Alpha 1-antitrypsin phenotypes in cirrhosis and hepatoma.

The phenotypic distribution of alpha-1-antitrypsin variants has been studied in 101 patients with cirrhosis and in 50 with cirrhosis plus hepatoma. 504 healthy Greeks served as controls. The ZZ and MZ phenotypes were found only once in the group of cirrhosis. The very low PiZ gene frequency suggests that the association of PiZ gene with cirrhosis is fortuitous. The FM phenotype has been observed in 14% of patients with hepatoma arised on cirrhosis and this incidence differed significantly between the two groups of patients and the controls. It is possible that cirrhotic patients phenotypically FM develop for as yet unknown reasons hepatoma in high percentage.

Distribution of Gm and Inv factors in two samples of the Greek population.

Gm and Inv polymorphism has been investigated on a sample of the general Greek population (N = 256) and on a sample of the Achaia population of Northwestern Peloponnusus (N = 150). The estimated frequencies were, respectively in the two groups: Gm1, 0.156 vs. 0.180; Gm1,2,0.010 vs. 0.017; Gm4,12, 0.834 vs. 0.803; Inv (+1), 0.125 vs. 0.140.

Serum protein markers (Hp, GC, C3) in patients with colon cancer.

The phenotypes and gene frequencies of three serum protein systems--Hp, GC and C3--were studied in 184 consecutive patients from all over Greece with colon cancer. Healthy Greeks studied previously in our department served as controls. No significant differences were found between patients and controls concerning GC and C3. Significant differences were found in the Hp system; the frequencies of the Hp*1 gene and the Hp 1-1 phenotype were significantly higher in patients than in controls.

GC and C3 serum groups in ulcerative colitis.

The phenotypes and allele frequencies of two serum protein systems (GC and C3) were studied in 91 consecutive patients with ulcerative colitis and compared with healthy controls. No significant differences were found as far as GC was concerned. However, significant differences were observed in C3: the C3*F allele and C3FS phenotype were more frequent in patients than in controls.

Cisapride, metoclopramide, and ranitidine in the treatment of severe nonulcer dyspepsia.

Cisapride, a prokinetic drug with a novel mechanism of action, was compared with another prokinetic drug, metoclopramide, and an H2-blocker, ranitidine, in the treatment of nonulcer dyspepsia. In a double-blind study, 60 patients with severe dyspeptic symptoms received cisapride 5 mg TID, metoclopramide 10 mg TID, or ranitidine 150 mg BID for 8 weeks. Symptoms were evaluated during treatment and 4 weeks after the end of therapy. All three drugs effectively controlled the symptoms of chronic functional upper gastrointestinal tract disorders. The prokinetic drugs, particularly cisapride, were significantly better than ranitidine in controlling symptoms, especially reflux symptoms. All three drugs were generally well tolerated; cisapride in particular was associated with fewer adverse effects.

Effect of omeprazole on H. pylori after two weeks of treatment.

The "clearing" effect of omeprazole 20 mg/daily on Helicobacter pylori (HP) was studied in 32 HP positive patients with duodenal ulcer (DU). Endoscopy and antral biopsies were made before and after two weeks of treatment with Omeprazole. HLO test and histology after Giemsa stain were used for the detection of HP. Fourteen of 28 (50%) initially HLO positive and 12 of 30 (40%) initially histological HP positive patients became HP negative. Nine of 23 (39%) patients, initially HP positive by both methods became HP negative. "Clearing" of HP in this particular group of patients did not have any influence on ulcer healing. It is concluded that omeprazole used for two weeks has a clearing effect on HP in a high enough percentage of patients with DU.

Complement activation in atherosclerosis: effect of angiography and surgery.

Complement activation in the plasma of patients with severe atherosclerosis (arterial occlusive or aneurysmatic disease) was investigated in this study. The effects of angiography and reconstructive arterial surgery (RAS) were also assessed. Atherosclerosis was not found to be associated with systemic complement activation. Angiography resulted in high levels of C3 breakdown products. Surgery caused a significant degree of systemic complement activation in both patients with atherosclerosis and controls. Post-operative levels of C3 breakdown products were significantly higher in atheromatic patients than in controls, most likely due to the insertion of dacron arterial prostheses in the first group.

Non-cardia gastric adenocarcinoma and Helicobacter pylori infection.

The seroprevalence of IgG antibodies to Helicobacter pylori (Hp-Ab) was studied in 47 patients (29M, 18F, mean age +/- SD: 62.44 +/- 12.63 years) with non-cardia gastric carcinoma using an enzyme-linked immunosorbent assay. Controls were: a) 50 healthy people well-matched with the cancer patients; b) 50 patients with dyspepsia well matched with the cancer patients. Hp-Ab were detected in 72.3% of patients with gastric carcinoma, in 68% of healthy people and in 88% of dyspeptics. No significant associations were found between H. pylori infection and gastric cancer patients as a whole (odds ratio: 1.23, 95% CI: 0.514-2.95). In addition, no significant associations were found between H. pylori infection and the various subsets of cancer patients. These findings do not support an association between H. pylori infection and non-cardia gastric carcinoma in Greece.

A recently recognized entity associated with the treatment of acute promyelocytic leukemia: the retinoic acid syndrome.

All-trans-retinoic acid is an effective induction treatment for acute promyelocytic leukemia. Although retinoic acid is well tolerated by the majority of patients with this disease, a potentially fatal complication of this kind of treatment has been reported: "the retinoic acid syndrome". We describe this syndrome, which occurred in one of our patients and was successfully treated by the administration of dexamethasone.
