Comparative study of calcium-channel blockers on cell proliferation, DNA and collagen syntheses, and EGF receptors of cultured gingival fibroblasts derived from human nifedipine, nicardipine and nisoldipine responders.

Our previous study indicated that fibroblasts derived from patients reactive to nifedipine might be susceptible to the other calcium-channel blockers (nicardipine, verapamil and diltiazem) in terms of cell proliferation, DNA synthesis, and collagen synthesis. Thus, the present investigation was designed to clarify the cross-reactivity among dihydropyridine calcium-channel blockers (nifedipine, nicardipine, and nisoldipine). Human gingival fibroblasts derived from seven, two, and one patients who developed gingival overgrowth as a result of nifedipine, nicardipine, and nisoldipine medications, respectively, were examined in terms of the effect of calcium-channel blockers (nifedipine, diltiazem, verapamil, and nicardipine) on cell proliferation, DNA synthesis, collagen synthesis, and the number of epidermal growth factor (EGF) receptors. Phenytoin was used as a positive control. With most of the calcium-channel blockers and phenytoin, fibroblasts from patients reactive to nifedipine and nicardipine medications gave a better cell proliferation rate, DNA synthesis, and an increased number of EGF receptors, compared to non-drug-treated control. However, this was not the case for calcium-channel blockers tested in fibroblasts from patients reactive to nisoldipine medication.

[Application and improvement of aflatoxin analysis in foods using a multifunctional column and HPLC].

In an earlier report, we developed a rapid, sensitive and clean method consisting of non-chloroform extraction, clean-up on a commercial multifunctional cartridge column and HPLC with fluorescence detection for the analyses of aflatoxins. In this report, we applied this method to analyze aflatoxins in nuts, giant corn, cereals, spice and black teas. The method was effective for macadamia nuts, walnuts, hazelnuts, brazil nuts, giant corn, rice, wheat and buckwheat, and the recoveries of aflatoxins B1, B2, G1 and G2 spiked in them at the level of 10 ng/g were 85-106%. However, in the chromatograms of spices and black tea, many background peaks were observed. Therefore, we added a purification step with an affinity column to the clean-up of these samples with the multifunctional cartridge column. After the additional purification, most of the background peaks were gone. The recoveries of aflatoxins B1, B2 and G1 spiked at the level of 10 ng/g were 71-112% except for the case of B2 in white pepper (48%). The recoveries of G2 were 49-95%.

[Anti-neutrophil cytoplasmic antibody--enzyme immunosorbent assay].

Enzyme immunosorbent assay(ELISA) is a very useful method to determine anti-neutrophil cytoplasmic antibody(ANCA), which is an important serological marker for pauci-immune type systemic vasculitis and necrotizing glomerulonephritis. A test was made using new myeloperoxidase(MPO)-ANCA ELISA(A), employing native MPO purified from the neutrophils in sputum as a liquid phase antigen. Furthermore, the ELISA(B) using MPO, composed of one large and one small subunit, was tested as solid phase antigen. The intra-assay and inter-assay CV of the new ELISA(A) were 3.92 to 6.75% and 5.0 to 8.1%, respectively. Close ANCA titer correlation was shown between the new MPO-ANCA ELISA(A) and the conventional ELISA, using native MPO from peripheral neutrophils as solid phase antigen. ELISA(B) showed low MPO-ANCA detection sensitivity compared to ELISA(A) and to conventional ELISA. ELISA using native MPO from neutrophils in sputum as liquid phase antigen is useful for MPO-ANCA detection. There might be an ANCA which recognizes only native form MPO.

Effects of tandospirone, a serotonin-1A agonist, on the hypothalamo-pituitary-gonadal axis of male patients.

BACKGROUND: Serotonin (5-HT) agonists are reported to affect prolactin (PRL) and gonadotropin secretion. A small study was conducted on male patients with generalized anxiety disorders to investigate the clinical and neuroendocrinological effects of the 5-HT(1A) receptor agonist tandospirone (TDS) on the hypothalamo-pituitary-gonadal (HPG) axis. METHODS: The subjects for the present study included 11 male outpatients. Informed consent was obtained from all subjects involved in this study. The endocrine studies were done before and during TDS administration. Psychiatric ratings were done using the Japanese version of the Spielberger's State-Trait Anxiety Inventory. RESULTS: We found that both state- and trait-anxiety scores were significantly reduced by TDS treatment; there was no significant difference in PRL, gonadotropins or testosterone (T) between the patients and normal controls; TDS administration showed significant stimulatory effects on PRL and T; and PRL change between baseline and TDS steady state (DeltaPRL) did not show a significant correlation with improvement in state or trait-anxiety scores. CONCLUSIONS: Our results indicate that PRL response may not provide a clinical predictor; (2) PRL level may not reflect the level of anxiety, and 5-HT(1A) may have stimulatory effects on PRL. However, further, double-blind evaluation with a larger sample would be needed for clarification of effects of 5-HT(1A) on the HPG axis.

