OBJECTIVES: French guidelines recommend screening all patients for virus infection prior to cryopreservation of their semen. In case of viral risk, the use of specific high secure CBS straws is recommended. The objective of this work was to evaluate the microbiological risk by testing all semen samples before cryopreservation. PATIENTS AND METHODS: Fifty one patients underwent a semen culture before cryopreservation. RESULTS: The fifty one patients were classed into 3 groups following semen culture results: group I: negative culture (39/51, 76.47%), group II: positive culture with microbiological contamination (7/51, 13.73%) and group III: positive culture with pathogen (5/51, 9.8%). For 3 patients of the latter group, we tested a three-layer density gradient to eliminate bacteria before Assisted Reproductive Techniques. DISCUSSION AND CONCLUSIONS: This paper discusses the risks related to microbiological contamination and the options available in this case.

The authors present a retrospective and longitudinal study of the predictive factors of mortality in patients having an implanted automatic defibrillator. The population comprised 127 patients implanted between September 1988 and September 1997. There were 107 men with a mean age of 57.7 +/- 13 years. The left ventricular ejection fraction was 39.3%. The proportion of coronary patients was 68%; 20% of patients had atrial fibrillation and 5% were in Class III of the NYHA classification. The indications were: resuscitated cardiac arrest (N = 56) and poorly tolerated ventricular tachycardia (N = 71). The follow-up period was 30 +/- 25 months. There were 23 early and 10 late complications. Seventy-two patients had received an electric shock; 57 had an appropriate shock. There were 23 arrhythmic storms (ventricular arrhythmia requiring at least 2 shocks in less than 24 hours) in 17 patients. The operative mortality was 1.1%; at 1 year, the global survival was 93.9 +/- 2.2%; cardiac survival was 94.7 +/- 2.1%; survival without sudden death was 98.3 +/- 1.2%. Multivariate analysis isolated predictive factors for mortality; atrial fibrillation was predictive for global mortality; an ejection fraction < 30% and the fact of having received an appropriate shock were predictive of cardiac mortality; and an arrhythmic storm was predictive of sudden death.
Medical records of 81 older patients (65 years of age and over) who underwent electroconvulsive therapy (ECT) at a university-affiliated private geriatric hospital were reviewed to evaluate the safety and efficacy of this treatment for depression in the "young-old" (65 to 80 years) compared with the "old-old" age group (over 80 years), a group that has not yet been adequately studied. Information was obtained regarding demographics, medical and psychiatric diagnosis, medications, indications for ECT, number and laterality of treatments, outcome, and complications. Thirty-nine patients 80+ years of age (mean age, 85 +/- 3.2) were compared with 42 patients 65 to 80 years of age (mean age, 74 +/- 5.2). Statistical analysis was performed using confidence intervals of the difference in proportions of patients in each group. There were no significant differences in the demographics, number and laterality of ECT treatments, indications for ECT treatment, medical diagnosis, medications, or prior history of falls, but psychiatric diagnoses differed slightly. Patients over 80 years had significantly more cardiovascular complications and falls (95% confidence interval) and tended to have a worse ASA (American Society of Anesthesiologists) scale rating and a somewhat less successful outcome. This study confirms the role of ECT as a relatively safe and effective treatment, which may be lifesaving in selected depressed older patients. Prospective studies are needed to understand better the long-term outcome and to prevent the morbidity and mortality associated with ECT in this frail, high-risk older group.
All dirty surgical instruments received into the unit are processed through an Automatic Surgical Instrument Cleaning and Solvent Drying System. This system had been in continuous use since its installation in August 1983 until May 1986, without any significant production problem arising. During May 1986, and for a prolonged period afterwards, persistent separation of the surfactants contained in the Arklone W occurred. This separation manifested itself as a thick Cheeselike surface scum on the cooled solvent. At the onset the only recourse available in order to maintain the plants productive capacity was to replace the contaminated solvent with fresh following each occurrence. This constant replenishment obviously proved to be an extremely time consuming exercise. No additional material cost was involved as ICI readily supplied all the solvent requirement completely free of charge throughout the whole period.

An epidemiological survey of hepatitis B virus (HBV) infection has been carried out from 1974 up to 1978 in four laboratories, with the aim of identifying high-risk groups in the population. Radioimmunoassay procedures were used in order to detect the hepatitis B surface antigen (HBsAg) and the corresponding antibody (anti-HBs) in serum samples from over 100,000 individuals of various geographical origin (mainly from Ligury or from other areas in Northern Italy). The groups of population under monitoring included: individuals without hepatic illness just after admittance into hospitals (these groups were found to be adequately representative of the corresponding open population), groups of children and boys from the open school population, individuals living in various communities (foundling hospital, children college, recruits, institutionalized old people, subnormal individuals and their assistance staff), non-assistance working categories (workers from metallurgical and chemical industries, shipped seamen), hospital assistance personnel, dialyzed and transplanted renal patients, blood donors. The results obtained showed a considerable variability of HBV infections among the examined groups of population. In particular, the frequency of HBsAg and/or anti-HBs detection appeared to be significantly affected by each of the following epidemiological parameters: geographical area, sex, age, pregnancy, life in communities and contacts with hepatitis patients, mental deficiency, non-assistance working activities, assistance activity in the hospital environment, hemodialysis and transplants, selection of population groups by HBsAg screening.
A to O bone marrow transplantation was performed in a 25-year-old male affected with severe aplastic anaemia, the donor being an HLA compatible brother. Three plasma exchanges had to be performed with an Aminco separator to remove the original and recurring anti-A isohaemagglutinins. The dynamics of O to A blood group conversion were followed by means of differential agglutination. An early wave of marked dyserythropoiesis was observed in the engrafted marrow. Mild to moderate GvHD was treated successfully with MTX, bolus high dosage 6-methylprednisolone and, at relapse, with intravenous ALG.

The biosynthesis of glucagon was studied by using the recirculated, isolated perfused rat pancreas. [(3)H]Tryptophan was initially incorporated into acid-ethanol-extractable protein, which on gel filtration was eluted with a molecular weight of about 9000 and contained a small amount of glucagon immunoreactivity. With longer incubation [(3)H]tryptophan incorporation into a second peak was obtained in an identical position with that of the majority of rat glucagon immunoreactivity. This peak of labelled protein exhibited migration characteristics on polyacrylamide-gel electrophoresis identical with those of rat glucagon and was identified as newly synthesized glucagon by demonstration of specific binding and dissociation behaviour with glucagon antibodies. The incorporation of [(3)H]tryptophan into acid-ethanol-extractable protein was inhibited by cycloheximide. High concentrations of glucose increased [(3)H]tryptophan incorporation into high-molecular-weight protein but decreased incorporation into proteins smaller than cytochrome c. The pattern of [(3)H]leucine incorporation into protein was similar to that of [(3)H]tryptophan.
