Transient mitral regurgitation: an adjunctive sign of myocardial ischemia during dipyridamole-thallium imaging.

A patient developed transient exacerbation of a mitral insufficiency murmur and a reversible posterior wall perfusion defect during dipyridamole-thallium imaging. Coronary angiography showed significant stenoses of both the right and the circumflex coronary arteries that supply the posterior papillary muscle. Cardiac auscultation for transient mitral incompetence, a sign of reversible papillary muscle dysfunction, is a simple and practical adjunctive test for myocardial ischemia during dipyridamole-thallium imaging. It may confirm that an isolated reversible posterior wall myocardial perfusion defect is truly ischemic in nature as opposed to an artifact resulting from attenuation by the diaphragm.

Apical hypertrophic cardiomyopathy with spontaneous post partum coronary artery dissection.

Primary coronary dissection is usually a catastrophic event with a fatal outcome; less than 20 cases of long term survival have been reported. A 32-year-old woman with apical hypertrophic cardiomyopathy who survived a spontaneous dissection of the left anterior descending coronary artery and acute anterior wall infarction two days postpartum is described. To the authors' knowledge, the simultaneous occurrence of these two rare conditions has never been previously reported. Thallium-dipyridamole imaging was performed to estimate the amount of residual viable myocardium in the infarcted area and to evaluate the hemodynamic significance of the residual luminal narrowing. Myocardial perfusion imaging can be useful to decide on surgical versus conservative treatment of these patients.

Satanism in a psychiatric adolescent population.

In a university affiliated adolescent psychiatric facility, providing approximately 250 consultations per year, an unsuspectedly high prevalence of preoccupation with "satanism" was found in referred adolescents. Interested by the phenomenon, the authors have identified and documented eight cases in an attempt to isolate common characteristics among the cases. Initially a link between the marginal cult belief and general maladjustment was hypothesized, specifically delinquent behaviour. The study confirmed this trend and showed a significant impairment in the social adjustment of these adolescents. One of the most striking findings was the high prevalence of family disruption and parental abuse. Furthermore, a wide range of psychiatric symptoms were found in our subjects. This study raises concerns over the psychological development of adolescents who are subject to high levels of psychosocial stress. It will hopefully encourage further work in the area of increased susceptibility towards beliefs and indoctrination.

14q+ abnormality with probable t(8;14)(q24;q32) in a young Haitian immigrant with acquired immunodeficiency syndrome and concomitant Burkitt's-like lymphoma.

Cytogenetic analyses were carried out in a young Haitian immigrant with acquired immunodeficiency syndrome and concomitant Burkitt's-like lymphoma associated with massive bone marrow infiltration. A characteristic 14q+ abnormality was found in all bone marrow cells examined. Although chromosome abnormality involving band 8q24 was not evident in all the cells examined, some karyotypes show that the typical t(8;14)(q24;q32) is most probably present. No other complex rearrangements could be identified. This is the first report of concomitant acquired immunodeficiency syndrome and Burkitt's-like lymphoma in the Haitian community. Our cytogenetic findings provide further evidence for the role of specific chromosomal rearrangements in Burkitt's-like lymphoma oncogenesis in the setting of acquired immunodeficiency syndrome.

A therapeutic and discontinuation study of clobazam and diazepam in anxiety neurosis.

The efficacy and safety of clobazam and diazepam were compared in 40 patients with generalized anxiety disorder in a 28 day non-placebo-controlled trial followed by a 3-day single-blind placebo washout. The two drugs were equally anxiolytic, and the anxiety symptoms of most patients were brought under control within 1 week. Both drugs were found to be nontoxic. Diazepam had slightly greater sedative properties than clobazam, which was the patients' preferred drug after 28 days of treatment. During placebo washout, anxiety returned to pretreatment levels. Abrupt withdrawal did not result in rebound phenomena.

Determination of the optimal concentration of lithium for the prophylaxis of manic-depressive disorder.

A group of 29 bipolar manic-depressives completed a 12-month double-blind cross-over trial of low-dose and high-dose lithium prophylaxis. Twelve patients relapsed, and significantly more of the relapses occurred during the low-dose 6-month phase of the trial. There was a trend for relapse to occur within 2 months of an abrupt drop in plasma lithium level, and to occur more often in women than in men. The efficacy of low-dose lithium prophylaxis and the significance of rebound relapse are discussed.

Isolation of an influenza A virus from seals.

Influenza A virus of serotype Hav1 Neq1 (H7N7 by the 1980 revised influenza typing system proposed by WHO experts) was repeatedly isolated from lung and brain tissues taken from harbor seals (Phoca vitulina) found suffering from pneumonia on Cape Cod Peninsula (U.S.A.) in the winter of 1979-1980. The seal isolates, although of a serotype identical to some fowl plaque virus strains, were harmless to chickens and turkeys in transmission experiments. An earlier human infection by a Hav1 Neq1 influenza virus and the serologic relatedness of this avian serotype with the equine 1 serotype are cited in support of the view that influenza viruses with these antigenic characteristics seem to have a facility to pass from birds to mammals.

Case report: An outbreak of herpesvirus myeloencephalitis in vaccinated horses.

In the foaling season of 1977, five vaccinated horses in a Standardbred breeding stable were affected with herpesvirus myeloencephalitis. Respiratory and abortigenic forms also occurred in other individuals on the premises. Equine herpesvirus type 1 was isolated from the brain of one case of myeloencephalitis and from lungs of two aborted fetuses. Twelve of 16 horses demonstrated fourfold or greater increases in titres to equine herpesvirus type 1.

Isolation of Ureaplasma from bovine granular vulvitis.

Cultures for mycoplasmatales, viruses and bacteria were made from bovine vulvar swabs to determine whether ureaplasma was associated with a clinical granular vulvitis observed in 16 Ontario dairy herds. Ureaplasma was isolated from 23.5% of 34 clinically normal cows, 74% of 27 cows with mild to moderate vulvar hyperemia but no discharge and 100% of 20 cows with acute vulvar hyperemia accompanied by purulent discharge. There were statistically significant differences in rates of isolation among clinical groups. Mycoplasma bovigenitalium was isolated from 7.7% and 20% of cows with moderate or acute vulvitis respectively but not from normal cows. Haemophilus somnus was isolated from 25% of cows with acute vulvitis. There were no significant differences in isolations of Escherichia coli, Corynebacterium pyogenes and alpha-hemolytic streptococcus between normal and clinically affected animals. Cultures of 135 repeat samples from 33 cows revealed that ureaplasma persisted in some animals for at least three months. No viruses were isolated from any of the animals in this study.

An outbreak of infectious laryngotracheitis in commercial poultry flocks in Ontario.

An outbreak of infectious laryngotracheitis was observed in Ontario commercial poultry flocks from September 1974 to June 1975. Fifty-five flocks, totalling over 848,770 birds were clinically affected by this disease in the southern region of the province. Sixty-nine percent (38/55) of the exposed flocks were in Wentworth country and the regional municipality of Niagara. Hemorrhagic tracheitis with histological evidence of intranuclear inclusion bodies in syncytia of tracheal epithelial cells were the salient pathological findings.