[Hepatic intra-arterial infusion chemotherapy using polymer-coated 2.9 Fr implantable catheter-port system--initial experience].

We developed a new 2.9 Fr implantable catheter-port system (reservoir) to perform arterial infusion chemotherapy in patients with unresectable liver tumors. This study was undertaken to evaluate the feasibility of placing this new reservoir system in patients in whom placement of a 5 Fr-reservoir system would seem to be difficult because of severe stricture, tortuosity or angulation of the hepatic artery. A new reservoir system was successfully implanted in 25 patients during 27 sessions. After a side hole was opened, a 2.9 Fr catheter was inserted in the distal hepatic artery or in the gastroduodenal artery in 24 sessions. The catheter tip was then fixed with coils to prevent catheter dislocation. In the other 3 sessions, a 2.9 Fr catheter was inserted without catheter fixation in the replaced right hepatic artery and a small sized collateral artery towards the liver. Arterial infusion chemotherapy was done without any trouble after 20 sessions. Catheter dislocation was found after 2 sessions in which the catheter tip was not fixed or inadequately fixed. Early arterial occlusion was found after placing a 2.9 Fr catheter in the replaced hepatic artery and the small sized-collateral artery after 4 sessions. In conclusion, although sequellae should be evaluated over a long-term period, implantation of a new 2.9 Fr reservoir system is technically feasible and useful in performing arterial infusion chemotherapy.

[A case of IgA nephropathy associated with silicosis].

A 51-year-old male who had been working as a building wrecker for 20 years, was admitted to our hospital in June 1999 for proteinuria and hematuria examination. He started this work in 1978. Twelve years later, severe coughing and bloody sputum began and he was diagnosed as having silicosis in 1995. Urinalysis on admission showed proteinuria(294 mg/day), microhematuria(20-30/hpf), RBC cast and granular cast. High serum IgA(770 mg/dl) and high serum interleukin-6(IL-6) (3,280 pg/dl) were found. A renal biopsy showed mild mesangial matrix expansion and mesangial cell proliferation with IgA deposition, which was diagnosed as IgA nephropathy. Chest X-rays showed multiple small nodular lesions on both lung fields indicating silicosis. In Nov. 1999, he resigned from his job as a building wrecker because of increasing coughing and bloody sputum associated with body weight loss. Within 3 months after stopping this work, coughing and bloody sputum disappeared and the abnormal urinalysis findings returned to normal. Serum IgA and serum IL-6 data improved to 462 mg/dl and 2.5 pg/dl, respectively. It is suggested that silicon exposure might be related to the pathogenesis of IgA nephropathy in this patient.

An efficient synthesis of dinitrile derivatives by the reaction of oxime esters or acid anhydrides with cyanotrimethylsilane catalyzed by La(O(i)()Pr)(3)

The reaction of oxime esters with cyanotrimethylsilane (Me(3)SiCN) under the influence of a catalytic amount of lanthanide compounds produced alpha-trimethylsilyloxydinitrile derivatives in excellent yields accompanied with the formation of trimethylsilyl oxime ethers. Among the lanthanoid catalysts examined, La(O(i)()Pr)(3) was found to be the best catalyst. The reaction seems to proceed through the formation of acyl cyanides as intermediates, followed by the addition of Me(3)SiCN to them. Additionally, the reaction of acetic anhydride with Me(3)SiCN catalyzed by La(O(i)()Pr)(3) gave 1-trimethylsilyloxyethane dinitrile. Thus, various alpha-trimethylsilyloxydinitriles were synthesized in good yields by allowing oxime esters or acid anhydrides to react with Me(3)SiCN in the presence of a catalytic amount of La(O(i)()Pr)(3).

Eosinophil counts and plasma fibrinogen in patients with vasospastic angina pectoris.

