Effects of in vitro addition of infectious pancreatic necrosis virus (IPNV) on rainbow trout Oncorhynchus mykiss leucocyte responses.

Infectious pancreatic necrosis virus (IPNV) did not replicate in rainbow trout macrophages cultured in vitro, but a limited increase in viral titer was observed when total leucocytes were infected with the virus. Macrophages did not respond directly to IPNV, in that IPNV did not trigger the respiratory burst of these cells, even when the macrophages were previously stimulated. Supernatants were generated from leucocytes and macrophages exposed to the virus, using a variety of incubation periods and viral titers, to investigate whether virus stimulation induced the secretion of any immunomodulatory substances from these cells. No effect on macrophage responses was detected when these supernatants were added to new macrophage monolayers. However, IPNV produced a significant reduction of LPS-induced B cell proliferation, in a dose-dependent manner, indicative of at least one immunosuppressive effect induced by this virus.

Comparison of different procedures for serotyping aquatic birnavirus.

The current classification of aquatic birnaviruses is based on seroneutralization assays with polyclonal antibodies. In this study a comparison of several procedures used for serotyping aquatic birnaviruses was made with 10 virus strains (4 reference strains from salmonids and 6 birnaviruses isolated from turbot [Scophthalmus maximus]). The relationships among the birnavirus strains were studied by seroneutralization assay with polyclonal antibodies and by immunodot assay with both polyclonal and monoclonal antibodies. The results were compared with a presumptive classification obtained from analysis of restriction enzyme patterns of cDNA products obtained by PCR amplification. No correlation was found among the results obtained by the different procedures. The seroneutralization and the immunodot assays with polyclonal antibodies were not useful in classifying these birnaviruses strains; however, patterns of reaction with monoclonal antibodies emphasized the individuality of the strains, particularly in the case of two strains (231 and 460) whose patterns did not correspond to established serotypes. The application of PCR and restriction enzyme analysis is a promising system for approaching the classification of this viral group on the basis of genomic differences and similarities. The variable results obtained in this comparison lead us to think that the current classification of aquatic birnavirus may not be the most accurate and there is a need for modification incorporating recent isolates, not only from salmonid species but also from marine fish.

The effects of culture-site, depth, season, and stock source on the prevalence of Marteilia refringens in cultured mussels (Mytilus galloprovincialis Lmk.) from Galicia, Spain.

Samples of a marteiliad parasite in Mytilus galloprovincialis Lmk. from NW Spain were identified as Marteilia refringens, using light and electron microscopy. The most important lesions in the mussel tissues were found in the digestive tubules at the moment of the release of the sporangia, resulting in a breakage of the epithelial cells of the digestive tubules. The plasmodia of a Marteilia-like organism were also found in the gills, accompanied by a severe host reaction in 1 of the 4,200 examined mussels. The prevalence of the M. refringens parasite was monitored for 4 yr and correlation was found with prevalence, site, and depth of mussel culture. No correlation was found with geographical origin of the mussels. Mussels collected from the inner part of the Ría de Vigo were more frequently infected (38.5%) than those from the middle (29.0%) and outer parts (5.5%). Mussels from 2-m samples showed a higher mean prevalence of M. refringens than those from the 5-m samples (13.3 and 8.3%, respectively).

Application of 3 techniques for diagnosing birnavirus infection in turbot.

Three procedures for avoiding viral amplification by cell culture were evaluated for the diagnosis of birnavirus infections of turbot (Scophthalmus maximus) tissues. Immunodot assay using monoclonal and polyclonal antibodies was not satisfactory as a detection system because of false positive reactions. Although immunofluorescent assay of liver and kidney smears was an adequate method for a rapid diagnosis of the infection, blood smears from infected fish did not show higher levels of fluorescence than those from uninfected fish. However, the detection of birnavirus by polymerase chain reaction (PCR) amplification directly in fish tissues was successful and seems to be a promising system for the diagnosis of fish birnavirus.

Amoebic gill infection of turbot, Scophthalmus maximus.

Amoebae were found to cause severe gill tissue damage in turbot, Scophthalmus maximus L, from a grow-out facility in northwestern Galicia (Spain). The nature and extent of lesions along with negative results of bacteriological and virological examination made this agent responsible for mortalities in four turbot stocks supplied with water from a single source. We present our findings, although we failed to isolate amoebae, since there was a clear evidence of their primary role in the development of disease condition and occurrence of mortalities. In addition, this is a record both of a new host endangered by amoebae in intensive cultures and pathogenesis of the gill lesions.

