Atrial natriuretic factor and sympathetic activation in human heart failure.

The neurohormonal changes occur early, have important prognostic value, and may play a role in the evolution and progression of heart failure in man. Atrial natriuretic factor (ANF) is a natriuretic and vasorelaxant peptide. Previous studies indicated that plasma ANF provides prognostic information and, ANF levels closely related to both severity of disease and catecholamine levels but, it is still unclear if high circulating levels of ANF, which are present in heart failure constantly, may be to correlate with sympathetic nervous activity in man. Thus, the aim of the present study was to investigate the relations between the release of ANF and the sympathetic system in human heart failure. We studied 18 patients with heart failure (CHF) and a control Group (n = 14) of healthy subjects. To induce adrenergic activation in physiologic way patients were underwent to a low-exercise by cycle-ergometer in supine position. Blood was collected at rest, and immediately after exercise for determination of plasma levels of ANF, norepinephrine and epinephrine levels. ANF values at rest were 35.9 +/- 19.2 pg/ml in controls and 190.7 +/- 34.2 pg/ml (p < 0.001 vs controls) in CHF patients. As well norepinephrine levels showed higher values in patients (295.7 +/- 47.8 pg/ml), than in normal subjects (143.5 +/- 33.3 pg/ml; p < 0.01). In CHF patients epinephrine levels were 100.1 +/- 21.2 pg/ml (p < 0.01 vs controls). ANF levels were in normal subjects 87.9 +/- 19.2 pg/ml (p < 0.01 vs rest) after exercise. [Furosemide long-term inhalation therapy in patients with nasal polyposis].

Study aimed principally at applying the data recently gathered with regard to Bronchial Reactivity to rhinology, with special regard to non-atopic or allergic hyperresponsiveness. In the past decade, Ultrasonic Nebulized Distilled Water (U.N.D.W.) was recognized as one of the best test in Bronchial Reactivity. In allergic and non-allergic rhinitis, the Authors tried to use U.N.D.W. to reproduce symptoms of rhinitis. Results show that U.N.D.W. could be included in the Nasal Hyperreactivity test. Secondly, but not less interesting, is the prevention of experimental rhinitis with U.N.D.W. by using Furosemide, a diuretic administered by inhalation. The prevention of Ca++ penetration into nasal cell can block any mediator-activation. Such procedure may be applied in polyposis after surgery in order to prevention recurrence, data show the complete applicability of this pharmacological treatment in children as well.

Decreased respiratory tolerance from intermittent asphyxia in rat sucklings.

To study the respiratory tolerance of rat pups to intermittent asphyxia induced by exposure to closed rebreathing, we randomized newborn rats from four litters into two treatment groups. Respiratory tolerance was defined as the time interval during asphyxia until the first episode of 30 seconds of apnea. Rats in the experimental group were asphyxiated once daily during the first 4 days of life. Rats in the control group were asphyxiated once on day 4 of life. Pups exposed to intermittent asphyxia had a significant reduction in respiratory tolerance compared with that of littermates asphyxiated once at the same age. These findings suggest that the ability of the young mammal to withstand re-exposure to asphyxia may be impaired.

[Ultra-low laparoscopic rectal resection and colo-anal anastomosis. Experimental study on swine].

Nowadays oncologic surgery is defining new criteria in the treatment of rectal cancer: preservation of sfincterial function, ultra-low resections with distal margin at only 2 cm distally to the tumor, role of mesorectum as preferential site of lymphatic diffusion, preservation of lombo-aortic and pelvic nerves. Laparoscopy is showing good results in bowel surgery so as previously got on biliary tract: less visceral manipulation, less stimulation of immunologic system, less pain, early resumption of peristalsis and food intake, better recovery, less hospital stay. We experimented on pig ultra-low laparoscopic resection of the rectum, with preservation of sfincterial function, and mechanical anastomosis at the upper edge of the sfincterial ring. The evaluation of surgical technique, post-operative supervision, and follow-up instrumental control (anal manometry, endo-rectal ultrasonography, sfincterial electtromanometry, Rx barium enema) show technical feasibility and confirm a better recovery with regular defecatory function.