Epidemiologic studies have suggested a relation between white blood cell (WBC) counts and the incidence of coronary heart disease. However, the relation between vasospastic angina pectoris (VAP) and WBC counts remains to be elucidated. To clarify the relation between differential and WBC counts in VAP, we compared the hematologic values, blood chemical values, plasma fibrinogen levels, C-reactive protein levels, and coronary risk factors in patients with spontaneous attacks of VAP (n = 39) with those in patients with stable effort angina pectoris (EAP, n = 35) and in control subjects (n = 19). Patients with VAP were further divided into mild VAP (n = 22) and severe VAP groups (n = 17). There were no differences in the coronary risk factors, body temperature, total WBC counts, and C-reactive protein levels among the control, EAP, mild VAP, and severe VAP groups, except that the high-density lipoprotein cholesterol in the EAP group was significantly lower than that in the control group (p <0.01). In contrast, the eosinophil counts were significantly higher in the severe VAP group than in the other 3 groups (p <0.01). Plasma fibrinogen levels were also significantly higher in the severe VAP group than in the other 3 groups (p <0.05). The follow-up study for differential and WBC counts in patients with VAP (n = 23) demonstrated that, after medical therapy, the eosinophil counts were significantly decreased to the some level as those in the control group (p <0.0001). Thus, the eosinophil counts and plasma fibrinogen levels could predict the severity of VAP. Furthermore, a follow-up study in patients with VAP suggests that coronary vasospasm could result in an increase in eosinophil counts.

Pathologic evaluation of surgical margins and local recurrences after breast-conserving surgery without irradiation.

This study was undertaken to evaluate the status of margins of the excised breast tissue using our own method. We also determined the indications for breast-conserving surgery without irradiation by examining the characteristics of patients with local recurrence and comparing relapse-free survival (RFS) and overall survival (OS) of patients who underwent wide excision without irradiation with those of 267 patients who underwent total mastectomy. Eighty-two patients with a 3 cm diameter or less invasive carcinoma were treated with wide excision and axillary dissection between 1987 and 1996. Patients who histologically had four or more axillary lymph node metastases, positive pathologic margins, or a high degree of in situ ductal carcinoma around the main tumor in consecutive step-sections were excluded from this study. During a median follow-up of 6 years (range 2-11 years), six patients (7.3%) had local recurrence and five (6.0%) had regional or distant recurrences as their site of first failure. At 11 years the life-table values for RFS and OS for the wide excision-treated group were 84.7% and 92.1%, respectively, compared with 85.0% and 90.0%, respectively, for patients treated by total mastectomy. RFS and OS were similar in the two treatment groups. Results in the present study indicate that if the patients treated by breast-conserving surgery are carefully selected and there are no foci in the pathologic margins, there is a low degree of in situ ductal carcinoma around the tumor, and no multicentricity, it might be unnecessary to administer

Different expression of calreticulin and immunoglobulin binding protein in Alzheimer's disease brain.

Both calreticulin (CRT) and immunoglobulin binding protein (Bip) have a role in the folding and assembly of oligomeric membrane proteins in the endoplasmic reticulum (ER). Recent studies have demonstrated the generation of beta-amyloid protein (Abeta) 1-42, a key peptide for amyloid deposits, in the ER. We, therefore, examined the localization and expression of CRT, Bip and their mRNA by immunohistochemistry, Western blot, in situ hybridization and semiquantitative reverse transcription polymerase chain reaction (RT-PCR) in both neurologically normal and Alzheimer's disease (AD) brains. Two polyclonal anti-CRT antibodies gave similar positive staining of CRT in neurons and glia. In neuronal cells, the cytoplasm, nucleoli and their processes were positive for CRT. In glial cells, perinuclear staining was frequently seen and the processes of some glial cells were also stained. In AD, these antibodies stained clearly damaged neurons but the number and the intensity of positive cells were decreased compared to controls. Processes of microglial cells were markedly positive in the AD white matter. Western blots using an anti-CRT antibody showed significantly lower immunoreactive bands in AD than control brains. By in situ hybridization, the number of neurons which express the CRT mRNA was less in AD than in controls. Using RT-PCR, the relative levels of the CRT mRNA in AD brains were also found to be significantly lower than those in controls. On the other hand, the number of Bip-positive cell, the production of Bip and the expression of mRNA for Bip did not differ between control and AD brains. These results suggest that CRT may be a multifunctional protein in human brain, and that the weak expression of CRT and the positive staining of microglial processes in AD brain may be part of the pathological processes in AD.

