Proliferation kinetics of human endometrium during the normal menstrual cycle.

A radioautographic analysis of the number and distribution of labeled nuclei after in vitro incorporation of radiothymidine was carried out in human endometrium during the normal menstrual cycle. The highest rates of proliferation were recorded on cycle days 8 to 10 in the upper one third of the functionalis layer. Coinciding with intraglandular secretory activity on cycle day 19, the rate of proliferation of gland cells decreased near zero levels and remained unchanged thereafter. Midsecretory phase arteriolar coiling and stromal predecidualization were associated with increased isotope incorporation. DNA synthesis in premenstrual endometrium was limited to the surface epithelium, predecidua, and vascular endothelium. The endometrium of the isthmus and cornual regions and of the basalis zone demonstrated relatively constant and comparatively lower rates of proliferation throughout the cycle. Regional and cellular variations in nucleic acid synthesis in cyclic endometrium are likely to reflect differences in physiologic functions in target cells in response to cyclic hormonal stimuli.

The Kevorkian curette. An appraisal of its effectiveness in endometrial evaluation.

Office endometrial biopsy with the Kevorkian curette was performed in 400 patients presenting with abnormal uterine bleeding or other endometrial cancer-risk indicators. The use of this instrument has proved to be a safe, simple, inexpensive, and highly reliable outpatient procedure with excellent patient acceptance. It has provided tissue for adequate diagnosis in 91.8% of the cases, and the diagnostic accuracy when controlled by D&C and/or total abdominal hysterectomy (TAH) was 96.2%. As a result, 73.5% of the women required no further surgical procedures for either diagnostic or therapeutic purposes. In the postmenopausal age group, adenocarcinoma was diagnosed in 7 of 177 (3.9%) women. This procedure is highly recommended for early office diagnosis of uterine pathology and, in particular, endometrial adenocarcinoma and its presumed precursor lesion, adenomatous hyperplasia.

Extraskeletal myxoid chondrosarcoma with "chordoid" features (chordoid sarcoma).

The light and electron microscopic features of a so-called chordoid sarcoma arising in the area of the gastrocolic ligament are described and illustrated. Despite the tumor's histologic resemblance to chordoma, the ultrastructural markers are those of chondroid rather than chordoid neoplasms. These are well-developed, vesciular granular endoplasmic reticulum, intracytoplasmic glycogen, lipid bodies, and a collagen-poor extracellular matrix with electron-dense granules, presumably representing chondroitin sulfate. Similar submicroscopic alterations have been observed in extraskeletal myxoid chondrosarcomas. Because of the distinctive morphologic appearance of this peculiar lesion, the term "myxoid chondrosarcoma with chordoid features" is suggested to reconcile histologic and ultrastructural features and histogenesis.

[Cytology of metastases of rare tumors in the pleura (author's transl)].

In the Oncocytological Center of Zala Country were investigated in the recent years 2378 cases from effusions of pleural cavity. Ten of the tested cases had been proved to be metastases of sarcomatous lesions. Three of them were myosarcoma, two of them were malignant melanoma and one fibromyosarcoma, reticulosarcoma, osteosarcoma and sarcomatous stage of Hodgkin disease, each. The author gives a review of the cytological characteristics of metastatic sarcomatous lesions. Up to the opinion of the author there are some characteristics which can help in the typing.

Studies on the cytodynamics of experimental endometrial regeneration in the rabbit. I. Historadioautography and ultrastructure.

The kinetic and fine structural characteristics and site of origin of the resurfacing uterine lining, as well as the influence of ovarian hormonal stimuli on the reparative processes of experimentally injured endometrium in the rabbit, were studied by means of in vivo historadioautography and electron microscopy. The first-generation, resurfacing spindle-shaped cells lacked mitosis and expanded by ameboid migration. This was followed by a rapid increase in nuclear deoxyribonucleic acid synthesis, which presumably provided for accelerated mitosis-mediated surface epithelial proliferation and maturation. By Day 3 after injury, endometrial regeneration was completed with a return of labeling index to base-line levels. The newly formed surface lining was derived exclusively from the persistent surface epithelium bordering the denuded regions. The isotope uptake and morphologic alterations of the regenerating endometrium in castrated rabbits which were similar to those of intact rabbits suggest that endometrial repair is not mediated by the hormones of the reproductive system.

Ultrastructural studies on the morphogenesis of psammoma bodies in ovarian serous neoplasia.

