Cytopuncture in the follow-up of breast carcinoma.

Between 1970 and 1979, at the Centre Anti-Cancéreux René Huguenin, 118 lymph nodes and 135 subcutaneous nodules were investigated by fine needle puncture in the follow-up of patients previously treated for breast carcinoma. Irradiated or contralateral breasts were not included in this study. Fine-needle puncture without aspiration was generally used. The results indicated a good correlation between cytology and histology or between cytology and the clinical course when there was no surgical control. This method is of particular interest in the differential diagnosis between recurrent carcinoma nodules and foreign body reactions on surgical scars or miscellaneous lesions not connected with breast carcinomas.

[The risk factors and prevention of recurrences in rectal surgery].

The authors report their own experience of 34 patients operated on for rectal cancer. In 8 cases abdomino perineal resection and in 26 anterior resection with anastomoses (6 in second time) were performed. Operatory mortality was nil and with an average follow up of 34 months 5 local recurrences were observed 16%. Among the risk factors examined the most important seems to be the stage of the disease; imperative it is therefore a correct surgical approach which warranted a significant reduction of local recurrences. This surgical approach includes a safety distal clearance from the tumour (at least 2.5 cm.), radical lymphadenectomy and complete removal of the mesorectum.

[Concentration of asbestos fibers in urothelial tumors and in the neoplasm-free bladder wall].

The aim of our study was to search the eventual presence of asbestos fibers in the neoplastic tissue of 13 cases of bladder carcinoma and in the normal vesical tissue of 12 control cases without neoplasia and operated upon for other urologic pathologies. To all patients a standardised questionnaire has been administered in order to ascertain the entity of the eventual exposition to asbestos. The organic component of the tissues has been oxidated at low temperature in atomic oxygen plasma and the residual inorganic component has been analysed by transmission electronic microscopy and energy dispersion spectrometry. We have found asbestos fibers only in two over the 25 examined samples; in both cases they belonged to patients affected by the neoplasia and in one of them also by pulmonary asbestos. The procedures that we used in this study, in respect to our other experiences, allowed us to reduce the risk of contamination of the tissues by asbestos, as it is shown by the lower number of cases with positive finding of fibers and by lower concentration of fibers in the positive cases.

[Warthin's tumor. Description of 2 cases].

Warthin's tumor is a debated pathological condition. There are many controversies concerning this disease such as its origin (hereditary or acquired), its nature (whether neoplastic or no), its topographical relation with the salivary glands, its relationship with possible concomitant pathologies (dysmetabolic, infective, neoplastic, etc.), the problems of diagnosis and of differential diagnosis with all possible causes of cervical swelling, and also the problems of surgical treatment in relation to the possible involvement of the salivary glands. In this paper two clinical cases are described. The first one is characterized by a latero-cervical bilateral Warthin's tumor, apparently independent of the salivary glands, associated with rhinopharyngeal lymphatic hyperplasia and nodular colloid goiter. The second case is characterized by a latero-cervical uni-lateral Warthin's tumor, linked by a slender layer with the ipsilateral parotid gland, associated with breast cancer. Starting point from the discussion of these two cases, the Authors make remarks in order to answer some of the aforementioned questions.

[The role of fine needle biopsy in the cytologic diagnosis of thyroid nodules].

The study was conducted an 298 patients suffering from thyroid nodules who underwent FNAB and successively surgery. The results of FNAB, compared with histology showed a sensibility of the technique of 76.6%, a specificity of 100%, an efficiency of 97.3%, with a negative prognostic value of 97% and a positive one of 100%. The authors stress the role of FNAB in the diagnosis of thyroid nodules in permitting, apart from where follicular tumours are concerned, an efficient identification of malignancy.

[Asbestos fiber count in extra-pulmonary tissues].

Most autoptic tissues of three men (two with pulmonary asbestosis and one without any exposure to asbestos) were analyzed by transmission electron microscopy and energy dispersion spectrometry for the quantitative and qualitative evaluation of asbestos fibers. Preliminary results indicate that only in cases with occupational exposure were found asbestos fibers (in lung, pleura , bladder, kidney and liver). These results allow interesting speculations on some environmental pathogenetic questions and deserve further investigations.

[Bassini's technique versus Trabucco's technique in the treatment of inguinal hernia: randomized comparative study].

The authors have carried out a perspective randomized trial between two homogeneous groups, each one of 30 pts., consisting of 50 males and 10 females with an average age of 42 (18-65) affected by an uncomplicated inguinal hernia. Group A underwent a Bassini repair technique and group B underwent a Trabucco's operation. The early results in terms of clinical conditions and days of post-operative course and period of return to work were analysed with Shapiro-Wilk and t tests. The group B patients had a significant reduction of the post-operative period (2.3 days vs 3.46), a significant reduction of post-operative pain (present in all group A patients vs 6.6%) with rapid return to normal functions and to work. All these conditions reduce the costs of the operation for society and the health service. There were no statistically significant differences in terms of morbidity and long-term follow-up (about 9 months).

Monoethylglycinexylidide (MEGX) test in patients with different liver diseases.

We have studied the levels of the MEGX test in a heterogeneous group of 50 patients with chronic liver disease and with hepatic tumours and we have compared it with the routine LFTS commonly used to assess liver function and with the Child-Pugh Classification system. Our results demonstrate a statistically significant relationship between MEGX levels and prothrombin levels, and between MEGX and alkaline phosphatase and a highly significant relationship between MEGX and cholinesterase. In the group of patients with cirrhosis we found a statistically significant difference amongst the MEGX levels in the 3 classes of the Child Classification system. The MEGX test is a good index in evaluating hepatic function and it is also quick and easy to perform and capable of determining residual liver function. The test can also be used for preoperative assessment in patients with focal hepatic lesions, especially in those with a previous history of cirrhosis, and in patients with functional hepatic disease.

[A facilitated technique for donor hepatectomy: experimental study in the pig].

A facilitated technique of porcine donor hepatectomy is reported underlying the possibility to make an easy procedure without in situ portal perfusion, so other abdominal organs supplied by superior mesenteric system can be harvested at the same time. The viability of those grafts procured without in situ portal perfusion has been compared with an other group of livers harvested with in situ aortic and portal flushing. The evaluation of the histology and early graft function two hours after riperfusion has shown no differences between both groups.