Effects of chronic neuroleptic administration on the hypothalamo-pituitary-gonadal axis of male schizophrenics.

1. The authors investigated the effects of long-term neuroleptic administration on the hypothalamo-pituitary-gonadal (HPG) axis of regularly medicated schizophrenic male patients. 2. The subjects were 56 patients who were diagnosed according to the DSM-IV criteria for schizophrenia. Each patient gave informed consent for the research involved in this study. Based on the dosages of neuroleptics, the subjects were classified into two age-matched groups: those with lower dosages (LD) and those with higher dosages (HD). 3. (1) The mean level of the blood testosterone (T) in the LD group was significantly lower than that in the normal group, although the level was within normal range. There was no significant difference in the mean level of the blood T for the HD and the normal groups, or for the HD and the LD groups. (2) There was no significant difference in the mean levels of the blood luteinizing hormone between all groups. (3) Also, there was no significant difference in the mean levels of the blood follicle-stimulating hormone between the groups. (4) The mean levels of the blood prolactin in both the LD and HD groups were significantly higher than that in the normal subjects, but there was no significant difference between the LD and HD schizophrenics. 4. Overall, these results seem to indicate that (i) in male patients with chronic schizophrenia, HPG function is impaired, and (ii) chronic neuroleptic medication stimulates the HPG axis in schizophrenic patients with impaired HPG function.

Subchronic toxicity of chlorpropham (CIPC) in ICR mice.

Male and female ICR mice were given 0, 1875, 7500 or 30,000 ppm of chlorpropham (CIPC) in the diet for 13 weeks. Methemoglobin levels of male and female mice in the 7500 and 30,000 ppm groups were significantly elevated. Hemoglobin concentration, packed cell volume, mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration and white blood cell count of male and female mice in the 30,000 ppm group were significantly increased. Dose-dependent splenomegaly was observed in male and female mice in the 7500 and 30,000 ppm group. Congestion, increased hemosiderin deposition and increased extramedullary hematopoiesis in the spleen, hematopoietic cell hyperplasia and hemosiderin deposition in bone marrow was observed dose dependently in male and female mice in the 7500 or 30,000 ppm group. Eosinophilic granular cytoplasm of hepatocytes, sinusoidal dilatation, hemosiderin deposition, extramedullary hematopoiesis and necrosis of hepatocytes were observed in the liver of male and female mice in the 30,000 ppm group. Hemosiderin deposition was increased in the kidney of male and female mice in the 30,000 ppm group. Administration of CIPC in diet for 13 weeks caused methemoglobinemia and splenomegaly in ICR mice.

Clinicopathologic study associated with long-term survival in Japanese patients with node-negative breast cancer.

This study was undertaken to determine the absolute and relative value of blood vessel invasion (BVI) using both factor VIII-related antigen and elastica van Gieson staining, proliferating cell nuclear antigen (PCNA), p53, c-erbB-2, and conventional prognostic factors in predicting relapse-free survival (RFS) and overall survival (OS) rates associated with long-term survival in Japanese patients with node-negative breast cancer. Two hundred patients with histological node-negative breast cancer were studied. We investigated nine clinicopathological factors, including PCNA, p53, c-erbB-2 using permanent-section immunohistochemistry, clinical tumour size (T), histological grade (HG), mitotic index (MI), tumour necrosis (TN), lymphatic vessel invasion (LVI) and BVI, followed for a median of 10 years (range 1-20). Twenty-one patients (10.5%) had recurrence and 15 patients (7.5%) died of breast cancer. Univariate analysis showed that BVI, PCNA, T, HG, MI, p53, c-erbB-2 and LVI were significantly predictive of 20-year RFS or OS. Multivariate analysis showed that BVI (P = 0.0159, P = 0.0368), proliferating cell nuclear antigen (PCNA) (P = 0.0165, P = 0.0001), and T (P = 0.0190, P = 0.0399) were significantly independent prognostic factors for RFS or OS respectively. BVI, PCNA and T were independent prognostic indicators for RFS or OS in Japanese patients with node-negative breast cancer and are useful in selecting high-risk patients who may be eligible to receive strong adjuvant therapies.

Independent prognostic value of serum hepatocyte growth factor in bladder cancer.

