggs. Infected animals had severe hepatic hemosiderosis, indicative of the anemia observed grossly. Evidence is presented that spirorchidiasis is prevelent in sub-adult loggerhead sea turtles, is responsible for extensive lesions and may be responsible for significant debilitation and mortality.

A measure of community health needs and actions in a rural area of Iraq. The Abu-al-Khasib experience.

A longitudinal enquiry was carried out among randomly selected households in four villages in Abu-al-Khasib district of Iraq, to study health needs as perceived by the community and actions people take when the need arises. Almost three-quarters of the spells of sickness as perceived by the community were among mothers and children. Most of the morbidity was found to be within the scope of preventive services at the local health centre level. Health centre services were underutilised. Only half of the spells of sickness were reported to the local health centres for medical care; a reason for this phenomenon is the emerging preference for care at the hands of clinical specialists. The action the community takes when a health need arises depends on their perception and this is determined by rapid social and educational development. The need to increase demand for normal care, especially that of mother and child, is stressed; this care may be strengthened through introduction of a domiciliary component.

A diagnostic study of pregnancy experiences of married women in Iraq.

A pilot survey was recently undertaken among 301 married women below 50 years of age and having been pregnant at least once. These women lived in a rural environment: the first group in a peri-urban area which is developing rapidly, and the second in a traditional rural area. The survey elicited information on various aspects of the 1,476 pregnancies reported by the women in the sample, including infant mortality, complications in pregnancy, abortions, as well as the number of medical consultations. Data show that mother and child health services, on the whole, are not fully appreciated by mothers, who only visit the clinic when they have a problem. It is evident that health care is equated with curative treatment and that the positive dimensions of prevention and health maintenance are not properly perceived. The fact that in the more conservative areas 98.1% of births take place at home shows the existence of a traditional health system which is not sufficiently taken into account by health planners. In fact, local birth attendants and midwives represent for the health services untapped manpower which, if adequately trained, could help to eliminate the risks currently associated with home births.

Effects of adenosine 5'-diphosphate on bovine glutamate dehydrogenase: diethyl pyrocarbonate modification.

Initial rate kinetic studies and reduced coenzyme binding studies with bovine glutamate dehydrogenase have shown that an enzyme-NAD(P)H-glutamate abortive complex is a major participant in the overall enzyme-catalyzed reaction. The allosteric regulator ADP is shown to activate the enzyme by destabilizing this abortive complex. Destabilization of this abortive complex with concomitant activation is also achieved by the ethoxyformylation of a single histidine residue per subunit in the hexameric enzyme. ADP, in addition to its activatory effects, is shown to (a) remove the nonlinearity from the Lineweaver-Burk plots which is attributable to negative cooperativity and (b) inhibit the enzyme as a competitive inhibitor with respect to coenzyme under the appropriate conditions. Ethoxyformylation with diethyl pyrocarbonate, which mimics the activatory effects of ADP, has no effect on the negative cooperativity shown by this enzyme. A model for the action of ADP is proposed in which ADP activates glutamate dehydrogenase by binding to a regulatory binding site and blocks the negative cooperativity by mimicking the natural coenzyme at the active site of the enzyme.
