Bridging a gap in oesophageal atresia using Rehbein's technique: dilatation of a thread canal.

During the last 12 years, operations were performed on 144 infants with oesophageal atresia. In 18 of them who were suffering from an atresia with long gap between segments, perlon threads were laid surgically. Continuity of the oesophagus was attempted using Rehbein's technique. The newborn had type II and IIIb atresia according to Vogts' classification. 11 of these 18 infants survived the third week of life. The fibroepithelial canal that had developed along the perlon thread, was dilated in steps in 6. The clinical course and results are reported.

[Omphalocele and gastroschisis: problems in intensive medical treatment].

Aetiologically and pathognomically, omphalocele and gastroschisis are different forms of occlusive disorders in the anterior abdominal wall of newborns. Their prognosis has clearly improved in recent years, owing to progress in surgical and conservative treatment, primarily on account of improved intensive medical care of the children concerned. Courses and survival rates of newborns with either malformation have proved to depend strongly on the initial condition of the patients. The authors' own studies, in agreement with most of the literature, have shown that the incidence of severe syndromes of malformation has been comparatively high among newborns with omphalocele, whereas children with gastroschisis are usually small-for-date newborns with low birth weight. This has been a frequent limitation to successful treatment. Major importance, in terms of intensive medical treatment, should be attributed to postoperative completely parenteral feeding, throughout the early postsurgical phase during which the patient is particularly endangered from recurrent disorders of passage, as well as to artificial respiration due to respiratory constraint on account of elevated diaphragm and disproportionality between the abdominal cavity and retroposed viscera. Early administration of amino acids, carbohydrates, fat, trace elements, immunoglobulins, and vitamins, with doses being systematically increased by a prespecified pattern, has proved to be necessary to counteract catabolism accompanied by somewhat severe disorders in wound healing, subacute obstruction, and massive attacks of septic infections. This is possible only through central venous catheterization. Resulting thrombotic or septic complications may be mitigated by partial heparinization of the children.(ABSTRACT TRUNCATED AT 250 WORDS)

Anastomotic leak and recurrent fistula following operation of oesophageal atresia.

Experiences from 14 disruptions of oesophageal sutures and with 8 cases of recurrent fistula from 1973 to 1982 are described. Healing of an early oesophageal insufficiency could be achieved by conservative means in 3 newborns. Conservative therapy seems to be possible if life-threatening clinical conditions are reduced effectively. 6 oesophago-pulmonary and recurrent oesophago-tracheal fistulas were closed operatively, two cases closed spontaneously.

Degradation of biogenic oligosaccharides by beta-N-acetyl-glucosaminidase secreted by Entamoeba histolytica.

beta-N-Acetylglucosaminidase secreted by Entamoeba histolytica was extracted from the growth medium by affinity chromatography on CH-Sepharose 4 B coupled to p-aminophenyl-1-thio-beta-2-acetamido-2-deoxyglucopyranoside. The enzyme was further purified by isoelectric focusing, by sequential chromatography on DEAE-cellulose and Sephadex G-150, and by preparative disc gel electrophoresis. Chitobiose (betaGlcNAc1-4GlcNAc) derived from chitin as well as the oligosaccharides betaGlcNAc1-4 betaGlcUA1-3GlcNAc, betaGlcNAc1-4 betaGlcUA1-3 betaGlcAc1-4GlcUA, and betaGlcNAc1-4 betaGlc-UA1-3 betaGlcNAc1-4 betaGlcUA1-3 betaGlcNAc1-4GlcUA derived from hyaluronic acid were tested as potential physiological substrates. All these oligosaccharides are susceptible to action of beta-N-acetylglucosaminidase from E. histolytica. Under identical conditions chitobiose is cleaved 38-48 times faster than hyhyauronate oligosaccharides. No release of N-acetylglucosamine was observed when glycopeptides from ovalbumin were used as substrate. The pH optimum of hydrolase activity was 4.5 when chitobiose was used as substrate. Optimal hydrolysis of aluronate oligosaccharides was observed at pH 3.0 for trisaccharide and pH 2.0 for tetra- and hexasaccharide, respectively. Estimation of molecular weight by means of gel filtration gave values of 75 000. The isoelectric point was 5.02 beta-N-Acetylglucosaminidase from E. histolytica does not act on macromolecular chitin and hyaluronic acid.

[Therapy of alcohol withdrawal delirium with a new hypnotic].

The treatment of alcohol withdrawal symptoms which suddenly appear after an operation, is discussed. Therapy with etomidate as long term sedation is described. 500 mg of etomidate (125 mg circumflex or equal to 1 ml) soluted in 50 ml physiological saline was given by perfusor (0.6 - 0.8 mg/kg b.w./h). Therapy-time lasted from 48 - 56 hours. The influence on respiration, circulation and laboratory report was investigated. We discuss the comparison in the advantage and disadvantage of this therapy to other medications.