Transmission electron microscopic studies have been carried out on psammoma bodies in two benign and seven malignant papillary serous neoplasms of the ovary. Ultrastructurally, psammoma bodies are composed of microcrystals in all respects similar to calcium-phosphate apatite crystals of bone. The formation of psammoma bodies is initiated intracellularly, in both the neoplastic epithelial cells and stromal histiocytes. The initial seeding site of apatite crystals is served by lipid-rich intracellular vesicles. These structures are produced in association with autophagocytosis in the neoplastic epithelial cells and heterophagocytosis of extracellular lipidic material in the stromal histiocytes. Extracellular lipids presumably derive from dehiscent tumor tissue. The close relationship between larger intraepithelial calcific bodies and microfilaments suggests that the latter provide supportive matrix for further intracellular calcification. Large extracellular psammoma bodies result from fused calcific bodies which have been extruded from calcified cells. Mineralization of extracellular collagen fibers is not observed. The results provide supportive evidence to the concept that psammoma bodies in ovarian papillary serous neoplasms and probably in other neoplastic and non-neoplastic conditions are a consequence of dystrophic calcification associated with cellular degeneration.

Scanning and transmission electron microscopic studies of vaginal adenosis and the cervical transformation zone in progeny exposed in utero to diethylstilbestrol.

Much attention has recently been focused on vaginal adenosis and other cervical and vaginal alterations occurring in female offspring exposed to diethylstilbestrol (DES) in utero. These alterations consist of heterotopic glandular epithelium in the vagina, a characteristic cervical transformation zone, and the frequent presence of a cervical collar, or hood. It is believed that these changes occur during embryonic differentiation of the lower genital tract following failure of squamous epithelium to migrate in a cephalad direction to replace columnar epithelium which normally lines the vagina and ectocervix. A number of cases clear-cell carcinoma have occurred in DES-exposed girls who also had adenosis. In an attempt to characterize the cellular populations of these areas of adenosis, we have made an ultrastructural study which is herein reported.

Mucinous tumors of the ovary. II. Ultrastructural features of mucinous cystadenocarcinomas.

On the basis of light microscopic and scanning and transmission ultrastructural observations, ovarian mucinous tumors of borderline malignancy contain two types of mucinous epithelium--an endocervical type and an intestinal type. The fully malignant tumors were composed solely of intestinal-type epithelium. These findings may have prognostic and therapeutic significance.

Studies on the cytodynamics of human endometrial regeneration. II. Transmission electron microscopy and histochemistry.

The morphologic observations made in this study indicate that postmenstrual endometrial resurfacing occurs by proliferation of epithelial cells originating from the stumps of residual basal glands and uterine surface lining, bordering the denuded areas, respectively. Endometrial stromal cells or glands of the residual spongiosa are not involved in uterine surface repair. The mechanism whereby the new surface develops, it is proposed, is by endomitotic division. This contention is supported by the accelerated nuclear DNA and intracytoplasmic RNA synthesis, and absence of mitosis in the newly formed surface epithelial cells. The results indicate, furthermore, that uterine surface reconstruction, which is completed by cycle day 5, is independent of ovarian hormonal stimuli, whereas further endometrial growth, connective tissue remodeling, and metabolic enzyme synthesis are influenced by the gradual increase of preovulatory estrogens.

Studies on the cytodynamics of human endometrial regeneration. I. Scanning electron microscopy.

Stereoscopic morphologic studies on regenerating cyclic endometrium indicate that endometrial repair lasts 48 hours, beginning between cycle Days 2 to 3 and ending between cycle Days 4 to 5. During this period, endometrial repair is focused on the neoformation of surface epithelium and is initiated only when and where the zona basalis is denuded from the overlying secretory spongiosa. The surface epithelium is derived from a simultaneous proliferation of dual origin: first from the exposed ends of basal glands and second from the persistent and intact surface lining of cornual and isthmic regions, bordering the denuded areas. The lack of estrogen-dependent morphologic alterations (mitosis and ciliogenesis) in the uterine mucosa during the regenerative period suggests that the initial factors and cellular processes operating in endometrial reconstruction are purely a reflection of tissue repair and are independent of cyclic hormonal stimuli.

The ultrastructural morphology of gynecologic neoplasms.

Our knowledge of the morphology and pathogenesis of malignant neoplasms of the female genital tract has traditionally depended heavily on their light microscopic characteristics. The introduction of transmission, and most recently, scanning electron microscopy, into the field of gynecologic pathology has resulted in a considerable improvement in the diagnosis of genital cancers that are difficult to classify and has provided valuable information for a better understanding of their subcellular dynamics and pathogenetic development. This paper describes and illustrates the ultrastructural alterations that are considered specific for the most common malignant and potentially malignant neoplasms of the genital system in this context. The value of electron microscopy in the morphologic study of genital cancers warrants its increased use in routine diagnostic pathology, as well as further evaluation of it in basic research in this challenging area of investigation.
