Influence of sodium on experimental renovascular hypertension in rats.

1. Rats on normal sodium diet (group 1) and on chronically maintained low sodium diet (group 2) were studied during a control period, after clipping the renal artery (two-kidney, one-clip hypertension) and after nephrectomy (one-kidney, one-clip hypertension). 2. The low sodium diet neither prevented the development nor changed the severity of two-kidney, one-clip hypertension, and the latter was not accompanied by an increase in plasma renin activity. 3. After nephrectomy arterial pressure further increased and plasma renin activity decreased in group 1, and both remained unchanged in group 2. 4. Blood volume was the same in both groups 10 days before and 10 days after nephrectomy. 5. Sodium does not seem to be 'necessary' in the two-kidney, one-clip hypertension although it may play an enhancing role in the one-kidney model.

[Analysis of deoxyribonuclease I and II activity as a function of ionic strength and pH].

The action of deoxyribonucleases I and II has been studied as a function of ionic strength and pH, in the light of the theory of the ionic control of biochemical reactions (P. Douzou and P. Maurel (1977) Proc. Nat. Acad. Sci. USA, 74, 1013-1015). The pattern of DNA degradation by the two enzymes fits the general principles of the theory. However, the activity of DNAase II, a dimeric, basic protein (pI = 10,2) appears to be scarcely modulated by variables such as ionic strength and pH. This is reminiscent of what was elsewhere observed with the system double stranded RNA-seminal RNAase (also a very basic, dimeric enzyme), and could, therefore, tentatively be correlated with the dimeric and/or the very basic nature of the enzyme protein.

'Isolated' pulmonary valve stenosis as part of more widespread cardiovascular disease.

In 25 patients aged 6 days to 9 years presenting as 'isolated' pulmonary valve stenosis, histology of the myocardium of right and left ventricles, coronary arteries, and ascending aorta has shown abnormality in one or all these areas. Myocardial necrosis, old and recent, unrelated to coronary occlusion was frequent. Myocardial 'dysplasia' involving both ventricles, and resembling hypertrophic cardiomyopathy (HOCM, ASH) was found in 10 and a relation of this to myocardial injury in the fetus is postulated. Varying degrees of coronary occlusion were frequently seen in both right and left coronary arteries. The histology of the ascending aorta was abnormal showing 'higgledy-piggledy' disorder of smooth muscle components in 12 (48%). In a different series of 53 patients who had pulmonary valvotomy for apparent 'isolated' pulmonary valve stenosis there were 14 with clinical evidence of left ventricular abnormality consistent with the pathological changes described, 2 with the same aortic histological changes, and 2 with macroscopical left ventricular hypertrophy. Two of them developed classical hypertrophic cardiomyopathy years later. It is suggested that when pulmonary valve stenosis presents with a thick tricuspid poorly mobile valve, particularly in infants or in patients with evidence of other congenital stigmata, it may be part of a more widespread cardiovascular abnormality. This should be recognized and considered in the evaluation of surgical patients and late survivors who may show unexpected clinical features.

Accessory thyroid in the anterior mediastinum: case report.

A case of accessory thyroid in the anterior mediastinum, physically separated from the thyroid gland, is reported. The mediastinal thyroid was incidentally discovered during the preoperative evaluation of a patient with breast carcinoma. The extreme rarity of the case is outlined.

Bilateral tonsillar metastasis of gastric adenocarcinoma.

The occurrence of metastatic tumors in the tonsils, although rare, must be considered in the differential diagnosis of tonsillar lesions. In particular, the review of the literature reports only 7 cases of tonsillar metastasis of gastric carcinoma. The authors describe a case of adenocarcinoma involving bilaterally the palatine tonsils and the lingual tonsil, in a patient previously operated due to gastric adenocarcinoma. The clinical appearance of the lesion could be also consistent with lymphoproliferative or infectious diseases. The clinical and pathologic differential diagnosis of this peculiar case is discussed, together with a brief review of the literature.
