[Legal aspects of failure in dentistry].

Conflicts between dentist and patient are increasing in a disquieting way. The reasons for this should be discovered. It seems indispensable to modify the attitude of the practitioner towards his patient, because the latter may have justified claims. To ignore them would mean to cause more trouble, indeed to lead water on the mill of those who would eliminate our liberal profession. Let us not forget that for many people, the practice of liberty is an insupportable challenge. It will be the attitude and psychological feeling which may enable us to avoid that state control of the whole profession.

[Sensitivity of bacteria causing urinary tract infections towards terizidon (author's transl)].

In agar diffusion tests 2603 bacterial strains of species known to cause urinary tract infections were tested routinely in regard to their sensitivity towards Terizidon, a derivative of cycloserine. In order to relate the results which were obtained in terms of the diameter of the inhibiton zone, to the minimal inhibitory concentrations (MIC), 304 of these strains were tested additionally in agar dilution tests. The MICs of the other strains were estimated from the results of these tests. Since it is known that after the oral administration of 500 mg Terizidon the urine contains, on average, more than 128 mug/ml Terizidon for 12 hours and longer, it may be concluded from the results of this investigation that 75% of the strains of E. coli and Citrobacter, 45% of enterobacter, 40% of Proteus mirabilis and enterococci, 35% of the indole-positive Proteus strains and 30% of Pseudomonas aeruginosa would have been successfully attacked by Terizidon in the case or urinary tract infections. By contrast, Klebsiella must be reagarded as being completely resistant to this antibiotic. It follows that the administration of Terizidon is mainly indicated in the treatment of acute urinary tract infections with E. coli as the predominant causative agent.

Studies of the neutropenia in kala-azar: results in two patients.

Two patients with kala-azar were studied, one with DF32P (diisopropylfluorophosphate) and one with 51CR (chromate), in an attempt to elucidate the mechanisms producing neutropenia in this disease. The granulocyte half-life was found to be reduced in both patients, with pooling and probable destruction occurring in the spleen and, to a lesser extent, in the liver. Bone marrow neutrophil reserve, estimated by the response to intravenous hydrocortisone hemisuccinate, was found to be markedly reduced in both patients. An enlarged marginal granulocyte pool in one patient indicated that the neutropenia may also be due to altered intravascular granulocyte distribution.

The structure of symplasmic early oocytes and their enveloping sheath cells in the polychaete, Platynereis dumerilii.

1. Early oocytes of Platynereis dumerilii are found in clusters floating in the coelom. The oocytes of a cluster form a syncytium which is enveloped by several sheath cells. 2. At stage 2, only a single sheath cell per cluster remains, and it penetrates the group of rounded oocytes, enveloping each one of them. At stage 4, this cell contains a reticular basket made up of bundles of filaments and is inferred to be degenerating, from the presence of vacuoles, clumps of pigment-like material, and atypical mitochondria. 3. Synaptonemal complexes are typical of the nuclei of stage 2 oocytes. Oocytes of stage 4 (early vitellogenesis) contain stacks of endoplasmic reticulum in a distinctive arrangement, with interspersed electron-dense masses. Similar masses accumulate in the cytoplasm close to the nucleus and adjacent to the nuclear pores. 4. From the present observations, a physical supporting rather than a nutritive function is attributed to the sheath cell, which ensures cohesion among the oocytes connected with each other throughout the cluster phase by cytoplasmic bridges. This finding is discussed with respect to conclusions drawn from oocyte transplantation experiments.

Early clinical experiences with a heart assist device.

A method of left heart (left atrium-aorta) support after open heart surgery is described. Thoracic reentry is not required when support is terminated. The system has been employed in 10 patients, 4 of whom are long-term survivors.

Experimental studies on human health effects of air pollutants: I. Design considerations.

Because of the possible threat to public health posed by photochemical air pollution, a need exists for experimental studies of short-term respiratory effects of air pollutant exposure in humans. Such studies require rigorous control and comprehensive documentation of the experimental air environment and exposure conditions to ensure that results are both reliable and relevant to public health questions. In addition to biochemical, behavioral, and clinical evaluations, comprehensive pulmonary testing is required to assure that effects at different levels of the respiratory tract are detected. An experimental design based on these principles is described. Studies using this design have shown a wide range of sensitivity to the pollutant ozone and important adverse health effects in sensitive individuals under exposure conditions similar to those experienced during ambient pollution episodes.
