In a retrospective study of 162 cases of rheumatoid arthritis we found that 24 patients (14.8%) had presented with microscopic haematuria with or without proteinuria. Renal biopsy had been performed in 15 of these 24 patients. Apart from the classical lesions of extramembranous glomerulonephritis, amyloidosis and interstitial nephritis, 60% of histological results showed lesions of mesangial glomerulonephritis. These lesions seemed to be independent of maintenance treatments, but they might have been facilitated by the chronic inflammation kept going by the rheumatoid disease itself.

The authors report the main data of a series collected on a rheumatology unit where the bony signs were most obvious. Survival was comparable with that of large series in the literature. The prognosis seems to be linked to numerous factors therapeutic and etiologic (age, sex, type of monoclonal immunoglobulin) visceral and blood involvement. The diagnosis is now made earlier, but the differential diagnosis includes the other monoclonal immunoglobulin gammapathies.

The authors report the results of MRI performed in 20 patients presenting clinical symptoms of subacromial pathology. The MRI results are correlated to the results of arthrography or arthro CT scan and also to surgical results. MRI is an excellent technique if a precise protocol with T2 weighted images is respected in order to visualize tears or minor perforations of the supraspinatus muscle.

The authors report 6 cases of carcinoma of the ovary presenting during pregnancy in the Department of Gynecology and Obstetrics of La Grave University Hospital, Toulouse, between 1972 and 1994. Analysis of this series is followed by a review of the literature which reveals that carcinoma of the ovary fortunately remains a rare event during pregnancy. All reported series conclude that the diagnosis is made at an early stage. Management depends in great part on the age of the pregnancy and stage of malignancy. During the first 6 months, immediate surgery is essential, with sacrifice of the pregnancy, except at stage IA1. During the final 3 months, surgery is delayed until fetal maturity. Some teams feel that conservative treatment followed by chemotherapy is possible, thereby preserving the pregnancy. The choice of the couple must always be taken into account when making these management decisions.
