Goodpasture's syndrome complicated by nocardia asteroides infection.

A case of Goodpasture's syndrome--an immunological disorder characterised by a proliferative glomerunlonephritis with haemorrhagic pulmonary alveolitis--is described. There has been complete functional recovery of the presenting renal failure, but the immunosuppressive therapy was complicated by a severe episode of metastasising nocardiosis.

Modular single-needle haemodialysis; ultrafiltration characteristics.

In vitro and in vivo work has been carried out examining a modular single-needle system for fistula haemodialysis. Ultrafiltration characteristics of the single-needle system are defined and conclusions upon flow patterns are reached. Applying the mean blood pressure formula to the extracorporeal system pressures, the transmembrane pressure can be calculated and the ultrafiltration rate forecast. High transmembrane pressures can be modified, with sacrifice of blood flow, by amendment of the blood flow phase durations.

Desamino arginine vasopressin (DDAVP) as a diagnostic agent.

The use of 1-desamino-8-arginine vasopressin (DDAVP) was examined as a diagnostic agent, within the context of the comprehensive one day renal function test. Thirty-three normal young subjects were examined in an 8 1/2 hour day. It was found that a 20 microgram intranasal dose of DDAVP consistently resulted in the production of a urinary osmolality to 800mosmol/kg or more, with no untoward effect. DDAVP can advantageously replace pitressin tannate in oil as a diagnostic agent.

Renal biopsy: a review from experience.

A review of the use of percutaneous renal biopsy is presented, based on 12 years of personal experience. The place of biopsy today, and its role in diagnostic and investigative nephrology is discussed.

Normal radiological renal size: simple estimation from single urogram series.

A normal range of kidney size can be defined from a single, technically adequate, excretory urogram series. Relation of kidney length to distance between L1 to L3 bodies inclusive gives a better indication of normal size, than comparison between kidney length and a single vertebra. The span of the normal range of kidney length to lumbar spine ratio is such that its application to a given radiological examination will be of marginal value.

Gastric acid secretion in chronic renal failure.

Contrary to the small amount of published evidence, but in accordance with clinical impression, we have found an increased incidence of peptic ulceration in people with chronic renal failure. Hyperacidic secretion in response to a standard pentagastrin test occurs in patients established on long-term dialysis treatment. The traditional liability of azotaemic patients to peptic ulceration seems not to be decreased by adequate long-term dialysis and indeed may be worsened.