Shape of proteins S3 and S17 from the small subunit of Escherichia coli ribosome.

The shapes of proteins S3 and S17 purified from the 30 S subunit of Escherichia coli A19 were studied by hydrodynamic methods. The proteins have s020,w values of 2.1 +/- 0.1 S and 1.2 +/- 0.1 S and D020,w values of 7.4 +/- 0.5 . 10(-7) cm2/s and 11.4 +/- 0.6 . 10(-7) cm2/s. The respective molecular weights determined by sedimentation equilibrium are 25 800 +/- 500 and 9900 +/- 300. The intrinsic viscosity values for the two proteins are 5.8 +/- 0.3 ml/g and 4.2 +/- 0.2 ml/g. From these hydrodynamic parameters a slightly elongated shape for S3 and a globular shape for S17 have been concluded.

Shapes of proteins L1, L9, L25, and L30 from the 50S subunit of the Escherichia coli ribosome, determined by hydrodynamic studies.

Proteins L1, L9, L25, and L30, purified by a nondenaturing method from the 50S ribosomal subunit of Escherichia coli A19, have been characterized. The four proteins were studied under conditions which resemble those used for reconstitution experiments. These proteins have S020,W values of 2.0 S, 1.8 S, 1.8 S, and 1.0 S and D20,W values of 8.4 X 10(-7), 9.0 X 10(-7), 14.0 X 10(-7), and 15.0 X 10(-7) cm2/S. Apparent specific volumes at 20 degrees C are 0.738, 0.733, 0.700, and 0.735 mL/g for the four proteins. The respective molecular weights determined by sedimentation equilibrium are 25 000, 17 300, 12 000, and 6500. The intrinsic viscosity values for the four proteins are 4.0, 5.5, 3.6, and 3.2 mL/g. From these hydrodynamic parameters L1 and L9 appear to have globular or at most only slightly elongated shapes, whereas L25 and L30 appear to be definitely globular.

[The problem of early diagnosis in discrete peripheral nerve lesions. Comparative study of F-wave latency and conventional motor nerve conduction velocity in healthy persons and diabetics (author's transl)].

In 71 healthy subjects and in 38 diabetics the shortest F-wave latencies were evaluated for the n.medianus, n.ulnaris, n.tibialis and n.peronaeus. The motor nerve conduction velocities calculated hereby for the upper and lower extremities were compared with the corresponding distal motor conduction velocities measured in the conventional manner. The comparison of the 2 differently defined motor conduction velocities in healthy and diabetic subjects yielded a better separation between the normal and pathological range in all 4 nerves with the method based on the F-wave latency. For the group of healthy subjects the statistical basis of a preliminary normal range of the F-wave latency was established and nomograms were provided on the basis of the 99% confidence limits which--knowing the length of the arm or leg--open the possibility to derive the corresponding motor conduction velocity.

Geometry of the protein S4 from Escherichia coli ribosomes.

The shape of protein S4 from Escherichia coli ribosomes in solution was determined by hydrodynamic methods and low-angle X-ray scattering. The molecular weight of 24000 determined by low-angle X-ray scattering is within 3% of that found by sedimentation equilibrium analysis and 8% of that determined by amino acid sequence work. The radius of gyration of 3.36 nm, the radius of gyration of the cross section of 0.41 nm and the hydrodynamic studies revealed that protein S4 is not spherical, but rather has a markedly extended shape. Calculations of different conformations, e.g. random coil, based on the parameters evaluated from hydrodynamic methods, revealed a rod-like structure of S4 with a length of 14 nm and a diameter of 1 nm. This is supported by a model of an equivalent scattering particle of uniform density based on all parameters obtained in this study.

[Surgical technique for institution and plastic closure of the tracheostoma. Ist communication].

Within the frame of intensive care we do no longer use the conventional tracheostomy but the so called "sewed-up tracheostomy". Between January 1971 until November 1973 31 children of all age-groups were treated by that method. Special care for patients with sewed-up tracheostoma is obviously facilitated. The replacement of cannulas has become easy. Plastic closure after decannulation offers good functional and cosmetic results. Surgical techniques, indications and criteria for institution and closure of the tracheostomas are described and 31 cases are evaluated. Complications occuring during the period of tracheostoma and subsequent to the plastic closure as well as the general fate of the children are reported.

[Laryngotracheoscopic findings before and after closure of a sewed-up tracheostoma in children with special regards to functional results. IIIrd communication (author's transl)].

13 children of all age groups, suffering from a respiratory insufficiency, and treated with a sewed-up-tracheostoma, were examined by tracheoscopies in regular intervals. The patients were followed up for at least two years and six months. 46 bronchoscopies were performed. Very quick epithelization of the transplantate with two typical forms of the transplantate could be found: development of a pad, and a prolaps of the transplantate. The development of a pad could be considered as the more favourable form with regard to the pulmonary function. The bronchological and roentgenological follow-up-examinations revealed good functional results in all cases except one.
