Regression formula for estimation of fetal weight with use of abdominal circumference and femur length: a prospective study.

Ultrasonographic fetal measurements from 293 singleton pregnancies were obtained within 7 days of delivery. Biparietal diameter, abdominal circumference, femur length, and actual birth weight data of the first 93 fetuses in the study were used as variables to determine the best mathematical model for relating estimated fetal weight to biparietal diameter, abdominal circumference, and femur length. With the aid of a computer, three regression equations were derived. The best model was Log10 (weight) = 0.77125 + 0.13244 (AC) - 0.12996 (FL) - 1.73588 (AC x AC)/1000 + 3.09212 (FL x AC)/1000 + 2.18984 (FL/AC); (R2 = 0.987). The accuracy of this formula was then compared prospectively, first with the formulas published by Shepard and coworkers, Rose and McCallum, and Hadlock and colleagues in the entire sample of 200 patients, second in 46 large, 101 appropriate, and 53 small for gestational age fetuses, and then in 44 fetuses of pregnancy complicated by diabetes. The difference between actual and estimated birth weights generated by the study formula had no systematic error (Student's t-test, P > 0.05) in cumulative data, and in small or appropriate and large for gestational age fetuses. As this derived formula is very cumbersome to manipulate, tables have been prepared with computer assistance to read the estimated fetal weight directly.

[Use of magnetic resonance in the diagnosis of neuro-cryptococcosis in the acquired immunodeficiency syndrome: study of 4 patients].

Neuro-cryptococcosis is a common opportunistic infection in AIDS or HIV infected patients. From a series of 10 neuro-cryptococcosis the four of them studied by magnetic resonance (MR) are reported. In AIDS patients a high suspicion of opportunistic infection of the CNS is needed as exemplified by two of the four patients who only presented cephalalgia. The other two patients suffered additional symptoms and signs of meningeal and CNS involvement, such as nuchal rigidity, cranial nerve palsies, papilloedema, gait ataxia and dismetria. Diagnosis was achieved (confirmed) by a positive culture, serology or indian ink test in CSF. CT scan did not contribute to the diagnosis and management of the patients. In contrast MR, showed in three of them a peculiar pattern of small, confluent, high-signal lesions, roughly symmetrically placed in the basal ganglia and the internal capsule. They probably correspond to the dilated Virchow-Robin spaces through which torulae migrate from the subarachnoid space.

[Cystadenoma of the extrahepatic bile ducts: a rare cause of cholestasis].

We report the case of a 45-year-old woman who underwent laparotomy for obstructive jaundice due to an uncommon cystic tumor located in the porta hepatis. Histology of the resected specimen disclosed a cystadenoma of the common hepatic duct. A review of 20 reported cases shows that the preoperative diagnosis is uncommon; wide resection appears to be the treatment of choice since these lesions are susceptible of local recurrence and malignant change.

Phase II trial with 5-fluorouracil and high-dose folinic acid, using new sequential dosing schedule, in pretreated advanced breast cancer patients.

In this study, 30 patients with metastatic breast cancer were treated with 5-Fluorouracil (5-FU) and high-dose Folinic acid, using a new sequential dosing schedule. Our treatment consisted of one day i.v. infusion of 500 mg/m2 of Folinic acid over two hours. One hour after the beginning of Folinic acid infusion, 5-FU (500 mg/m2) was given by i.v. bolus injection. The complete and partial response rates achieved (CR+PR) were 21% in a population of patients pretreated with chemotherapy including 5-FU. Cutaneous and bone metastasis responded best to our treatment. There were no treatment related deaths or withdrawals from the study. The drug related toxicities observed in this study were usually mild to moderate and easily controllable. Thus preliminary results of our study suggest that response rate, quality of life and time to disease progression for the responders improved by this sequential treatment.

Operable breast cancer: a review of neoadjuvant chemotherapy.

Neoadjuvant chemotherapy in operable breast carcinoma (Ia-IIb) may influence the disease development reducing recurrence rate. A survey was conducted from 1975 to 1989 in an attempt to evaluate if pre- and peri-operative chemotherapy is able to improve the survival rate. However, available literature shows that only some Authors observed an increased disease free-survival rate among N- patients.