PURPOSE: We retrospectively investigated whether the level of serum hepatocyte growth factor could predict the prognosis and extent of transitional-cell carcinoma of the urinary bladder. PATIENTS AND METHODS: Serum samples were collected from 113 patients with bladder cancer and from 200 healthy controls. Of the 113 patients, 59 had superficial bladder cancer and 54 had muscle-invasive cancer. Thirteen bladder cancer tissues (eight superficial and five muscle-invasive) were also collected. The levels of hepatocyte growth factor in the serum and tissues of these individuals were measured by enzyme-linked immunoadsorbent assay using hepatocyte growth factor antibodies. RESULTS: The levels of hepatocyte growth factor in the serum and tissues of patients with muscle-invasive cancer were significantly higher than those of patients with superficial bladder cancer (P <.0001 and P =.0054, respectively). The degree of elevation above the normal level of serum hepatocyte growth factor of the former (61.1%) was significantly higher than that of the latter (8.4%; P <.0001). The elevation was highest in patients with visceral metastasis (93.3%). Among patients with superficial bladder cancer, the overall survival rate of those with low levels of serum hepatocyte growth factor was significantly greater than that of those with high levels (P =.005). Among patients with minimally invasive bladder cancer, the disease-free and overall survival rates of those with high levels of serum hepatocyte growth factor were significantly lower than the same rates of those with low levels (P <.001 and P =.0028, respectively). CONCLUSION: Our study suggests that the level of hepatocyte growth factor in serum could be a predictor of patient survival and extent of bladder cancer.

The relation between anxiety and depressive symptoms in normal subjects and patients with anxiety and/or mood disorders.

OBJECTIVE: We investigated the associations between anxiety and depressive symptoms in normal subjects and patients with mood and/or anxiety disorders, using the Japaneses version of Spielberger's STAI and the Zung SDS. METHODS: The subjects for the present study were 60 normal subjects, 15 patients with anxiety disorders and, 12 patients with mood disorders meeting the DSM-IV diagnostic criteria. Informed consent was obtained from all subjects. RESULTS: 1) Both the mean total state-anxiety (S-anxiety) and trait-anxiety (T-anxiety) scores were significantly higher in patients with anxiety disorders and mood disorders than in normal subjects. 2) The mean total SDS score was significantly higher in patients with anxiety disorders and mood disorders than in normal subjects. 3) In normal subjects, there were significant positive correlations between the total T- and S-anxiety scores and total SDS scores. 4) In patients with anxiety disorders, there were significant positive correlations between the total T- and S-anxiety scores and total SDS scores. 5) In patients with mood disorders, there were nonsignificant positive correlations between the total T- and S-anxiety scores and total SDS scores. CONCLUSION: Our results might provide support for the existence of common underlying mechanisms to anxiety and depression.

Epitope analysis of myeloperoxidase (MPO) specific anti-neutrophil cytoplasmic autoantibodies (ANCA) in MPO-ANCA-associated glomerulonephritis.

AIM AND METHODS: Epitope analysis of sera from 20 patients with myeloperoxidase anti-neutrophil cytoplasmic antibody- (MPO-ANCA) associated glomerulonephritis was examined by Western blotting using a panel set of recombinant deletion mutants of MPO. Sera from 19 patients reacted with recombinants of MPO heavy chain, whereas no serum reacted with the light chain regions. The high frequency sites were regions on the upstream of Met341 (Ha region), on the upstream of Met409 (Hb region) near the N-terminus of the MPO heavy chain and a region on the downstream of Gly598 (Hg region) near the C-terminus. The epitope recognition profiles were classified into 2 groups. Group A, which had 1 or 2 regions in Ha, Hb and Hg, and group B, which had all 3 regions. RESULTS: Incidence of alveolar hemorrhage (AH) and pulmonary fibrosis (PF) in group A was significantly higher than that in group B (AH: group A 9 of 13 (69.2%), group B 1 of 6 (16.7%) p < 0.05, PF: group A 10 of 13 (76.9%), group B 1 of 6 (16.7%) p < 0.05, AH and/or PH: group A 12 of 13 (92.3%) and group B 1 of 6 (16.7%) p < 0.01). Relapse rate for patients in the inactive stage in group A was significantly higher than that in group B (p < 0.05). T-cell reacted regions were Ha, Hb, Hg and the light chain of MPO recombinant fragments. Higher frequency of HLA typing with MHC class II DR9 was observed. CONCLUSION: These results indicate that MPO-ANCA recognizes the linear site of the heavy chain of the MPO molecule. The epitope recognition profiles are related to the clinical features, suggesting the pathogenesis of MPO-ANCA-associated glomerulonephritis.