Spontaneous reporting of adverse drug reactions to non-steroidal anti-inflammatory drugs. A report from the Spanish System of Pharmacovigilance, including an early analysis of topical and enteric-coated formulations.

Non-steroidal anti-inflammatory drugs (NSAIDs) are the third most commonly prescribed group of drugs in Spain. We present here the profile of adverse drug reactions (ADRs) attributed to them and reported to the Spanish System of Pharmacovigilance (SSPV) between 1983 and 1991, together with a preliminary analysis of topical, slow-release (SR) and enteric-coated (EC) preparations. Out of 18,348 reports of ADRs included in the SSPV database, 1609 (8.8%) implicated an NSAID. NSAIDs ranked second after antibiotics (15.1% of all reports) among the most commonly implicated drugs. Half of the patients were more than 55 years old, and 60% were women. Diclofenac (364 reports), piroxicam (282), indomethacin (197), naproxen (155), and ketoprofen (137) were the most commonly implicated NSAIDs in reports of ADRs. The most commonly reported ADRs were gastrointestinal (39%), cutaneous (20%), and those affecting the central and peripheral nervous system (9%). Seven reactions had a fatal outcome, and 138 were considered life threatening. Forty-nine reports included previously undescribed ADRs. There were 98 reports describing ADRs attributed to topical NSAIDs; 5 of these described 11 general reactions, such as duodenal ulcer, gastrointestinal bleeding, diarrhoea, dyspnoea, facial oedema, aggravation of bronchospasm, and angioedema. One hundred and sixty-eight reports referred to SR and EC preparations. The ratio of gastrointestinal to non-gastrointestinal reactions to SR-EC diclofenac was higher in the case of SR-EC diclofenac than in the case of plain diclofenac (P = 0.037); similarly, the ratio of CNS to non-CNS reactions to SR-EC indomethacin was also higher than the corresponding ratio with plain indomethacin (P = 0.002).(ABSTRACT TRUNCATED AT 250 WORDS)

Salmeterol-induced vertigo.

Dizziness, together with hypotension and faintness, are well-known adverse drug reactions of salbutamol that have been related to its cardiovascular effects. However, we could find no published reference to vertigo in patients taking salmeterol, in the absence of cardiovascular alterations. We present the case of a woman who experienced four independent episodes of vertigo lasting 36 h each, following four inhalations of salmeterol several months apart. The close temporal relationship between the inhalation of salmeterol and the onset of symptoms, as well as the positive re-exposures, reinforce this alleged association.

[Use of benzodiazepines at a general hospital].

Anxiety and insomnia are frequent troubles among hospitalized patients and are often related to the underlying diseases, their diagnosis and treatment or to other conditions of the hospital environment. The primary therapeutic strategy consists of modifying such circumstances. Benzodiazepines may be useful in some patients but patterns of hospital use have been scarcely evaluated. We have studied the use of benzodiazepines in medical and surgical departments of our hospital for a 2-month period. From a total of 754 admitted patients, 116 (15.4%) received benzodiazepines orally, presumably for anxiety and insomnia. The proportion of treated patients differed remarkably oscillating between 4% and 58%. Mean daily doses of benzodiazepines were lower than defined daily doses. The schedules were remarkably rigid: 1) in none of the cases the initial dose nor therapeutic schedule were changed, 2) in most cases, schedules in the elderly were the same as in younger patients, and 3) in 80% of cases, the treatment was maintained till the patient was discharged. The present study emphasizes the necessity to modify overall strategy of the use of benzodiazepines.

Application of fast atom bombardment and tandem mass spectrometry to the identification of organic micropollutants in water.

Organic compounds in raw and drinking water, not amenable to gas chromatography/mass spectrometry, were characterized by fast atom bombardment (FAB) and collision-induced decomposition/mass-analysed ion kinetic energy (CID/MIKE) spectrometry. Anionic, cationic and non-ionic surfactants were successfully identified by comparison of their FAB spectrum and CID/MIKE of prominent ions with commercial standards. Organic dyes were spiked to water extracts and the detection limit for some of them reported.
