A 70-year-old man presented with large areas of brawny edema of the skin and neck and weakness of the muscles of the forearm. Biopsy revealed scleredema adultorum with inflammatory changes in the forearm muscle. An electromyogram showed a mixed pattern compatible with myositis.
An elderly patient presenting with severe tetany secondary to hypocalcaemia causing bilateral fractures of the femoral necks, was proved to have coeliac disease. Severe muscle pains, an organic mental syndrome and personality changes are rare complications of coeliac disease and are reversible with treatment.

Cholestatic jaundice associated with chills, pruritus and blood eosinophilia developed in a patient who received prajmalium bitartrate therapy for ventricular arrhythmia following acute myocardial infarction. Discontinuation of the drug resulted in a spontaneous improvement in the clinical and biochemical findings. Challenge by prajmalium bitartrate caused rapid reappearance of the clinical and biochemical features. In immunological studies, deposits of IgG and IgA were detected at the bile canaliculi by fluorescent staining, and the patient's lymphocytes produced macrophage migration inhibition after in vitro incubation with prajmalium bitartrate. Thus, laboratory results support the assumption of an allergic mechanism.
A case of renal carcinoma with hepatic dysfunction and hypercalcemia is described. The literature on Stauffer's syndrome has been reviewed and the pathogenesis of this syndrome discussed. It is stressed that the presence of hepatic dysfunction should not be considered a contraindication to surgery since its manifestations have been observed to regress in many cases following removal of the tumor.

Splenomegaly is an unusual finding in Crohn's disease and can be persistent or transient. In three febrile patients it was so prominent that the diagnosis of lymphoma was considered. In two additional patients the splenomegaly was an incidental finding. It is probably a nonspecific response to the basic disease in most cases, or occasionally a response to the chronic liver disease that can occur in inflammatory bowel disease.

A patient with metastatic insulinoma who suffered from severe hypoglycaemic attacks was treated with 7-5 g of streptozotocin (Upjohn). Shortly after the start of treatment, the hypoglycaemic attacks ceased; fasting blood glucose levels and serum immunoreactive insulin values returned to normal upon completion of the treatment. Two years after treatment, the patient is well and active and has not had any attacks. The oral glucose tolerance test and tolbutamide stimulation test now show serum immunoreactive insulin excretion patterns similar to those seen in diabetics.