Expression of sucrase-isomaltase and dipeptidylpeptidase IV in human small intestine and colon.

One monoclonal antibody (8A9) against the human sucrase-isomaltase complex and one (4H3) against the human dipeptidylpeptidase IV were produced in the rat and used to immunolabel thin frozen sections of human small intestine and colon. Both enzymes were found to be expressed in the poorly differentiated crypt cells of the small intestine as well as in the mature villous cells, and very low levels were found to be expressed in the colon. Homogeneous immunolabeling of the whole colonic epithelium with the monoclonal antibody 4H3 was often observed, whereas labeling with the monoclonal antibody 8A9, if any, was either restricted to a few crypts and plateaus. The two antibodies were used to perform specific immunoprecipitation of the corresponding antigen, the N-terminal sequence of which was determined after sodium dodecyl sulfate-polyacrylamide gel electrophoresis purification and electroblotting, and were compared with those of other species. In secretor blood group A humans, both the sucrase-isomaltase and the dipeptidylpeptidase IV have type 3 blood group A determinants.

[Anthropometric assessments in dental esthetics].

Various parameters related to the mobility of the upper lip in a sample of 200 patients, have been considered in order to establish the correlation these have with the extent of lip mobility and the relative visibility of their teeth. From this study it was clear that a large number of patients, with a low index of tooth visibility, could benefit, from the viewpoint of their periodontal health, from the application of prosthetic margins above the gums. This would not damage the aesthetic aspect because the margins in question would only be visible with the lips contracted or, in certain patients, never.

[The use of aminoglutethimide in the treatment of metastatic breast cancer].

It has been demonstrated that AG inhibits the peripheric conversion of androstenedione into estrogens and produces an adrenal suppression, equivalent to surgical adrenalectomy. As breast cancer is responsive to hormonal therapy, AG has been used in the treatment of metastatic breast cancer. Since 1984, at I Clinica Ostetrica e Ginecologica of University of Torino, 48 patients with advanced breast cancer, previously treated with chemotherapy and hormonotherapy, have been given AG 1 g/day and hydrocortisone 40 mg/day. Six patients (12%) obtained CR, nine (19%) a PR, eleven (23%) a SD, twenty two (46%) a PD. The efficacy of AG has been evaluated as related to the following prognostic factors: menopausal status, hormone receptors, disease free interval, age, prior tamoxifen exposure. This study shows that bone involvement is the most responsive localization to AG. In conclusion AG is still an effective therapeutic choice in metastatic breast cancer treatment.

Peritonitis due to rupture of retroperitoneal teratoma: computed tomography diagnosis.

Retroperitoneal benign cystic teratoma is relatively rare in adults. We report a case of traumatic rupture of primary retroperitoneal teratoma into peritoneum that produced chemical peritonitis. It was demonstrated by computed tomography, which showed fat-fluid levels in the peritoneum. We suggest that the demonstration of fat-fluid levels in the peritoneum could be a reliable sign of intraperitoneal rupture of abdominal teratoma and subsequent chemical peritonitis.

Expression of a subclass of human blood group A antigenicity in A+ rabbit jejunum: specific glycosylation of the glycocalyx and a 140 K brush border glycoprotein.

A monoclonal antibody (14 A4) raised against human A erythrocytes has been produced. It specifically reacts with a subclass of human blood group A determinants. Whereas all the major secreted and membrane-bound glycoproteins of A+ rabbit jejunum epithelium bear human blood group A-like determinants recognized by anti-A polyclonal serum or a monoclonal antibody with broad specificity (Cl 3.3), expression of the A-subclass recognized by 14 A4 is very restricted. The contents of secretory granules of Paneth cells but not the mucins of goblet cells were labeled by 14 A4. In the enterocytes, glycans recognized by 14 A4 were present in the glycocalyx, on an early expressed 140 K glycoprotein of brush border membranes and also on a glycoconjugate of the basolateral membrane of immature crypt cells. In the jejunal brush border membrane of blood group A secretor humans, only one glycoprotein of molecular weight 140 K bears the A-subclass recognized by 14 A4.
