Regulation of type I collagen mRNA translation by TGF-beta.

We found that TGF-beta caused a sustained increase in type I collagen production up to 48 hr after addition to human lung fibroblast cultures. Northern analysis demonstrated that although TGF-beta increased alpha 1(I) mRNA levels 4-fold at 24 hr and 3-4-fold at 48 hr after addition to cultures, there were minimal or no effects on alpha 2(I) mRNA levels at these time points. In vitro translation of RNA derived from TGF-beta-stimulated cells yielded a 2-3-fold increase in the amount of alpha 2(I) peptide when compared with the in vitro translation of RNA from unstimulated cells. Taken together, these studies showed that TGF-beta increased the translatability of the alpha 2(I) transcript. To examine whether increased translatability of the alpha 2(I) transcript resulted from structural changes in the 5' and 3' untranslated regions, primer extension and S1 nuclease protection assays were employed. These studies demonstrated no gross structural changes in the untranslated regions of the alpha 2(I) transcript induced by TGF-beta-stimulation. Overall, our results suggest the presence of a TGF-beta-regulatable factor which controls translation of type I collagen mRNAs.

Compliance with CAPD prescription is good.

OBJECTIVE: To determine compliance with continuous ambulatory peritoneal dialysis (CAPD) prescription. DESIGN: In the first part of the study, we examined the deliveries of dialysate to our CAPD patients (n = 67) between 1992 and 1995 and compared these to the CAPD prescription. In the second part of the study, we performed two inventory checks in a cohort of patients (n = 26) in one urban Canadian center. Compliance was defined as using more than 90% of prescribed dialysate. RESULTS: Based on dialysate deliveries, 95% of our CAPD patients were compliant. Based on home inventory checks, 12% of patients were noncompliant. CONCLUSIONS: Compliance with CAPD prescription is remarkably good in our patient population. These data contrast with American data and could partially explain differences in mortality rates for CAPD patients between the two countries.

The cystic vein: the significance of a forgotten anatomic landmark.

The cystic vein, a portion of biliary anatomy whose insignificance in open gallbladder surgery led to its being relegated to mythology, has been rediscovered by the magnified view of laparoscopic surgery. Its presence is an important anatomic feature that helps distinguish between cystic duct and common hepatic duct, thus diminishing the risk of inadvertent bile duct injury during laparoscopic cholecystectomy.

Laparoscopically assisted surgery for colonic perforation with peritonitis--a case report.

Elective laparoscopic colonic surgery is increasingly recognized as feasible and perhaps preferential. A case of laparoscopically assisted surgery for trauma to the rectum with bacterial peritonitis is presented. It presents an example of the application of this modality to the treatment of iatrogenic colon perforations and perhaps selected diverticulitis.
